    Notes: Summary Dermatofibrosarcoma protuberans (DFSP) is an unusual cutaneous neoplasm that originates in the dermis. This tumor belongs to the so-called low grade soft tissue sarcomas. The recurrence rate after simple surgical treatment is very high, and initial radical surgical treatment is indicated. In recurrences, the infiltrative growth often leads to invasion of vital structures and may ultimately lead to amputation of extremities. In the case reported, there was an initial misdiagnosis of dermatofibroma; the lesion recurred and was reported as DFSP. It was resected radically, and the defect was covered by a radial forearm flap. The radial forearm flap offers an effective one-stage method of soft tissue reconstruction which allows early mobilization and rehabilitation. After excision of a recurrence on the margin of the free flap 36 months after the first treatment, a deep infiltrating recurrence 12 months later made amputation necessary. The location of DFSP on the plantar aspect of the foot has not been reported previously. A brief review of the literature is presented.

    Description / Table of Contents: Zusammenfassung Das Narbenkarzinom (Marjolin's ulcer) ist eine seltene Erkrankung, die auf dem Boden instabiler Narben, chronischer Fistel, Ulzera und Röntgenschdden entstehen kann. Anhand von 23 eigenen Beobachtungen der Jahre 1976 bis September 1990 wird das Krankheitsbild dargestellt. Im Vergleich mit früheren Veröffentlichungen wird festgestellt, da\ das Karzinom, das auf dem Boden venöser Unterschenkelulzera entsteht, relativ am heufigsten vertreten ist, das Röntgenkarzinom oder Brandnarbenkarzinom hingegen eher in den Hintergrund getreten ist. Insgesamt scheint das Krankheitsbild seltener geworden zu sein. Da der chirurgischen und adjuvanten Therapie analog zur Therapie anderer Karzinome Grenzen gesetzt sind, kommt der Prävention und Früherkennung vordringliche Bedeutung zu. Obwohl der Tumor vornehmlich an der Körperoberfläche entsteht and somit dem diagnostizierenden Auge gut zugänglich ist, finden sich noch immer Geschwülste mit verblüffender Verwilderung, die der Diagnostik viel zu spät zugeführt werden oder aber auch verkannt werden, wie dies bei einem Dekubitalkarzinom im eigenen Krankenkollektiv der Fall war. Bezüglich Prevention muß weiterhin darauf hingewiesen werden, daß die Gefahr der malignen Entartung allgegenwärtig ist und daB sie somit ins Kalkül gezogen werden muß, wenn instabile Narbenverhältnisse vorliegen.
    Notes: Summary The scar tissue carcinoma is a rare disease which arises from the floor of unstable scars, chronic fistulae, ulcera and radiation injuries. The clinical pictures of 23 cases between 1976 and September 1990 have been elucidated. Compared to earlier findings it must now be stated that the development of cancer in stasis ulcera is more frequent than in burn scars or X-ray cancer. Generally, the number of scar tissue carcinomas seems to decrease. Since surgical and adjuvant therapies are — like in any other cancers — limited, prevention and early diagnosis have become of major importance. Though the cancer grows mainly on the body surface and could, therefore, be easily recognised, we still found tumours of remarkable dimension and disturbance, which were either shown to the physician too late or remained unrecognised by the diagnostician.

    Description / Table of Contents: Zusammenfassung Das maligne Schwannom (MS) stellt einen Tumor der Schwann'schen oder Nervenscheidenzellen dar, der am hdufigsten im Bereich der unteren und oberen Extremität, Stamm and Kopfregion auftritt. Wir berichten über den dritten Fall eines in der Mamma lokalisierten MS. Der tastbare, 1,2 cm im Durchmesser große Tumor fand sich im oberen inneren Quadranten der rechten Brust einer 27j dhrigen Fran. Nach sonographischer und mammographischer Darstellung, sowie Feinnadelbiopsie wurde der Tumor chirurgisch entfernt.
    Notes: Abstract Malignant schwannoma (MS) is a tumor of the Schwann or nerve sheath cells, most frequently occurring in the lower and upper extremities, trunk and head region. We report the third known case of MS of the breast, which occurred in a 27-year-old woman. The palpable tumor, about 1.2 cm in diameter, was localized in the upper inner quadrant of the right breast. After ultrasonography, mammography and fine needle aspiration cytology, the tumor was removed surgically.

    Notes: Abstract The diagnostic value of the decrease in percentage of immunoglobulin G1 (%IgG1) in breast cancer was analyzed with special emphasis on early tumor stages. IgG1 and total IgG were preoperatively measured in the sera of a total of 801 individuals using a modified quantitative affinity chromatography. Group A consisted of 174 healthy individuals of both sexes, group B of 324 female patients with benign breast disease, and group C of 303 patients with invasive and non-invasive breast cancer. Within group C, 13 patients presented with intraductal carcinoma, and 22 patients with a pT1a-tumour (diameter less than 0.5 cm). The %IgG1 values were compared among groups A, B and C. In addition, correlations were sought between %IgG1 values of group C and tumor size, stage (UICC), histopathological grade and oestrogen (ER) and progesteron receptor (PR) expression. The mean value of %IgG1 in group A was 63.3 ± 0.5 s.e.m., in group B 57.75 ± 0.4 s.e.m. and in group C 52.37 ± 0.5 s.e.m. The differences of mean values were highly significant between all three groups. Sensitivity and specificity of %IgG1 to discriminate between group A and C were 75% and 87%, and between group B and C 62% and 63%, respectively. The significant decrease of %IgG1 in total serum IgG is able to distinguish patients with breast cancer of more than 5 mm in diameter from healthy controls and patients with benign breast diseases. Finally, calculated posterior probabilities revealed that within certain concentration limits %IgG1 may provide predictive information with high xprobabilities.
