    Notes: Two studies examined the relation between narcissism, self-concept clarity, negative emotions, and aggression based on theoretical assumptions proposed by Baumeister, Smart, and Boden (1996). Narcissism and self-concept clarity were examined as predictors for anger, depression, and verbal aggression following ego-threat, which was operationalized by a bogus performance feedback on an intelligence test. The second study also examined the mediating effects of participants' negative emotions to provide an additional explanation for the aggressive reactions after failure. As expected, narcissism and self-concept clarity were significant predictors of negative emotions and aggression after failure. In accordance with our hypothesis, high narcissists with low self-concept clarity reacted with anger and aggression after failure, whereas less narcissistic individuals with high self-concept clarity showed feelings of depression and no aggression. The results also indicated that aggression was always directed toward the source of the ego-threatening feedback. Additionally, anger and depression could predict the aggressive response after failure but they did not mediate the relation between narcissism, self-concept clarity, performance feedback, and aggression.

    Notes: The present research tested the hypothesis that activity inhibition (as measured by the picture story exercise) moderates the relationship between stress and mood. Based on prior research that shows that (a) individuals high in activity inhibition restrain emotional and motivational impulses and (b) inhibiting negative emotion may lead to further emotional impairments, we assumed that individuals high in activity inhibition show evidence of increased negative mood when they are confronted with stressful events. Study 1 found evidence of increased negative mood-relevant thought among individuals high in activity inhibition who were confronted with the threat of social rejection. Study 2 found high negative mood among individuals high in activity inhibition who experienced many daily hassles. Also, participants high in activity inhibition reported a disproportionally higher amount of daily hassles in the middle (but not at the beginning) of a university semester. We suggest that this pattern of results can be accounted for by Wegner's (1994) theory of ironic effects of the suppression of thought and emotion.

    Notes: Background : Droperidol is useful for postoperative sedation in infants and children after cardiac surgery because it provides sedation and akinesia with minimal respiratory depression. However, droperidol has been associated with QT prolongation and ventricular arrhythmias. We investigated, if neuroleptanalgesic doses of droperidol led to QT prolongation and cardiac arrhythmias in children undergoing cardiac surgery.Methods : We retrospectively analysed electrocardiogram rhythm strips that were obtained before and in time increments after a 100 μg·kg−1 intravenous bolus of droperidol in 20 children undergoing cardiac surgery. The longest QT interval was determined in each ECG and corrected for heart rate (QTc). All arrhythmias were recorded.Results : Droperidol led to a significant increase in QTc time that was still present at 15 min but had resolved within 30 min after the bolus. No associated arrhythmias were observed.Conclusions : The statistically significant prolongation of QTc time after a sedative dose of droperidol is of concern because it may increase the risk for malignant cardiac arrhythmias. A large, prospective study is necessary to identify the true risk for arrhythmias after droperidol in this patient population, but our study suggests that any arrhythmogenic risk, if present, will be very transient, since the increase in QTc time was limited to a period of less than 30 min after the bolus.

    Notes: In recent years, the palliative treatment of Hypoplastic Left Heart Syndrome (HLHS) with a three-staged surgical repair has gained widespread acceptance in North America and elsewhere and has significantly improved the life-expectancy of these children. We report on a child in whom the diagnosis of cystic fibrosis (CF) was made shortly after the first-stage palliation (modified Norwood procedure), and in which surgical palliation was successfully completed with second- (bidirectional Glenn) and third-stage (fenestrated Fontan) procedures. During this period, the child suffered several CF-related complications, i.e. repeated respiratory failure, meconium ileus and coagulation abnormalities, which required adjustment of the anaesthesia, surgical and intensive care management. Considering the trend to perform surgical corrections of major cardiac defects in the early neonatal period, before concomitant genetic diseases are discovered, paediatric cardiac anaesthesiologists, surgeons and intensivists may be confronted with more cases of open heart surgery in infants with CF in the future.

    Notes: We report the case of an 18-month-old girl with spinal muscular atrophy (SMA) that received 1 mg·kg–1 rapacuronium for laryngospasm during induction of anaesthesia. Within 15 min, we observed some diaphragmatic recovery and, after emergence from anaesthesia, the child demonstrated adequate respiratory efforts. However, the child showed diminished strength of the upper extremity muscles. Since the preoperative workup had revealed bulbar symptoms and laryngeal function could not be easily assessed, the patient was kept intubated until upper extremity strength had returned to preoperative levels. Small doses of midazolam had been given to reduce the patient’s anxiety but the patient was extubated within 5 h without any complications. Train of four (TOF) monitoring of the right adductor pollicis muscle, performed during anaesthetic recovery, was equivocal. In SMA, muscle groups are differentially affected so that TOF responses may be inconclusive and not reflect the state of the upper airway muscles. To our knowledge, this is the first report of use of a nondepolarizing neuromuscular blocking agent in a child with SMA.

    Notes: Zusammenfassung Beim Eintreten eines akuten Herzstillstandes ist nach der Diagnosestellung das wichtigste dassofortige Fassen des Entschlusses, mittels der direkten Herzmassage einzugreifen. Der Zugangsweg ist transthorakal zu wählen, nur bei bereits eröffnetem Abdomen kann die sub-oder transdiaphragmale Massage versucht werden. Gleichzeitig ist die Intubation und Betmung mit reinem Sauerstoff durchzuführen und unerläßlich. Als unterstützende Maßnahme ist die Injektion von CaCl2 oder BaCl2 bei Herzstillstand, von KCl bei Kammerflimmern, sowie in jedem Falle von Novocain in den Herzmuskel zu nennen. Intrakardiale Adrenalininjektion ist nur erlaubt, wenn spontane Kontraktionen in genügender Zahl und Stärke vorhanden sind. Die Prognose des Zwischenfalles hängt von der Dauer der Hirnischämie ab, außerdem ist sie um so günstiger, je weniger der Organismus durch chronische Anoxämie oder konsumierende Erkrankungen vorgeschädigt ist. An Hand der neueren einschlägigen Literatur wird der gesamte mit dem Herzstillstand zusammenhängende Fragenkomplex ausführlich besprochen, insbesondere wird auch auf die Pathogenese sowie die Prophylaxe des Zwischenfalles eingegangen. Berichtet wird über 10 eigene Beobachtungen von Herzstillstand; es konnte einmal eine völlige Wiederherstellung und 4mal eine befristete Wiederbelebung erzielt werden; in 5 Fällen war die Therapie erfolglos. Die Fälle werden eingehend besprochen und an ihnen wie auch aus der Literatur erneut das Vorgehen zur Therapie des Zwischenfalles aufgezeigt. Von besonderem Interesse ist eine eigene Beobachtung von etwa 5 min dauerndem Herzstillstand, wobei es zu umschriebenen neurologischen Ausfallserscheinungen kam und bei der Obduktion im Gehirn histologische Veränderungen der Ganglienzellen festgestellt werden konnten.
