Résumé Sur 118 tableaux vasculaires, dont 50 angiogrammes en séries, on a pu évidencier les modifications morphologiques du glioblastome. La comparaison entre le tableau artériographique et l'examen histologique a révélé une ample correspondance: on a surtout pu expliquer le manque de visualisation de la tumeur dans le tableau vasculaire, á cause des fortes modifications prolifératives des vaisseaux ou á cause des zones étendues de nécroses et de kystes. Avec l'aide d'une angiographie en séries dont les temps étaient contrôlables, on a pu établir les caractéristiques pathologiques et circulatoires du glioblastome qui furent employées pour le diagnostic différentiel en comparaison avec d'autres espèces de tumeurs. Comme éléments fonctionnels caractéristiques du glioblastome on a observé une grande prolongation du temps de la circulation cérébrale, la visualisation de vaisseaux tumoraux jusqu'au bout de la phase capillaire prolongée dė tout le cerveau, ainsi qu'une représentation précoce des vaisseaux afférents et efférents en consequence de la diminution de la résistence périphérique, á cause de la présence de court-circuits artéro-veineux intratumoraux.
Abstract:
Summary Based on 118 vascular pictures, among which were 50 serial angiograms, it was possible to prove the morphologic modifications of the glioblastoma. The comparison between the arteriographic picture and the histological state showed a large concordance. Above all it was possible to explain the missing visualization of the tumour in the vascular picture, it being due to strong proliferative modifications of the vessels or to extensive zones of necrosis and cysts. With the help of a timed serial angiography it was therefore possible to esiablish the pathologic characteristics of the blood flow of the glioblastoma and to utilize them in comparison with other tumour-types for the differential diagnosis. The characteristic functional signs of the glioblastoma proved to be a considerable prolongation of the blood flow time of the whole brain, the visualization of the tumoral vessels as far as the end of the prolonged capillary phase of the brain and a premature filling of the afferent and efferent vessels due to the reduced peripheral resistance caused by an arterovenous shunt in the tumour.

Zusammenfassung An 118 Gefäßbildern, darunter 50 Serienangiogrammen, ließen sich die morphologischen Veränderungen des Glioblastoms nachweisen. Der Vergleich zwischen Form der arteriographischen Darstellung und histologischem Befund ergab eine weitgehende Übereinstimmung, wobei insbesondere eine fehlende Anfärbung der Geschwulst im Gefäßbild durch starke proliferative Gefäßveränderungen oder ausgedehnte Nekrosen und Cysten zu erklären war. Mit Hilfe einer meßbaren Serienangiographie konnten darüber hinaus die kreislaufpathologischen Besonderheiten des Glioblastoms festgestellt und im Vergleich zu anderen Tumorarten differentialdiagnostisch ausgenutzt werden. Als charakteristische funktionelle Merkmale des Glioblastoms erwiesen sich dabei eine erhebliche Verlängerung der Kreislaufzeit des Gesamthirns, die Darstellung der Tumorgefäße bis zum Ende der verlängerten kapillaren Phase des Gesamthirns sowie eine frühzeitige Füllung der zu- und abführenden Gefäße infolge des durch arteriovenöse Kurzschlüsse verminderten peripheren Widerstandes innerhalb der Geschwulst.
