    Notes: Summary Five cases of germinoma occurring in the infundibular region are presented. The clinical symptoms and treatment as well as the cytological and histological pictures, including the ultrastructural findings, are described. When unassociated with other neoplastic germ cell elements, infundibular germinoma is considered to be a highly radiosensitive tumour with a good prognosis.

    Notes: A case of a large, completely excised left-sided primary ovarian chondrosarcoma occurring in a 61-year-old woman is described. There was no evidence of metastases. Histologically the tumour was a pure chondrosarcoma, which was mainly well differentiated. The patient is well and disease free at 4 years. Review of the literature revealed no other reported cases of pure chondrosarcoma of the ovary. The possible histogenetic origin of this neoplasm is discussed.

    Notes: A case of primary ganglioneuroblastoma located in the anterior mediastinum is described. The tumour was found incidentally at autopsy in a 61-year-old woman who died of coronary artery thrombosis. There was no evidence of metastases. Histologically the tumour was a ganglioneuroblastoma of the diffuse type. There was no pure neuroblastomatous tissue present and maturation to ganglioneuroma was not observed. Regressive changes like fibrosis and calcification were present. Review of the literature failed to reveal any recorded cases of ganglioneuroblastoma occurring in the anterior mediastinum. Although ganglioneuroblastoma occurs predominantly in children, 24 cases occurring in adults have been reported in the literature.

    Notes: The presence of calcitonin and of carcinoembryonic antigen (CEA) was studied in six cases of medullary carcinoma of the thyroid using an immunoperoxidase technique. In five cases the material was obtained surgically and in one at autopsy. Tissue from primary tumours as well as from metastases was studied. Calcitonin and CEA were identified within all the tumours studied, although their pattern of distribution and staining intensity varied both within the cells and within the tumour. Some parts of the tumour contained both CEA and calcitonin, while others stained positively only for one of these substances. In some parts of the tumour there was no positive staining for either substance. Within the cells, CEA showed a typical linear distribution along cell surfaces, while calcitonin showed a more even cytoplasmic distribution and the deposits were more granular. Normal tissue surrounding tumour deposits did not show positive staining. It is considered that cells of medullary carcinoma of the thyroid contain both calcitonin and CEA. Identification of CEA and calcitonin in tumour tissue can be used as a diagnostic aid to identify medullary carcinoma of the thyroid. It is considered that these substances are being produced by this tumour and can be used as tumour markers.

    Notes: Serum alpha-fetoprotein (AFP) and serum alpha-1 antitrypsin (AAT) was determined in 24 patients with germ cell neoplasms of the gonads and extra-gonadal sites.Although when serial determinations are performed a correlation between AAT and AFP can be seen, 71 determinations of AAT as well as AFP in the serum of the patients with germ cell tumours gave the following results: when the AFP-concentration was below 20 ng/ml, the AAT values were always below the upper limit of the normal range (4.0 g/l); with AFP-values well above 20 ng/ml, the AAT concentration was elevated (〉4.0 g/l) in only 33% of the cases; in all the sera with an elevated AAT-level, the AFP-concentration was—mostly considerably—enhanced; and in 45% of the sera with normal (or lowered) AAT, there was a diagnostically meaningful elevation of the AFP-concentration. In addition data from 9 patients with tumours not of germ cell origin undergoing therapy showed considerable variations of AAT-values at normal AFP concentrations.It is therefore considered that although there is evidence that AAT is produced by EST, serum AAT is not a useful monitor of disease activity in these patients, especially when compared with serum AFP.

    Notes: Serum alpha-fetoprotein (AFP) was studied in 237 patients with germ cell neoplasms of the gonads and extra-gonadal sites. Whenever possible serial determinations were performed. All patients in remission had normal serum AFP. Raised serum AFP was seen in patients with active disease whose tumours contained endodermal sinus tumour (EST). This included 29 patients with testicular tumours, 11 with ovarian, 3 with mediastinal, and one with a retroperitoneal tumour. In all these patients serum AFP levels were high and correlated well with the amount of EST within the tumour. Serum AFP became normal when the disease was eradicated. There was a gradual rise with recurrence of the disease. Slightly raised serum AFP (below 60 ng/ml) was present in two patients with embryonal carcinoma examined preoperatively. In four similar cases serum AFP was normal. All patients with pure teratoma had normal serum AFP. Seventeen patients with pure seminoma whose serum was examined preoperatively had normal serum AFP at all times. These findings indicate that raised serum AFP is associated with the presence of EST within the tumour, and AFP is a good and reliable tumour marker in patients with germ cell neoplasms containing EST.

    Notes: Summary Ultrastructural features of muscle spindles were studied in biopsy material from 100 patients suffering from classical rheumatoid arthritis. Thickening of the outer capsule, increased amount of extracellular ground substance within the inner capsule, and marked thickening of the basement membrane of capillary blood vessels supplying the muscle spindles were observed. Chronic inflammatory cells and macrophages were present within the spindles. Changes affecting the intrafusal muscle fibres were also seen. They were manifest as atrophy and degeneration of the intrafusal muscle fibres, absence of the specialised junctional complexes, and of the intercellular bridges, microladders and satellite cells. It is suggested that the changes affecting the intrafusal muscle fibres are probably secondary. Damage to the myelinated nerves was present, while the sensory and motor nerve endings were well preserved.

    Notes: Summary Muscle changes were studied in biopsy material obtained from 100 patients suffering from classical rheumatoid arthritis. The abnormalities consisted of denervation atrophy of type II muscle fibres, degenerative changes in the sarcoplasm including presence of nemaline rods, and changes within the interstitium: namely perivascular nodular myositis, lymphocytic accumulations, different stages of vasculitis and abnormalities within the intramuscular nerves and muscle spindles. The muscles examined were always severely affected. It is considered that the simultaneous presence of these abnormalities is suggestive of rheumatoid arthritis. The importance of histochemical studies is emphasized. The literature concerning muscle changes in rheumatoid arthritis is reviewed.

    Notes: Summary Carcinoid tumors of the ovary are uncommon, but 150 cases of primary ovarian carcinoids and 40 cases of carcinoid tumors metastatic to the ovary have been reported. Primary carcinoid tumors of the ovary are divided into insular, trabecular, strumal and mucinous types. The insular type is most common, followed by the strumal type. The majority of primary ovarian carcinoids occur in association with mature cystic teratoma, but a considerable number present in pure form. Only the insular type is associated with the carcinoid syndrome. The age incidence of patients with ovarian carcinoids shows a wide range but most patients are postmenopausal. Primary carcinoids of the ovary are invariably unilateral. They form a solid nodule within a cystic teratoma, or when pure a solid yellow-grey mass, and vary from microscopical to large tumors measuring in excess of 20 cm in the longest diameter. The metastatic carcinoids are nearly always bilateral and scattered tumor deposits are present throughout both ovaries. Primary ovarian carcinoids metastatize only occasionally, and should be treated as ovarian tumors of low malignant potential.
