Zusammenfassung An 162 Meerschweinchen wurde das immunologische Modell eines lokalen Shwartzman-Phänomens (1-SP) der Haut (ohne Endotoxin) von de Wecku. Mitarb. reproduziert, und zwar durch Vorsensibilisierung, Präparation und Auslösung mit Dinitrochlorbenzol (DCNB) bzw. Neo-Salvarsan (Neo-S) bestimmter Chargen. Nach Überwindung zahlreicher methodischer Schwierigkeiten gelang die Sensibilisierung mit eindeutiger Spätreaktion der Haut für DNCB 19mal und für Neo-S 43mal. 26mal konnte ein 1-SP ausgelöst werden. Es wurden 16 histologische Kontrollen durchgeführt. Die Auslösung des 1-SP gelang sowohl homolog als auch heterolog. Gerinnungsanalysen wurden 1 oder 8 Std nach der i.v. Auslösung bei 19 Tieren mit positivem und 20 Tieren mit negativem 1-SP durchgeführt. Die i.v. Injektion von Neo-S allein erzeugte bereits einen individuell unterschiedlich stark ausgeprägten Gerinnungsdefekt vom Typ einer Verbrauchsreaktion. 1 Std nach homologer und heterologer Auslösung eines I-SP wurden Gerinnungsstörungen annähern gleichen Ausmaßes nachgewiesen, wobei sich die Tiere mit positivem und mit negativem Phänomen hinsichtlich ihres Gerinnungsdefektes — abgesehen von den Prothrombinwerten — nicht signifikant voneinander unterschieden. 8 Std nach Auslösung war dagegen nur bei den Tieren mit positivem I-SP eine Verminderung der Thrombocyten und der Gerinnungsfaktoren nachweisbar. Mit Coli-Endotoxin gelang eine heterologe Auslösung des I-SP überraschenderweise nicht, obwohl selbst nach 8 Std ebenfalls eine deutliche Gerinnungsstörung nachweisbar war. Insgesamt sprechen diese Ergebnisse gegen eine maßgebliche Beteiligung des Gerinnungssystems bei der Entstehung des lokalen Shwartzman-Phänomens.

Summary The immunological model of a local cutaneous Shwartzman phenomenon (without endotoxin) described by de Weck and coworkers has been reproduced in 162 guinea pigs. Presensitization, preparation and provocation was carried out with dinitrochlorbenzol (DNCB) or neo-salvarsan (Neo-S) of certain charges. DNCB-Sensitization with a marked delayed reaction was successful in 19 cases and Neo-S-sensitization in 43 cases. A local Shwartzman-phenomenon (1-SP) could be challenged 26 times. 16 biopsies were taken. The provocation of the 1-SP was achieved homologically as well as heterologically. Blood coagulation has been studied in 19 animals with positive and in 20 animals with negative reactions one and eight hours after intravenous challenging of the 1-SP. The intravenous injection of Neo-S alone produced a defect of blood coagulation of the consumption type, the degree of which varied individually. One hour after homological or heterological provocation alterations of blood coagulation of nearly the same degree were found; there were no significant differences with regard to the degree or the type of the coagulation defect in the animals with a positive and the animals with a negative reaction. 8 h after challenging however, only the animals with a positive reaction showed decreased platelet counts and coagulation factors. Surprisingly, heterological provocation of 1-SP was unsuccessful with coli endotoxin, although a distinct coagulation defect was demonstrated even after 8 h. These results argue against a considerable participation of the coagulation system in the pathogenesis of local Shwartzman phenomenon.
