    Description / Table of Contents: Abstract Wegener’s granulomatosis is a distinct clinicopathologic entity characterized by granulomatous vasculitis of the upper and lower respiratory tract and glomerulonephritis. This disease can present as a clinical picture which resembles sepsis and adult respiratory distress syndrome (ARDS). Wegener’s disease requires immunosuppression which can have detrimental consequences when used in sepsis. The following case report illustrates the diagnostic difficulties encountered by intensiv care physicians treating severe pulmonary failure and multiple organ dysfunction in Wegener’s granulomatosis appearing as ARDS with sepsis. Case report: A 19-year-old female patient had developed acute respiratory and renal failure after a prolonged period (many months) of antibiotic resistant otitis, sinusitis and mastoiditis. The patient had required intubation at another hospital and there was a history of tension pneumothorax and cardiopulmonary resuscitation during mechanical ventilation. Emergency extracorporeal membrane oxygenation (ECMO) for acute hypercapnic and hypoxic respiratory failure was instituted and the patient was transported to our institution while on ECMO. The patient was treated empirically for suspected pulmonary and systemic infection and received hydrocortisone (0,18 mg/kg/h) as part of a protocol-driven treatment of septic shock in addition to antibiotic and antimycotic regime. The use of ECMO was required for 10 and mechanical ventilation for another 50 days after admission. After successfull extubation, central nervous system dysfunction became evident with a somnolent and generally unresponsive patient. When the hydrocortisone dose was gradually tapered, the clinical status of the patient further deteriorated, pulmonary gas exchange worsened and she developed renal failure with proteinura and hematuria. A renal biopsy was performed demonstrating vasculitis and focal segmental glomerulonephritis, a systemic granulomatous vasculitis was suspected; the serum was tested for anti-proteinase 3 antibodies (PR3-ANCA) and turned out to be positive (17.5 U/ml; normal range 〈7 U/ml). The morphologic findings from renal biopsy, the positive test for antiproteinase 3 antibodies and the pulmonary-renal involvement with evidence of multisystem disease established the diagnosis of Wegener’s granulomatosis. Immunosuppressive therapy with cyclophosphamide and prednisolon was instituted resulting in rapid improvement with recovery of pulmonary, renal and central nervous system function within two weeks. The use of ECMO in this patient served as a life – saving immediate measure usefull to ”buy time” until a definite diagnosis could be established. ARDS represents an uniform pulmonary reaction to a large number of different noxious stimuli and disease entities. This case demonstrates that intensiv care physicians caring for critically ill patients with ARDS should include even rare causes of pulmonary injury into their differential diagnosis.
    Notes: Zusammenfassung Wir berichten über eine 19jährige Patientin, bei der unter dem typischen Bild eines schweren ARDS mit Multiorganversagen für insgesamt 10 Tage der Einsatz einer extrakorporalen Lungenersatztherapie (ECMO) erforderlich war. Therapieverlauf: Unter einer kalkulierten antibiotischen und antimykotischen Therapie sowie einer Behandlung mit Hydrocortison als adjuvanter Therapie bei septischem Schock besserte sich erst nach wochenlangem und kompliziertem klinischen Verlauf die Lungenfunktion soweit, daß eine Extubation möglich war. Die Patientin zeigte jedoch unverändert eine Mehrorgandysfunktion von Niere, Lunge und ZNS. In den folgenden Wochen nach Beendigung der Hydrocortisontherapie verschlechterten sich Nierenfunktion, pulmonaler Gasaustausch und Vigilanz wieder. Diagnostik: Der histologische Befund der Nierenbiopsie mit Arteriitis und Glomerulonephritis bei beidseitiger Vergrößerung der Nieren im CT und der Nachweis von Proteinase 3-ANCA im Serum ermöglichten letztlich bei Würdigung des gesamten klinischen Bildes und seiner genauen Vorgeschichte eine Diagnose: Wegener-Granulomatose. Durch immunsuppressive Therapie kam es innerhalb kurzer Zeit zu einer Remission mit vollständiger Erholung insbesondere der ZNS-Funktion. Schlußfolgerung: Dieser Fallbericht zeigt, daß im Einzelfall auch seltene Krankheitsbilder mit pulmonaler Beteiligung wie die Wegener-Granulomatose in die Differentialdiagnose des ARDS einbezogen werden müssen.

    Notes: Abstract Sacral insufficiency fractures develop over a period of time and show time-dependent changes. We report on 15 CT examinations of 5 patients with early-stage insufficiency fractures of the sacrum. In 4 patients only irregular sclerosis without distinct fracture lines was present in 7 of 8 fractures. Of these 4 patients; 3 exhibited intraosseous gas inclusions in a ventral part of a lateral mass; 5 of 8 fractures disclosed a ventral cortical break. When distinct fracture lines had developed in 1 patient, intraosseous vacuum phenomenon had disappeared. Fracture lines evolve over weeks to months and show central bone absorption. The fractures can heal as demonstrated in 4 of 6 fractures in 3 patients, can persist over 1 year without significant changes or can progress to pseudoarthrosis with bone destruction similar to neuropathic joint disease. Intraosseous vacuum phenomena can persist to this stage. Intraosseous vacuum phenomenon is recognized as a potential finding in the early stage of sacral insufficiency fracture, which also is true for irregular sclerosis and ventral cortical disruption.

    Notes: From the application pattern of surfactant-containing cosmetic products, it is inevitable that the major part of the chemicals will be discharged into waste water and eventually will enter the environment. Because the environmental compatibility of the products is based on the ecological properties of their raw meterials, the biodegradability and ecotoxicological behaviour of the product components and particularly the surfactants, form the central elements of the environmental compatibility assessment. The tools for this evaluation are standardized test systems, which are described and discussed on the basis of the ecological data of selected surfactants. De par le type d'application des produits cosmetiques contenant des tensioactifs, il est inevitable que la plus grande partie des substances chimiques soit evacuee dans les eaux usees et finisse par arriver dans l'environnement. Puisque la compatibilite environnementale des produits est basee sur les proprietes ecologiques de leurs matieres premieres, la biodegradabilite et le comportement ecotoxicologique des composants des produits, et en particulier des tensioactifs, forment les elements majeurs de l'evaluation de la compatibilite environnementale. Les outils de cette evaluation sont des systemes d'essai normalises, qui sont decrits et commentes d'apres les donnees ecologiques de tensioactifs choisis.

    Notes: High resolution stimulated gain Raman spectroscopy is used to investigate the collisional parameters of pure rotational S0(j=0–4) lines of H2 in pure H2 and H2–He mixture. Measurements are performed between 300 and 1000 K in a density regime where the lines are essentially collisionally broadened (typically 10 amagat). For the first time, these highly accurate measurements of the frequencies of pure rotational lines allow one to correct previously measured values that did not take into account the collisional frequency shift. For both collisional systems, the shifting coefficients exhibit a linear behavior with the square root of temperature, similar to the behavior already observed in the Q branch. The broadening coefficients of the S0 branch increase nonlinearly with temperature contrary to the Q branch. For the H2–He system, both these new S0(j) data and previously measured Q(j) data are analyzed using a modeling of the broadening coefficients in terms of elastic and inelastic contributions. These different contributions are analyzed as a function of temperature and of the rotational quantum number j. Preliminary quantum calculations are used to assess the validity of the model.     Notes: Photoemission spectromicroscopy data on Si–GaSe with 0.2 μm lateral resolution revealed laterally inhomogeneous interface Se–Si chemical reactions. The local Si 2p fits agrees with interface Si selenide parameters; the Se 3d peaks indicate a Si–selenide component; the Ga 3d peaks exhibit a metallic component whose intensity increases with Si coverage.     Notes: The switching of second-harmonic generation (SHG) at a C60 single-crystal surface has been observed in a pump-and-probe experiment. The SHG signal from a picosecond 1.17 eV laser pulse is suppressed by one order of magnitude upon illuminating the crystal surface with a 3.49 eV pump pulse. The nonlinear optical response is faster than 45 ps and persists for longer than 20 ns. SHG suppression to 1/e occurs at pump densities as low as 2.8 μJ/cm2. We suggest that nonlocalized excited electronic states determine the change in the nonlinear optical behavior. © 1996 American Institute of Physics.

    Notes: Summary Pilomatrixoma is a benign tumour of the cutaneous adnexa. Histologically. pilomatrixoma comprises masses of immature basophilic cells, small numbers of polygonal squamoid cells, few transitional cells, and clusters of ‘shadow cells’. The mechanism leading to the formation of shadow cells is still unknown. Skin biopsy specimens of pilomatrixoma (n= 15) were studied histologically, immuno-hislologically, and by applying the in situ end-labelling technique. The basal layer of the basophilic cells included most of the proliferating cells with high expression of bcl-2 and cytokeratin 19. The overlying basophilic cells showed a negligible mitotic activity, a high significant accumulation of p53 protein, and a heterogeneous, but progressive loss of bcl-2 and cytokeratin 19. They developed either into squamoid cells or into transitional cells. The squamoid cells were characterized as differentiated cells resembling mature keratinocytes of stratified mucosa. The transitional cells could be shown to represent apoptotic cells proceeding to shadow cells. The data suggest that apoptosis is the main mechanism leading to the development of the dead shadow cells and is most probably responsible for the banal biological behaviour of pilomatrixoma. Apart from that, pilomatrixoma represents a suitable biological model to study apoptosis in humans.

    Notes: Scanning photoelectron spectromicroscopy experiments on the edge of patterned Si overlayers on GaSe revealed the presence of an inhomogeneous reaction with a metallic-like Ga phase separation. The Si–Se chemically shifted components show lateral variations and a thickness-related SiSe2/SiSex (x=0.5, 1, and 1.5) ratio on a micrometer scale. The dependence of the peak intensities on the overlayer thickness suggests an initial layer-by-layer coverage until (approximate)2 Å, followed by a clustering growth mode. © 1999 American Institute of Physics.

    Notes: We studied the deposition of Au, In, Ag, and Al overlayers onto clean-cleaved GaTe(001) surfaces at room temperature by x-ray photoelectron spectroscopy. Gold and In overlayers did not produce evidence of chemical reactions, and neither with cation nor anion outdiffusion. Gold overlayers exhibited no evidence of island formation and a layer-by-layer overlayer coverage. The deposition of In revealed the presence of clustering for a metal thickness higher than 6 Å. On the other hand, the deposition of Al lead to an exchange reaction with a metalliclike Ga phase separation. Core level analysis of the Ag–GaTe interface revealed no noticeable change of the shape of the peaks. The dependence of the peak intensities on overlayer thickness showed an initial layer-by-layer coverage until 1.8 Å, followed by a clustering growth mode. From about 3.7 Å Ag thickness, significant Te outdiffusion was observed. © 1998 American Institute of Physics.

    Notes: A microscopic chemical analysis of BiSrCaCuO (BSCCO) 2212 single crystals was performed with an unprecedented combination of high (submicron) lateral resolution and high energy resolution (100 meV or better). These performances, achieved with imaging synchrotron spectromicroscopy, enabled us to detect in a very small fraction of the investigated specimens, morphological and chemical inhomogeneities. Specifically, we found deviations from stoichiometry in the surface Sr content which have a subtle effect on the chemical environment of at least one other element, Ca. The different chemical environment was found to influence surface chemical reactions. © 1996 American Institute of Physics.
