    Notes: Erythema infectiosum is known to be caused by human parvovirus B19 and shows characteristic clinical skin manifestations in children, although adult cases of human parvovirus B19 infection do not always show such characteristic features. Recently, we experienced an epidemic adult cases of human parvovirus B19 infection and examined the erythematous skin lesion by immunohistochemistry and electron microscopy to clarify the pathogenesis of the skin manifestations. Light microscopic examination showed slightly irregular-shaped vessels in the dermis. By immunohistochemistry, using anti-human parvovirus B19 monoclonal antibody, positive reactions were found in endothelial cells. No immunoglobulins were found, but C3 deposits were present in the perivascular areas. By electron microscopy, virus particles were found in the cytoplasm of endothelial cells. An inflammatory reaction due to the direct human parvovirus B19 infection in dermal vessels seems to be an important factor in the pathogenesis of the skin manifestations.

    Notes: Abstract. Purpose: To analyze the influence of liver dysfunction and parenchymal pathology on the accumulation of superparamagnetic iron oxide (SPIO). Methods: We evaluated MR images of 13 patients having small hepatic neoplasms before and after administration of SPIO (10 μmol/kg). Biopsy and laboratory data confirmed the presence of severe cirrhosis in two patients, mild cirrhosis in four, chronic hepatitis in five, and normal livers in two. Degrees of liver dysfunction or liver parenchymal pathology were correlated with reductions in signal intensity of the liver and spleen after administration of SPIO. Signal intensity reduction was evaluated using a 1.5 Tesla MR unit. Results: Response to SPIO of the liver and spleen did not correlate with liver parenchymal pathology, although reductions in signal intensity of the liver were somewhat small in severely cirrhotic livers. There were slight correlations between signal intensity alterations of the liver and laboratory data such as the indocyanine green retention rate (correlation coefficient 0.47), albumin (0.36), total bilirubin (0.36), and serum glutamic oxaloacetic transaminase (GOT) (0.46). Signal intensity reduction of spleen did not correlate with liver function tests except for serum GOT. In patients with cirrhosis, heterogeneous structures were detected in the nontumorous portions of the liver. However, these did not prevent the diagnosis of small hepatomas. Conclusion: The uptake of SPIO showed some correlation with liver function but not with chronic liver parenchymal pathology. SPIO provided sufficient contrast between tumor and surrounding liver parenchyma among patients with chronic liver disease.

    Notes: Abstract Background: Multilocular cystic renal cell carcinomas (MCRCCs) are a recently described variety of renal cell carcinoma with characteristic pathological and clinical features. We found that the radiologic appearances of MCRCCs of smaller size did not fulfill the previously documented criteria of the MCRCCs. This study was conducted to analyze the radiologic characteristics of MCRCCs of smaller sizes. Methods: The radiologic findings of 13 multilocular cystic renal cell carcinomas of diameter ranging from 10–32 mm (average 22 mm) seen in nine patients were analyzed in correlation with pathologic findings. Results: On US, the tumors were predominantly hyperechoic (11 of 13 tumors) with or without small anechoic areas. Precontrast CT showed the lesions to be either hypodense or hyperdense depending on the presence of hemorrhage. Degree of contrast enhancement was usually slight. The mean increase in CT attenuation was 28 ±19 (mean±standard deviation) at dynamic phase and 12±10 at delayed phase. On MR imaging, signal intensities of the tumors were high both on T1- and T2-weighted images (7 of 9 tumors) due to proteinaceous fluid or hemorrhage. Dynamic enhanced MR imaging revealed irregular contrast enhancement within the tumor (5 of 6 tumors). Angiography failed to reveal neovascularity. Conclusion: Although multiple cysts were seen within the tumors pathologically, MCRCCs of smaller sizes appeared solid on radiologic examinations. However, contrast enhancement or neovascularity was very slight.

    Notes: Abstract. Background: Analysis of unenhanced and contrast-enhanced magnetic resonance (MR) images of struma ovarii, a rare benign neoplasm of the ovary, is the aim of this study. Methods: T2-weighted and Gd-DTPA-enhanced T1-weighted MR images of five histologically proven struma ovarii were evaluated retrospectively. Results: In all patients, unenhanced and contrast-enhanced T1-weighted MR images showed complex cystic masses composed of multiple cysts and a solid component, indicating the presence of large and small thyroid follicles. In four patients, the cyst fluid was hypointense on T1-weighted images and hyperintense on T2-weighted images. In one patient, the fluid was hyperintense on T1-weighted images and hypointense on T2-weighted images due to hemorrhage. Conclusion: A complex mass composed of multiple cysts and a solid component, indicating the presence of large and small thyroid follicles, appeared to be a characteristic MR finding of struma ovarii.

    Notes: Abstract Background: Assessments were made of 63 cases of fundic gland polyposis (FGP) unassociated with adenomatosis coli. These cases were evaluated by radiological examination over 2 years follow-up. All polyps were pathologically confirmed by endoscopic biopsies. Methods: Most cases were asymptomatic when diagnosed during mass radiological surveys of the upper gastrointestinal tract. The majority of patients ranged in age from 40–60 years, and the polyps numbered fewer than 20 in 55 cases (87.3%). Polyps were detected in the fundic glands using the congo red test and by biopsies. Results: All serum gastrin values were within the normal range. During the course of this study, the polyps of 13 cases (20.6%) increased and those of three cases (4.8%) decreased or resolved completely. Conclusion: From these findings it is considered that FGP are observed in stomachs with less atrophy, and that polyps follow courses in which they increase, decrease, disappear, along with atrophy.

    Notes: Abstract Angiomyolipoma is a benign mesenchymal tumor that has been reported frequently in the kidney but rarely in the liver. In the present study, we present four cases of hepatic angiomyolipoma with different radiologic appearances, discuss differential diagnosis, and review previously reported cases. One of our cases was followed for 8 years. Computed tomography (plain, enhanced, and dynamic study), magnetic resonance imaging (T1-weighted spin echo, T2-weighted spin echo, and dynamic study), ultrasonography, and angiography were performed in all cases. Although different radiologic appearances were observed in the tumors, based on different proportions of fat, blood vessels, and muscle, three cases were diagnosed as angiomyolipoma. In one case, it was quite difficult to make radiologic diagnosis; hepatocellular carcinoma with fatty metamorphosis in part was most likely suspected, but histopathological examination revealed angiomyolipoma with few fat elements. In the present study, early and prolonged enhancement of the lesion with the special pattern of time density/intensity curve was significant for angiomyolipoma, and we suggest that preoperative radiologic diagnosis of the lesion is possible in most of the cases. However, it can be quite difficult to distinguish angiomyolipoma from some hepatocellular carcinomas with fatty metamorphosis.

    Notes: Abstract Background: The purpose of this study was to evaluate computed tomographic (CT) findings for predicting the presence of intestinal necrosis in patients with closed loop and strangulating obstruction of the small bowel. Methods: Twenty-five patients with surgically confirmed closed loop and strangulating obstruction were divided into two groups with (n= 16) and without (n= 9) intestinal necrosis. By using univariate and multivariate statistical procedures, we evaluated the differences in CT findings between the two groups on the basis of the following six findings: bowel dilatation of strangulated loops (bowel dilatation), wall thickening of strangulated intestines (wall thickening), ascites, vascular dilatation of affected mesenteries (vascular dilatation), elevation of mesenteric attenuation (mesenteric attenuation), and radial distribution of the mesenteric vessels (radial distribution). Results: Of the six findings, ascites, vascular dilatation, mesenteric attenuation, and radial distribution provided significant discriminating findings between the two groups on univariate analysis. On multivariate analysis, mesenteric attenuation was the most important discriminative factor, followed by radial distribution and ascites. Using these three parameters, the CT was correlated with the surgical findings in 15 of the 16 patients in the necrosis group (sensitivity = 93.8%) and in eight of the nine patients in the nonnecrosis group (specificity = 88.9%). The overall accuracy was 92.0%. Conclusions: Mesenteric attenuation, radial distribution, and ascites, depicted on CT differentiate well between necrosis and nonnecrosis of the small bowelin patients with closed loop and strangulating obstruction.

    Notes: Abstract Inorganic phosphate (Pi) uptake by the marine cyanophyte Synechococcus sp. NIBB 1071 was studied using cells grown in an artificial seawater medium. The phosphate uptake was markedly enhanced in cells grown in the medium of low phosphate concentrations (phosphate-limited cells) than in cells grown in the phosphate-rich medium (phosphate-replete cells). The diagnosis of kinetics of instantaneous phosphate-uptake showed that V max of the former was more than two orders of magnitude greater than that of the latter, and the k m of the former was about 1/20 of that of the latter. The enhancement of the phosphate uptake was completed after a 40-h incubation of phosphate-replete cells in the phosphate-free medium. The activation was suppressed by chloramphenicol, an inhibitor of protein synthesis. The uptake developed in phosphate-limited cells was energy dependent and susceptive to osmotic shock, which suggests the involvement of a periplasmic phosphate-binding protein, analogous to that found in heterotrophic gram-negative eubacterial cells. The relationship between phosphate quota and growth rate, together with the kinetical data for phosphate uptake, predicted that ambient phosphate as low as 0.5 nM could support cell growth at a rate of one division per day. Results indicate that cells can grow rapidly even at phosphate concentrations as low as nanomolar levels. A possible regulatory mechanism of phosphate uptake in marine Synechococcus spp. is discussed in relation to a wide distribution of this picophytoplankton in the ocean environment.

    Notes: Abstract Of the two resting life-forms of the planktonic diatom Chaetoceros pseudocurvisetus Mangin formed during periods of nitrate depletion, resting spores survived at least 1 month after spore formation at 24 °C, while resting cells survived only for about 10 d at the same temperature. Under nitrogen limitation, resting cells exhibited higher specific death rates than resting spores at temperatures ranging from 5 to 30 °C. After nitrogen replenishment, resting spores required a certain lag period of about 1 d to initiate vegetative growth at levels of nitrate supply from 0.5 to 20 M, while resting cells initiated vegetative growth almost immediately. Resting spores exhibited an intracellular accumulation of the supplied nitrate during germination and initial vegetative growth. The resting cells, however, exhibited more active vegetative growth, closely coupled with the uptake of the supplied nitrate. The resting spores and resting cells appear to play different roles in the maintenance of populations under nutrient fluctuations depending on the interval length between nutrient fluxes in natural waters.
