    Description / Table of Contents: Summary Cat scratch disease (CSD) is a rarely recognized infectious disease in Germany. Only a few years ago the causative agent, Bartonella henselae, could be isolated. The typical clinical manifestations of CSD consist of skin changes at the inoculation site and a benign lymphadenopathy; other manifestations are rare. We report the case of a 47 year old woman, who developed a reactive spondylarthropathy with synovitis of finger joints, polyarthralgias of large- and medium-sized joints, and inflammatory spinal pain after a cat bite. The rheumatic manifestations resolved after 10 months by treatment with non-steroidal antirheumatic drugs. Only a few cases of rheumatic manifestations associated with CSD have been described in the literature. Because the prevalence of Bartonella henselae infection of cats is high in Europe, rheumatic manifestations might be more frequent. Diagnosis of CSD is now improved by the development of serological tests. We provide an overview of the clinical manifestations and the diagnostic criteria.
    Notes: Zusammenfassung Bei der Katzenkratzkrankheit (KKK) handelt es sich um eine in Deutschland wenig bekannte Infektionskrankheit. Erst vor wenigen Jahren konnte der Erreger, Bartonella henselae, isoliert werden. In der Mehrzahl der Fälle findet sich ein typisches Krankheitsbild mit Hautveränderungen im Bereich von Katzenbiß- oder Kratzwunden und einer benignen Lymphadenopathie. Weniger häufig kommt es zu weiteren Organmanifestationen. Wir berichten über eine 47-jährige Patientin, die nach einem Katzenbiß eine reaktive Spondarthritis mit Synovialitiden der Fingergelenke, Polyarthralgien der mittleren und großen Gelenke und entzündliche Lumbalgien entwickelte. Die rheumatische Symptomatik war zeitlich limitiert und sprach gut auf NSAR an. Bislang sind nur wenige Fälle von Gelenkbeteiligung im Rahmen einer KKK beschrieben worden. Da die Durchseuchung von Katzen mit Bartonella henselae in Europa jedoch hoch ist, könnte es sich um eine häufigere Ursache von Gelenkbeschwerden handeln, als bislang angenommen. Die Diagnostik der KKK wurde in den letzten Jahren durch die Entwicklung serologischer Teste erleichtert. Wir geben einen Überblick über die klinischen Manifestationen und fassen die diagnostischen Kriterien dieser Erkrankung zusammen.

    Notes: The dynamics of weed populations were ana lysed in intensified cereal-based cropping sys tems of the northern Guinea savanna in Nigeria, A total of four common weed associations were identified through cluster analysis. Five factors describing soil fertility conditions and field his tory best differentiated the weed communities according to a discriminant model. The analysis shows that maize-based cropping systems with a high frequency of cereal cropping and a low fre quency of noncereal cropping tended to be domi nated by weeds such as Commelina spp. and Kyllinga squamulata. As soil fertility declined, Vernonia spp. and Eclipta prostrata became more important. Increased frequency of noncereal crops in mixed cropping with cereals was associ ated with reduced incidence of weeds such as Leucas martinicensis, Oldenlandia corymbosa, Spermacoce verticillata, Litdwigia hyssopifolia, Celosia laxa, and,Ipomoea spp. Further diversifi cation of cereal-based systems to obtain a re duced frequency of cereals is likely to increase the incidence of Dactvloctenium aegyptium in crop fields. The information provides guidance for technology development and transfer on weed control for intensifying systems in the northern Guinea savanna of Africa.

    Notes: Abstract In patients with esophageal atresia (EA) and distal tracheoesophageal fistula (TEF) Gross type C and D, escape of ventilatory gases through the TEF exposes the stomach to gaseous distension with the risk of respiratory distress and gastric perforation. In seven patients with EA, Gross type C, primary esophagoesophagostomy was planned within the first 2 days of life. After induction of anesthesia by mask, a rigid ventilation bronchoscope was inserted into the trachea and the TEF was cannulated with a catheter that was pushed down to the stomach. After removal of the endoscope, the patient was intubated beside or over the inserted catheter. Until division and closure of the TEF, continuous suction over the catheter prevented gaseous gastric distention. Intubation of a distal TEF in patients with Gross type C and D EA can easily be combined with routine preoperative bronchoscopy, and is a safe and effective method to decompress a distended stomach and prevent respiratory distress and gastric perforation during controlled ventilation.

    Notes: Abstract Surgical repair of anterior abdominal wall defects is often complicated by a discrepancy between the eviscerated organs and the intra-abdominal space available. Primary closure of the abdominal wall may result in increased intra-abdominal pressure and consecutive circulatory impairment. We report the results of a retrospective and consecutive prospective study evaluating the influence of different witameters on mortality and morbidity in children with gastroschitis and omphalocele. Both studies demonstrated that real-time sonographic investigations, intraoperative Doppler duplex sonography, and colour-coded Doppler sonography provide the oppurtunity to collect objective intraoperative data. Our data indicate that intraoperative vascular assessment facilitates the discrimination between infants who benefit from primary closure and those in whom a staged repair is the treatment of choice.

    Notes: Abstract Prenatal diagnosis of gastroschisis (GS) and omphalocele (OC) has allowed improved antenatal and perinatal management. It was the aim of this study to compare prenatal findings and assess fetal outcome. Twenty-four fetus with GS and 33 with OC were diagnosed prenatally. Maternal serum alpha-protein (MSAFP), sonographic (US), and perinatal data were analyzed. The average maternal age for GS was younger than for OC (24 vs. 30 years). The median MSAFP values were 7.7 multiples of median for GS and 3.6 for OC. The initial US diagnosis was made at 22 weeks for GS and at 19 weeks for OC. A pathological karyotype was observed in 4% of the fetuses with GS and 33% with OC. The mortality of fetuses with OC was twice as high as that of those with GS. Accurate prenatal diagnosis of GS and OC as well as their differentiation are of crucial importance for both counselling of the parents and the resulting antenatal consequences, and provide the basis for optimal interdisciplinary co-operation at a perinatal tertiary-care center.

    Notes: Abstract Intraspecific variation among 84 isolates of the anamorphic fungusChaunopycnis alba from 26 different geographical locations was analyzed by investigating optimal growth temperatures, differences in the production of secondary metabolites and presence or absence of the cyclosporin synthetase gene. The genetic diversity was assessed using random amplified polymorphic DNA (RAPD). Analysis of these data showed high genetic, metabolic and physiological diversity within this species. Isolates from the Antarctic represented the most homogeneous group withinC. alba and together with isolates from the Arctic these polar strains differed from alpine, temperate and tropical strains by low optimal growth temperatures and by low production of secondary metabolites. Isolates from tropical climes were characterized by high optimal growth temperatures and by the production of comparatively diverse metabolite spectra. Most of the isolates that were similar in the combination of their physiological and metabolic characters were also genetically related. Isolates from different geographical origins did not show many similarities, with the exception of the cyclosporin A-producing isolates, and large diversity could be observed even within a single habitat. This leads us to the suggestion that for pharmaceutical screening programs samples should be collected from a diversity of different geographical and climatic locations. For the selection of strains for screening the RAPD assay seems to be the most powerful tool. It reflected the highest intraspecific diversity and the results corresponded well with the other characteristics.

    Notes: Abstract. During growth, bones change their dimensions rapidly with the changes involving both formation and resorption processes. Small cross-linked peptides coming from type I collagen molecules are excreted in urine when bone is resorbed. To date, conflicting results have been presented concerning the age- and puberty-related changes of urinary markers. The purpose of the present study was to verify the effect of age, gender, and puberty on the urinary excretion of type I collagen degradation products in healthy children and adolescents. Timed spot urines from 176 children (4–20 years old) and 50 young adults were analyzed. The concentrations of N-telopeptides of type I collagen (NTx), pyridinolines (Pyr), and deoxypyridinolines (Dpyr) were measured, and the results were normalized to creatinine. Age-related changes in cross-links excretion were observed. The levels decreased with age, and a peak of excretion was shown at the beginning of adolescence. Prepubertal levels of all the markers were four- to five-fold higher than in adults, and they decreased towards adult levels in late puberty. Girls had significantly higher levels of all biochemical markers than boys at pubertal stage 2. We also observed a remarkable effect of puberty on the levels of bone degradation products that was independent of age and gender. Our results indicate that bone resorption is high in children relative to that in adults, and that urinary levels of NTx, Pyr, and Dpyr change as a function of age, gender, and puberty.

    Notes: Abstract An analytical separation scheme is presented for the isolation of low-molecular-weight metal species (〈 10 kDa) in plants. After ultrafiltration of the aqueous plant extracts isotachophoresis or gel chromatography is used for pre-separation and HPLC at a cyclodextrin-phase for further separation of metal containing fractions. Trace metals are detected off-line by using either adsorptive stripping voltammetry for platinum or AAS for zinc and magnesium. It is shown that platinum and zinc detection closely correlate with pulsed amperometric detection (PAD), while magnesium behaves differently. PAD is carried out after post-column addition of sodium hydroxide, thus enabling a sensitive and selective detection of carbohydrates. It is demonstrated that cyclic voltammetry is a useful tool for further characterization of the PAD-detectable metal species. By comparison of the cyclic voltammograms of carbohydrate standards, including carbohydrate-metal species, with the respective voltammograms obtained from HPLC fractions, carbohydrate species isolated from the samples can be considered as being either partly oxidized sugars (sugar alcohols or sugar acids) or glycosidically bound sugars.

    Notes: Abstract. Despite improvements in surgical practice and postoperative care, the large vertical midline or transverse transperitoneal approaches used in abdominal aortic surgery are still associated with a relatively high perioperative morbidity and mortality rate even in patients who are considered good risks for undergoing aortic surgery. This significant perioperative morbidity is partly caused by the major surgical trauma. To decrease the surgical stress on these patients we have developed a less-extensive procedure for this type of vascular reconstruction. Technique: The abdominal aorta is explored using a special retractor through a short upper median minilaparotomy utilizing modified conventional surgical hand instruments. For an aortobifemoral graft implantation, a retroperitoneal tunnel is necessary. During the creation of this tunnel special care should be exercised to avoid troublesome hemorrhage with iliac and other vein lacerations. To overcome these difficulties, we have developed a new tunneling device, which allows us to have visual control of the tunneling procedure. This prototype device contains a semiflexible tube with an inflatable balloon and a flexible videoendoscope. It is introduced along the external iliac artery into the retroperitoneal space and creates a tunnel through step-by-step inflation of the balloon. After this, the graft is implanted in the usual manner. To date, 19 abdominal vascular reconstructions have been performed with this method.

    Notes: Abstract The multiple endocrine neoplasia type 1 (MEN1) locus has been previously localised to 11q13 by combined tumour deletion mapping and recombination studies, and a 0.5-Mb region, flanked by PYGM and D11S449, has been defined. In the course of constructing a contig, we have identified the location of the gene encoding the B56β subunit of protein phosphatase 2A (PP2A), which is involved in cell signal transduction pathways and thus represents a candidate gene for MEN1. We have searched for mutations in the PP2A-B56β coding region, together with the 5′ and 3′ untranslated regions in six MEN1 patients. DNA sequence abnormalities were not identified and thus the PP2A-B56β gene is excluded as the candidate gene for MEN1. However, our precise localisation of PP2A-B56β to this region of 11q13 may help in elucidating the basis for other disease genes mapping to this gene-rich region.
