Notes: Abstract Electrophysiological experiments in anesthetized cats and rats were performed in order to study the effects of dipyrone on single afferent fibers from the knee joint and on spinal cord neurons with knee joint input. The neurons were activated and/or rendered hyperexcitable by an acute inflammation in the knee joint. In the joint nerve in cats, intravenous dipyrone (25–100 mg/kg) reduced ongoing activity in 10/12 thinly myelinated afferents but only in 1/10 unmyelinated afferents; the responses to movements of the inflamed knee were reduced in 8/10 thinly myelinated but only in 3/10 unmyelinated units. The reduction of activity was significant 20–30 min after application and was maximal at 60–180 min. In the spinal cord of spinalized cats, intravenous dipyrone (25–100 mg/kg) reduced ongoing activity and/or responses to pressure onto the inflamed knee in 14/16 neurons and in non-spinalized rats similar effects were seen in 10/11 neurons. Effects on spinal cord neurons started 5–10 min after application and were maximal after 20–40 min. These data show pronounced suppression of inflammation-induced nociception by dipyrone and they suggest that the spinal cord is a major site of action of this compound.

Notes: Abstract Implemented on behalf of the Federal Ministry for Research and Technology (BMFT), a model is developed to trace the nutrient flow of nitrate in the soil and the groundwater on a supraregional scale. Research work is intended to indicate regionally differentiated hazardous potentials and thereby provide a basis for recommending comprehensive measures to protect groundwater in Germany. The adaption of the model to the hydrogeological and agricultural conditions of other states is possible in principle. This article focuses on the hydrogeological model parts. A high nitrate pollution of groundwater can be expected in all regions with intensive agricultural use of the topsoil. In particular, groundwater in solid rock areas is susceptible to nitrate pollution. There a rapid groundwater turnover and thus a short residence time for the groundwater in the aquifer is typical. Oxidizing aquifer conditions usually prevail in solid rock aquifers, preventing nitrate degradation. In many loose rock areas, in contrast, the groundwater has a low flow velocity and a long residence time in the aquifer. Because of a lack of free oxygen, a complete degradation of nitrate can occur, as long as iron sulfide compounds and/or organic carbon are available in the aquifer. A more detailed presentation of the whole research work is given in Wendland et al. (1993).

Notes: Summary Subclinical thyroid disorders have received increasing attention in recent years due to refined laboratory methods and a stronger emphasis on the role of preventive medicine. We performed a screening for thyroid-stimulating hormone (TSH) on 6884 persons in a working population. In cases in which TSH was not within the normal range we also measured the levels of triiodothyronine (T3), thyroxine (T4), and thyroxine-binding globulin (TBG). All persons who did not present with exclusion criteria or other nonthyroidal illnesses (n = 59) and the controls (n = 39) were submitted to thyrotropin-releasing hormone (TRH)-testing. Additionally, sonography of the thyroid was performed on 120 persons (59 subjects with abnormal hormone levels and 61 controls) to determine thyroid size and rule out morphological abnormalities. Based on the TRH test and T3, T4, and TBG measurements we found a prevalence of 0.03% (2/6884) for overt hyperthyroidism, 0.33% (23/6884) for subclinical hyperthyroidism, 0.09% (6/6884) for subclinical hypothyroidism, and 0.015% (1/6884) for overt hypothyroidism in the healthy population. In subjects with overt or subclinical hyperthyroidism the prevalence of goiters (thyroid volume 〉 18 ml in women, 〉 25 ml in men) was 28%. Of this group 48% had structural abnormalities. All persons with goiters and/or structural abnormalities were over 35 years of age. Among the euthyroid, 20% had thyroid enlargement, and the same proportion presented with structural abnormalities. There were no differences between the two age groups. In the group with overt/subclinical hypothyroidism 47% presented with structural abnormalities of the thyroid; however, none presented with thyroid enlargement. Thyroid nodules were found only in older persons (〉 〉 35 years) with euthyroidism or hypothyroidism. These data confirm the relatively high prevalence of functional and morphological abnormalities of the thyroid. An early substitution with iodine is warranted to prevent functional and morphological disorders of the thyroid in older age. People with subclinical hyperthyroid disorders must avoid exposure to iodine, which can cause an exacerbation of the disease.

Notes: Summary The effects of β -phorbol 12, 13-dibutyrate (PDBu) on the discharge properties of slowly conducting knee joint afferents (group III and group IV fibers) were studied to determine the role of protein kinase C in nociception. Extracellular single unit recordings were made from small filaments dissected from the medial articular nerve in cats anesthetized with alphachloralose. PDBu was applied intra-arterially close to the joint in concentrations of 10-6 up to 10-4 M. The afferents were classified as low-threshold and high-threshold units with regard to their sensitivity to passive noxious and innocuous movements of the knee joint. Following PDBu application, an excitation occurred in 28% of the group III and in 40% of the group IV fibers. An enhancement of responses to passive movements of the joint (sensitization) occurred in 37% of group III and 19% of group IV afferents. In summary, 37.5% of the low-threshold and 50% of the high-threshold fibers proved to be sensitive to PDBu. Most of the PDBu-positive units responded also to bradykinin, whereas only a few PDBu-positive units were sensitive to prostaglandin I2 and E2. We conclude from these results that, in a distinct population of slowly conducting joint afferents, protein kinase C is likely to be involved in the process of transduction. Thus, pain and hyperalgesia may be mediated at least partly by intracellular mechanisms that are linked to protein kinase C.

Notes: Abstract The dynamics of coupled biological oscillators can be modeled by averaging the effects of coupling over each oscillatory cycle so that the coupling depends on the phase difference φ between the two oscillators and not on their specific states. Average phase difference theory claims that mode locking phenomena can be predicted by the average effects of the coupling influences. As a starting point for both empirical and theoretical investigations, Rand et al. (1988) have proposed dφ/dt=Δω — K sin φ), with phase-locked solutions φ=arcsin(Δω /K), where Δω is the difference between the uncoupled frequencies and K is the coupling strength. Phase-locking was evaluated in three experiments using an interlimb coordination paradigm in which a person oscillates hand-held pendulums.Δω was controlled through length differences in the left and right pendulums. The coupled frequency ωc was varied by a metronome, and scaled to the eigenfrequency ωv of the coupled system K was assumed to vary inversely with ωc. The results indicate that: (1) Δω and K contribute multiplicatively to φ (2) φ =0 or φ = π regardless of K when Δω=0; (3) φ ≈ 0 or φ ≈ π regardless of Δω when K is large (relative to Δω); (4) results (1) to (3) hold identically for both in phase and antiphase coordination. The results also indicate that the relevant frequency is ωc/ωv rather than ωc. Discussion high-lighted the significance of confirming φ=arcsin(Δω/K) for more general treatments of phase-locking, such as circle map dynamics, and for the 1∶1 phase-entrainment which characterizes biological movement systems.

Notes: Abstract The records of 71 patients with cosmetic breast implants or reconstructed breast following surgery for cancer were reviewed restrospectively. Of these 47 (66%) had cosmetic augmentation and 24 (33%) had reconstructive surgery. Analysis of radiological findings and correlation with clinical data showed recurrence of certain patterns and assessed their clinical relevance. Evaluation of mammographic views proposed by Eklund et al. [1] was also made. The role of US, CT, and invasive procedures was also discussed.

Notes: Abstract In order to gain insight into the representation of articular pain of the knee at the supraspinal level, recordings were made from lateral thalamic neurons receiving input from afferent fibres of the knee joint in chloralose-anaesthetized cats. Dorsoventral penetrations were made through the ventral posterior lateral nucleus (VPL) using high intensity electrical stimulation of the medial articular nerve (MAN), which contains a high proportion (80%) of Aδ and C afferent fibres. All recording sites were verified histologically. Close retrograde injections (300 μl over 6 s) into geniculate artery of KCl (2 × isotonic), bradykinin (BK, 2.6 or 26 μg) and capsaicin (200 μM) were used to test the response properties of thalamic neurons. Of the 50 MAN-positive units tested, 20 showed a response to intra-arterial KCl; of these 20, 12 had a response to BK; 8 of these 12 units were additionally tested with capsaicin and all responded. KCl and capsaicin injections had similar mean response latencies (4.5 and 6.8 s), whereas BK had a longer mean latency (18.6 s). The mean peak response was greatest for capsaicin (168 impulses/s), then KCl (87.5 imp/s) and least with BK (36.4 imp/s). The mean response duration was longest with capsaicin (118 s), followed by BK (67.5 s) and least with KCl (27.9 s). Most of these were convergent wide dynamic range (WDR) neurons with a deep receptive field in the knee joint and hindlimb muscle and/or cutaneous distal hind limb digit, located to the dorsal or ventral periphery of the lateral division of the VPL, the VPLl. In addition, 8 neurons showed inhibitory responses to KCl and/or BK injections. The background activity of the VPLl neurons activated by saphenous nerve stimulation was inhibited by the nociceptive articular stimulus with a magnitude and time course which mirrored the excitatory responses in the periphery of VPLl. These results support the concept that the lateral thalamus plays an important role in mediating discriminative aspects of joint pain.

Notes: Summary Physicians looking for a position as pharmaceutical physician are usually uncertain whether they are suitable for this activity by training and personal abilities. The main requirements are a major interest in therapeutics and research as well as the ability to treat a disease as an entity instead of the individual patient. The pharmaceutical physician is a specialist in the area of drug development. Even by treating populations and diseases his ultimate goal is to contribute to better care for the individual patient. Requirements for training and the personal abilities necessary for the position of a pharmaceutical physician are described and recommendations given for applicants.
