    Notes: Summary The relative mortality of Type 1 (insulin-dependent) diabetes in Denmark during the period 1933–1981 was studied using a modification of Cox's regression model on the basis of two patient populations, ascertained in different ways and independently of each other. Initial analysis showed that the two groups could be combined completely into one common analysis. Relative mortality was the same for both sexes. The additional variables studied were age at diagnosis, current age, calendar year at diagnosis and calendar time during follow-up. All these interrelated variables were accounted for in the analysis. The analysis showed that relative mortality (a) decreased with increasing age at diagnosis; (b) increased from 1933 to a maximum in about 1965, after which it decreased; (c) increased with increased duration of diabetes to a maximum at 15–25 years, after which it declined.

    Notes: Summary We followed 1, 134 patients with Type 1 (insulin-dependent) diabetes, diagnosed between 1933 and 1952, until 1982 or death or until their emigration. Their age at onset of diabetes was under 31 years. Information concerning the development of persistent proteinuria was sought in every case. In 104 cases, the data were either questionable or the patient could not be traced. Twenty-nine patients developed non-diabetic proteinuria. Among the remaining 1,001 patients, 406 developed persistent proteinuria (350 died) and 595 did not (166 died). The incidence of persistent proteinuria was highest among men; it decreased with increasing year of diabetes onset from 1933 to 1952, and decreased with increasing age at onset. The relative mortality was extremely high among patients with persistent proteinuria, increasing to a maximum of about 100 at age 35 years. Patients not developing proteinuria had a relatively constant low relative mortality of about 2. The decreasing incidence of persistent proteinuria and the decreasing mortality with increasing calender year of diabetes onset resulted in a 50% increase in life-expectancy among patients diagnosed in 1950 compared with patients diagnosed in 1935. In patients who developed persistent proteinuria, relative mortality was higher in women than men at all ages. In patients who did not develop proteinuria, relative mortality was similar in men and women after the age of 35. Uraemia was the main cause of death in patients with persistent proteinuria, although cardiovascular deaths were more frequent than in patients without proteinuria. Thus, proteinuria is associated not only with death from uraemia but also from cardiovascular disease. It is concluded that the development of persistent proteinuria is a major life-threatening complication in patients with early-onset Type 1 diabetes. Patients who do not develop proteinuria have almost a normal life expectancy.

    Notes: Summary 15-Hydroxyeicosatetraenoic acid (15-HETE), a 15-lipoxygenase product of arachidonic acid, inhibits leukotriene B4 (LTB4)-induced chemotaxis of polymorphonuclear leukocytes (PMNs) in vitro. In this study the effects of intradermal injections of LTB4 were determined in the absence or presence of 15-HETE. For comparison intradermal injections of purified human complement split product C5a were performed in the absence or presence of 15-HETE. The skin response was evaluated by measuring the diameter of the wheal, the area of the flare and by intensity of the erythema (erythema index). LTB4 and C5a were injected at the concentration of 200 ng/ml. At this concentration the maximal skin response of LTB4 and C5a were equivalent. In contrast to C5a reaction, which resolved within 1 h, LTB4-induced skin response lasted up to 18 h. In all subjects the skin response was significantly decreased when LTB4 was injected together with 300 ng of 15-HETE. The decrease of wheal, flare, and erythema index averaged 81.9%, 56.6%, 53.6%, respectively, when all parameters were obtained at the maximal skin response. In contrast, the C5a-induced skin response was not affected by addition of 15-HETE, even when the final dose of 15-HETE was increased 10 times to 3 μg. The LTB4-induced reaction could last up to 18 h after injection. After the addition of 300 ng of 15-HETE the skin response resolved after 1 h. The present results demonstrate that 15-HETE is a specific inhibitor of the LTB4-induced skin response and brings additional evidence in support of the ability of 15-HETE to regulate the proinflammatory effects of LTB4 in vivo.

    Notes: Superconducting films of Bi2Sr2CaCu2Ox have been fabricated on single-crystal MgO substrates by a low-temperature in situ process. Using a substrate temperature Ts≈645 °C, metallic films with a superconducting onset of 90–100 K and an extrapolated Tc0=56 K have been obtained. X-ray diffraction shows the films to be c-axis oriented. Electron microscopy reveals that the films are not significantly smoother than films which were post-annealed at 865 °C, and that some segregation into nonsuperconducting phases had occurred. The exact mechanism by which crystallization and superconductivity occurs at such low temperatures is not yet known, but it can be speculated that the surface atoms are less constrained and thus have a smaller energy barrier to overcome in forming a crystal structure.

    Notes: The diffusion of ion-implanted Sn and Sb in silicon single crystals during rapid thermal annealing at 1000 and 1050 °C has been studied as a function of P donor concentration. For extrinsic/intrinsic carrier concentration ratios (n/ni )≥20, almost identical, extremely large diffusion coefficients are found for both impurities. The diffusion coefficients exhibit approximately an (n/ni )4 dependence. The onset of this dependence is accompanied by the appearance of a new defect-complex type containing Sb or Sn, respectively.

    Notes: Antibodies to Aspergillus umbrosus were investigated in 181 Danish farmers, 137 farmers spouses and a control group of 104 male blood donors with no relation to farming. By enzyme-linked immunosorbent assay (ELISA) higher levels of IgG antibodies were found in farmers than in their spouses and in non-farming controls (P 〈 0.0001). By double immunodiffusion precipitating antibodies occurred in 11 farmers, in none of farmers' spouses and in one control subject. Farmers and farmers' spouses who were non-smokers had higher IgG antibody levels than smokers in the same groups (P 〈 0.04). Farmers more than 30 years of age had higher IgG antibody levels than younger farmers (P 〈 0.04). Farmers rearing cattle tended to have higher IgG antibody levels than farmers without cattle. No correlation between antibody levels and respiratory symptoms or lung function parameters (FEV1 or FVC) could be demonstrated.

    Notes: Summary The frequency of nontraumatic rhabdomyolysis in diabetic ketoacidosis was investigated by serial measurements of the serum levels of myoglobin and the serum activity of creatine kinase isoenzyme MM in 12 consecutively admitted ketoacidotic patients. In 5 patients (Group 1) we found hypermyoglobinaemia and elevated activity of creatine kinase isoenzyme MM on admission to hospital, whereas these two variables were normal in 7 patients (Group 2). On admission significantly higher median blood glucose levels and higher median serum osmolality were found in Group 1 than in Group 2 (for blood glucose: 49.6 mmol/l versus 19.0 mmol/l, p〈0.02; for serum osmolality: 360 mosm/kg H2O versus 315 mosm/kg H2O, p 〈 0.05). Decreased renal function was found in Group 1 as reflected by significantly higher beta2-microglobulin serum concentrations in Group 1 compared with Group 2 on admission (median values 4.1 mg/l versus 1.7 mg/l, p 〈 0.01) and during the first 3 days of therapy. The serum concentration of hypoxanthine (an indicator of the cellular energy state) was elevated in all patients on admission, with no difference between patients with or without hypermyoglobinaemia. In conclusion, our findings suggest that nontraumatic rhabdomyolysis with hypermyoglobinaemia and elevated serum activity of creatine kinase isoenzyme MM may be a hitherto unrecognized common feature of diabetic ketoacidosis.

    Notes: Abstract The plasma concentrations of fibronectin were zero for 8 days in a 53-year-old male who was submersed for 5 min, but conscious at the time of admission to hospital. The patient developed multiple organ failure disseminated intravascular coagulation and finally died after 15 days. The low fibronectin values indicate prolonged severe reticulo-endothelial failure, and may be a prognostic sign in cases of drowning or asphyxia of other etiologies.
