    Notes: Summary Monozygotic twins, both with Klinefelter's syndrome, are presented. One of them had clinical and serological evidence of systemic lupus erythematosus (SLE), whereas the other did not show any immunological disorder. In relation to this case of discordant SLE the role played by exogenous factors in triggering the manifestation of this autoimmune disease under a certain genetic predisposition is discussed.

    Description / Table of Contents: Zusammenfassung Um kurzfristige Veränderungen der Gehirnfunktion zu erfassen, die durch die extra — intrakranielle Bypassoperation bewirkt wurden, untersuchten wir 17 Patienten vor und nach dieser Operation mittels neurophysiologischer und röntgen — computertomographischer Untersuchungsmethoden. Die Computertomographie hilft auf morphologischer Grundlage das Ausmaß des ischämischen Hirnschadens zu bestimmen und ist in einigen Fällen ein wertvolles Hilfsmittel, um vor der Operation die Chancen für eine klinische Befundbesserung abzuschätzen. Die Elektroenzephalographie zeigt bei der Hälfte der Patienten nach einem Hirninfarkt 2 Wochen nach der Operation, daß der ischämische Schaden zugenommen hat (Zunahme eines Herdhinweises), wohl bedingt durch die herabgesetzte intraoperative Hirndurchblutung bzw. den temporären Verschluß der anastomosierten Kortikalarterie. Unter den angewandten neurophysiologischen Untersuchungsmethoden (somatosensorisch evozierte Potentiale, relative und absolute Reizantwortperiode) scheinen die absolute und relative Reizantwortperiode am ehesten geeignet, bei Patienten mit passagerem neurologischem Defizit die spätere Entwicklung abzuschätzen. Hingegen fand sich keine Korrelation zwischen neurophysiologischen Untersuchungsergebnissen und späterem Verlauf der klinischen Symptomatik für die Gruppe der Hirninfarktpatienten (Tab. II).
    Notes: Summary In order to evaluate short-term effects of extra-intracranial arterial bypass procedure (EIAB) on cerebral function, we performed neurophysiological studies, combined with CT-evaluation, on 17 patients undergoing EIAB, pre- and postoperatively. CT helps to define, on a morphological basis, the extent of ischemic brain damage, and is, in some cases, a valuable tool in preoperatively assessing chances for a recovery. EEG shows, 2 weeks after surgery, in 50% of the patients after completed stroke, worsening of ischemic lesions as a result of reduced intraoperative cerebral perfusion and/or temporary occlusion of the recipient cortical vessel. Amongst somatosensory evoked potentials (SEP), and relative and absolute response periods (RPrel/ RPabs), RP abs and RPrel seem to have the highest potential in predicting later outcome in patients with TIA/RIND, while no correlation could be found between neurophysiological data and later clinical course for the completed stroke group (Table II).

    Description / Table of Contents: Zusammenfassung Zur Therapie der atlanto-axialen Instabilität unterschiedlicher Ätiologie (traumatisch, kongenital, rheumatisch und neoplastisch) wurde eine neue, einfache Methode entwickelt. Nach Implantation eines autologen kortiko-spongiösen Knochenspans in den interlaminären Raum wird beidseits des Spans eine metallene Kompressions-klammer auf die Bögen C1/C2 aufgesetzt. Die technische Konzeption der Klammer mit innerem Federzug garantiert die primäre Segmentstabilität; die interlaminäre Osteosynthese ermöglicht die sichere posteriore atlanto-axiale Spondylodese. Bei drei Patienten hat sich das Verfahren bewährt. Äußere Fixation und langfristige Immobilisation des Patienten sind unnötig. Komplikationen, bekannt von anderen Verfahren zur posterioren Fusion C1/C2, werden vermieden.
    Notes: Summary A new method of dorsal arthrodesis in patients with atlanto-axial instability of different etiology is described. In three patients solid laminar osteosynthesis C1/C2 was achieved by insertion of an autogenous cortico-cancellous bone graft into the interlaminar space and by a bilateral application of the lately developed metal clamp. The technical principles of the clamp guarantee a permanent pressure on the graft and a solid initial stability in the segment. Additional external fixation and immobilization of the patient are not necessary. Complications as known from other procedures of the atlanto-axial dorsal fusion will be prevented efficiently.

    Notes: Summary A CT/T 8800 (General Electric) CT scanner was used to study lumbar disc disease in fifty patients. In seven patients so-called computed tomography mexrizamide myelography was performed, whereas in the remaining cases no contrast enhancement was used. Multiplanar reconstructions were done. Irradiation exposure was measured during the investigations and compared with data during conventional myelography. We found that lumbar CT scanning is not a screening method in cases of lumbar disease, but the aimed use gives further information about the underlying process and its differential diagnosis. Multidirectional reconstructions reduce the number of diagnostic errors. This procedure does not increase the radiation exposure dose in comparison with conventional myelography.

    Notes: Summary The same histological types of tumor are found in multiple as in solitary meningiomas. Multiple meningiomas cannot be considered in every case to be a forme fruste of von Recklinghausen's disease. The incidence of multiple meningiomas in this paper is higher than had been reported before the introduction of computed tomography (CT) into clinical practice. The number of tumor nodes is determined more accurately by CT than by angiography because tumors are detected in patients without neurological deficit. CT has limitations in the verification of small meningioma nodules near the base of the skull and in the differentiation of multiple meningiomas from meningeal meningiomatosis or primary meningeal sarcomatosis.

    Notes: Abstract Following genetic counselling of a consanguineous couple because of a daughter born with peripheral hypoplasia of the left arm, ultrasonographic examination during the second pregnancy revealed marked hydrocephalus of the fetus in the 17th week of pregnancy. Pathologic examination of the female fetus disclosed severe cerebral, cardiac, and skeletal malformations including holoprosencephaly, absent corpus callosum, microphthalmia, facial clefts, tetramelic octodactyly, and cardiac defects. These findings indicate a possible genetically determined syndrome that appears to be distinct in spite of some overlap with other malformation syndromes.

    Notes: Abstract A case is described in which the family history and the sonographic findings during the 17th gestational week led to the suspicion of a short-rib-polydactylia syndrome of the Saldino-Noonan type and an abortion was induced. The ultrasound image disclosed cystic structures in the topographic area of the kidney, marked ascites and oligohydramnios and indicated a severe fetal developmental disturbance. Due to the oligohydramnios the extremities could not be interpreted sonographically. The pathological-anatomical findings confirmed the suspected short-rib-polydactylia syndrome of the Saldino-Noonan type. Typical skeletal changes, dysgenetic cystic kidneys and pancreatic fibrosis were especially indicative of the syndrome. The complete agenesia of the islets of Langerhans, present in our observation, has, as far as we know, previously not been reported in the case of short-rib-polydactylia syndromes.

    Notes: Summary Five cases of T-zone lymphoma were investigated with histologic, immunologic, and cytogenetic methods. The chromosome analyses were performed on lymphoma cells prepared immediately after removal of the lymph nodes. The chromosomes involved in structural rearrangements were nos. 1, 2, 3, 4, 14, and Y. Numbers 3, 5, 6, and 13 were lost by some tumors, and nos. 3 and 9 were gained. Chromosome 3 was involved most often in structural and numerical aberrations, whereas 14q+ markers occurred in only one case. The importance of multidisciplinary studies is pointed out.

    Notes: Abstract Scintiphotographic studies as a method for analyzing the brain perfusion in carotid cavernous sinus fistulae before and after surgical treatment with different techniques are presented. Twelve patients have been reexamined long after their operation to prove the sufficient crossover circulation through the circle of Willis while the homolateral carotid artery is impatent. Sufficient perfusion of both hemispheres could be well-documented by this radionuclide method. No neurological deficit was found in clinical examination either.
