    Description / Table of Contents: Zusammenfassung Die Prognose der in ökonomisch hoch entwickelten Ländern evident zunehmenden colorectalen Carcinome hängt entscheidend von frühzeitiger Diagnosestellung und Therapie ab. Die diagnostischen Möglichkeiten zur Früherkennung des Dickdarmcarcinoms werden aufgezeigt. Zur Therapie wird anhand von zwei anterioren Rectumresektionen unsere Operationstechnik mit konventioneller 3reihiger Naht und mit maschineller Anastomosierung gezeigt. Der Verzicht auf ein protektives Colostoma wird durch die niedrige Rate der Anastomoseninsuffizienzen (3,6% unter 1012 Colon- und Rectumresektionen) gerechtfertigt.
    Notes: Summary The prognosis of colorectal carcinoma in economically developed countries depends on early diagnosis and treatment. The different diagnostic procedures for early detection of carcinoma are shown. Our technic of resection of the large bowel without colostomy with the “3-rows suture technique” and with using a machine to form the anastomosis is demonstrated in two cases with anterior rectum resection. We do not use protective colostomy because of our low rate of anastomosis breakdown (3.6% among 1,012 resections of the large bowel).

    Description / Table of Contents: Zusammenfassung Häufigste Ursache der portalen Hypertension ist die Lebercirrhose, hierbei führend die alkoholtoxisch bedingte Cirrhose. Die wichtigsten Komplikationen der portalen Hypertension sind die akute Blutung aus Oesophagusvaricen, der Ascites, die Encephalopathie und die Splenomegalie. Es werden die derzeitigen Möglichkeiten der operativen Therapie aufgezeigt und der Indikationsbereich der operativen und konservativen Therapie abgegrenzt. Neue chirurgische Behandlungstechniken werden vorgestellt und ihre Ergebnisse anhand unseres großen Krankengutes demonstriert.
    Notes: Summary The main cause of portal hypertension is cirrhosis of the liver, most often alcoholic cirrhosis. The main complications of portal hypertension are hemorrhage due to esophageal piles, ascites, encephalopathy and splenomegaly. The modern operative treatment of portal hypertension and the indication of operative and nonoperative treatment are explained. New operation techniques are demonstrated, and their results in patients operated on in our hospital are discussed.
