    Notes: Summary Substitution of insulin for a dietary regime with or without oral hypoglycaemic agents prior to operation is, despite its general use, often not satisfactory because of lack of time and leads frequently to hypoglycaemia and sometimes to excessive insulin-antibody production. We therefore administered 1.0 g of Tolbutamidei.v. together with at least 100 g of carbohydrates per day to 36 non-insulin dependent diabetic patients, and compared the results with those of 28 similiar cases switched to insulin preoperatively in the usual way (I), and those of 27 patients receiving insulin before, during and after the operation (II). Interventions in the group on Tolbutamide i.v. (III) were more severe and lasted longer than those in the control groups. Blood-sugar levels on the operation day rose to 209 ± 76 mg% in the Tolbutamide group, (Control group I 203 ± 57, II 186 ± 52). Hypoglycaemia was observed in the control groups only. The method proposed avoids unnecessary insulin treatment and its consequences, and can be recommended, with correct selection of patients, for further evaluation during minor and more extensive operations.

    Notes: Summary The generalized form of von Winiwarter-Buerger's disease (WBD) occasionally involves the brain. However, pure cerebral forms of the disease were also described by Spatz and Lindenberg (“Spatz-Lindenberg's disease”, SLD). Both, the type I, which involves the large basal arteries, and the type II, which results in a sickle-shaped granular atrophy of the cerebral cortex, are often accompanied by (“vascular”) dementia, which Lindenberg and Spatz mainly attributed to the bilateral involvement of the second frontal gyrus by granular atrophy. Recently, synaptic deprivation of the cortical gray matter has been shown to occur in the dementia of Alzheimer type (DAT) and other neurodegenerative disorders. In DAT, the synaptic loss highly correlated with the degree of the mental impairment. We wanted to examine whether similar changes also occurred in dementia of vascular origin, for which SLD, although infrequent, is a typical example. In fact, we found that in three cases of typical SLD type II the synaptophysin immunoreactivity of the cortical neuropil in areas without overt infarcts or scar formation was as much reduced as in Alzheimer's disease. Although it must be taken into account that in the present cases the synapse loss might, at least in part, be due to secondary (Wallerian) degeneration as a result of the neuronal loss in the “watershed” regions of the arterial blood supply, it cannot be excluded that a decline of cortical synaptic contacts in areas without necroses or scars may occur as a primary event, contributing to the pathogenesis of the dementia. Final conclusions can only be expected from investigations into further cases of cerebro-vascular disorders with and without dementia.

    Description / Table of Contents: Summary The mean cortisol secretion rate was significantly elevated in fifteen patients with acromegaly. The increase was especially pronounced in untreated patients while in treated ones the mean value was close to that of normal persons. Calculated as secretion rate per kg body weight there was also a significant elevation in patients with untreated acromegaly. Conversely, in treated patients the value was in the low normal range. In four patients with active acromegaly the cortisol secretion rate per kg body weight was significantly higher than in those with an inactive, untreated pituitary process. The corticosterone secretion rate in nine investigated patients with acromegaly was also significantly increased, both in absolute terms and expressed per kg body weight.
    Notes: Zusammenfassung Die mittlere Cortisolsekretion war bei 15 Akromegaliepatienten signifikant erhöht. Die Steigerung war besonders ausgeprägt bei den unbehandelten Patienten, während sich der Mittelwert bei den behandelten dem der Normalpersonen annäherte. Bezogen auf das Körpergewicht fand sich bei den unbehandelten Akromegalen ebenfalls eine signifikant erhöhte Cortisolsekretion. Bei den behandelten lag sie dagegen im untersten Normbereich. Bei vier Patienten mit einer floriden Akromegalie ließ sich gegenüber denjenigen mit einem unbehandelten, inaktiven HVL-Prozeß eine signifikant erhöhte Cortisolsekretion pro kg Körpergewicht nachweisen. Die Corticosteronsekretion war bei neun untersuchten Patienten absolut und bezogen auf das Körpergewicht signifikant gesteigert.

    Description / Table of Contents: Summary Experiences with a New Test-strip for Bloodsugar Determination. Blood-sugar values were estimated in 643 in- and out-patients with the new test-strip “Haemo-Glucotest”. Blood-sugar determinations performed with the Auto-Analyzer (Hoffmann) served as an objective control. The treatment of out-patients was determined according to the values of the “Haemo-Glucotest” strip and before the Auto-Analyzer results arrived. 80% of the estimated values varied from the Auto-Analyzer results by less than ± 20 mg-%. Preliminary therapy had to be modified in only 6% of the patients. The precision of the determination depends upon illumination and the ability for color discrimination of the person working with the test-strip. The “Haemo-Glucotest” strip is not only suitable for detection of hyperglycemia and hypoglycemia, but also for control of therapy in diabetic out-patients.
    Notes: Zusammenfassung Der Teststreifen Hämo-Glucotest wurde an 643 stationär und ambulant behandelten Diabetikern erprobt. Als objektiver Vergleich dienten die Blutzuckerbestimmungen nachHoffmann im Autoanalyzer. Die Behandlung der ambulanten Patienten wurde nach den Ergebnissen des Hämo-Glucotest vor Kenntnis der Autoanalyzerbestimmung festgelegt. 80% der geschätzten Werte wichen um weniger als ± 20 mg-% von den durch Autoanalyzer bestimmten Blutzuckerwerten ab. Die nach den Testergebnissen vorläufig festgelegte Therapie der ambulanten Patienten brauchte nur in 6% der Fälle korrigiert zu werden. Die Genauigkeit der Bestimmung ist von den Beleuchtungsverhältnissen während des Tests und vom Farbunterscheidungsvermögen des Untersuchers abhängig. Der Teststreifen eignet sich nicht nur zur Erfassung von Hypo- und Hyperglykämien, sondern auch für eine orientierende Blutzuckerbestimmung bei der ambulanten Einstellungskontrolle von Diabetikern.

    Notes: Abstract The temperature oscillation accompanying the photoacoustic effect generates a periodic variation of the vapor pressure of a liquid. The propagation of the oscillating concentration of the vapor in the inert cell gas (air) is described by a mass diffusion wave on which a convective motion of the gas is superposed. The diffusion wave characterized by the diffusion coefficient of the cell gas alone can be measured by the Mirage effect, whereas a microphone detects the total mass flux including the convective flux, which increases with temperature. On approaching the boiling temperature, the convective flow will govern the oscillating transport of mass. The photoacoustic signal is determined directly from the flux of heat and mass at the boundary between liquid and gas using the Gauss' divergence theorem. We have found that the temperature behaviour of the amplitude and phase angle of the photoacoustic signal depends on the length of the gas column in the cell. The contribution of thermal expansion to the photoacoustic signal is considered using the composite piston model. The results of the calculations agree fairly well with the experimental data.
