Notizen: Summary The effect of calcium infusion on the intravenous glucose tolerance was tested in hypocalcaemic patients suffering from renal insufficiency. It was shown that the delayed glucose disappearance rate in uraemics could be improved (p 〈 0.005) by the infusion of calcium gluconolactobionate despite the fact that the serum concentrations of potassium, urea nitrogen, and the blood pH were not altered. The basal insulin concentration was significantly depressed by the calcium infusion (p 〈 0.02). The serum calcium concentration was significantly correlated to the glucose assimilation coefficient in the uraemic patients (p 〈 0.01). — 3 hypocalcaemic patients without renal failure had a normal glucose tolerance and a normalization of the serum calcium concentration had no discernable effect. — A slight but significant decrease of the serum calcium concentration (p 〈 0.01) by EDTA-Na2 in normocalcaemic patients did not change the intravenous glucose tolerance. — It is concluded that hypocalcaemia may be one of the causes for the abnormal glucose tolerance in chronic renal failure.

Notizen: Summary In isolated human fat cells of the greater omentum and the mammary gland, the effect of glucose, fructose, and/or insulin was tested on the re-esterification rate of FFA measured by the balance method. It could be shown that in the absence of glucose no re-esterification activity was demonstrable. Glucose alone or fructose alone stimulated the re-esterification of FFA dose-dependently in isolated fat cells of the greater omentum, and to a minor degree in fat cells of the mammary gland. Insulin had no effect on the re-esterification rate of FFA in the presence or absence of glucose or fructose, whereas it significantly stimulated the incorporation of glucose-C into CO2 and lipids. It is concluded that the re-esterification of FFA in human adipose tissue, at least in vitro, is mainly controlled by glucose without need for insulin.

    Thema: Medizin
Notizen: Summary Hepatic candidiasis represents an increasingly difficult problem in immunocompromised patients. It has been recognized as a variant of disseminated candidiasis after a period of neutropenia. Establishment of diagnosis is difficult since liver biopsy as well as culture may be negative, in spite of the presence of fungus. Hepatic lesions are generally not detectable by ultrasound examination or computed tomography until the neutrophil count has returned to normal. The aim of this case report is to describe the problems related to the diagnosis and treatment of hepatic candidiasis following chemotherapy for acute leukemia. The report also provides further evidence that fluconazole, a new broad-spectrum antifungal drug, can be safely and effectively used for the long-term treatment of deep-seated hepatic candidiasis.
