    Notizen: Abstract Background: The purpose of the present study was to describe the various imaging features of primary malignant fibrous histiocytoma (MFH) of the liver, a rare tumor of mesenchymal origin. Methods: Sonography (n= 5), computed tomography (CT; n= 5), magnetic resonance (MR) imaging (n= 2), and hepatic arteriography (n= 3) in five patients who underwent partial hepatectomy for tumor resection were retrospectively reviewed and correlated with pathologic findings. Results: All tumors were clearly demarcated from surrounding hepatic parenchyma in sectional imaging with (n= 2) or without (n= 3) a fibrous capsule, which was pathologically verified. Internal architecture of abundant fibrosis, myxoid degeneration, and/or hemorrhagic necrosis reflected the sonographic, CT and MR imaging findings. Marginal tumor staining without definite tumor vasculature was the main feature of hepatic arteriography. There was no intratumoral calcification. All three tumors involving the right lobe of the liver invaded the right hemidiaphragm. Conclusion: Although there were no unique findings of primary hepatic MFH, a combined interpretation of various imaging modalities may elucidate the malignant nature of the tumor.

    Notizen: Abstract Background: To investigate radiologic and pathological features of intestinal tuberculosis with abdominal complications. Methods: Twenty-two patients with 23 surgically proven complications (nine intestinal obstructions, eight perforations, three fistulae, and three intestinal bleeds) were analyzed. Medical records, radiologic studies, and pathologic examinations were reviewed with special emphasis on searching for the common features in each group of complication. Results: The most important single feature in seven of the nine patients with intestinal obstruction was the presence of stricture. In the remaining two patients, bowel adhesion was a primary cause of obstruction. In eight patients with intestinal perforation, both obstruction and ulcerations in the dilated proximal loop were the important features in six, and multiple deep ulcerations without obstruction was a primary cause in the remaining two. The common features in three patients with fistulae were focal or multiple strictures, severe adhesions, and fibrotic bowel wall. Intestinal bleeding originated from diffuse mucosal ulcerations. The abdominal complications occurred during antituberculous therapy in 10 of the 22 patients. Conclusion: Understanding the radiologic and pathologic features of intestinal tuberculosis with complications help in making an appropriate clinical decision for the treatment strategy. Close observation is necessary, especially in those patients who are acutely ill during antituberculous medical therapy.

    Notizen: Abstract Background: To evaluate the computed tomographic (CT) features of colonic wall thickening in cirrhotic patients and to determine their prognostic value. Methods: We retrospectively reviewed 28 cirrhotic patients with colonic wall thickening (≥10 mm) on CT. Twenty-six of the 28 patients had hepatocellular carcinoma. The severity of hepatic dysfunction was determined by using the Child–Pugh classification. We analyzed the patterns of bowel wall thickening and degree of portal hypertension on CT and the survival periods after initial CT detection of colonic wall thickening. Results: The involved segment of the colon was diffusely thickened with either scalloped or nodular circumferential configuration. In all patients, the thickened colonic wall enhanced poorly. Although the ascending colon was involved in all patients, the transverse (n= 14) or descending (n= 5) colon was also simultaneously involved. Most patients exhibited an advanced stage of portal hypertension on CT. The median survival period of 25 patients who expired was 34 days, and 21 patients (84%) expired within 3 months. Conclusions: Colonic wall thickening on CT can be used as one of the indicators of poor prognosis in cirrhotic patients. Advanced liver cirrhosis with significantly severe dysfunction is the likely cause of mortality. Therefore, a less aggressive therapeutic approach is recommended if hepatocellular carcinoma is coexistent in these patients.

    Notizen: We propose an alternative mutual fund performance index which addresses the benchmark problem and controls for economies of scale in managing mutual funds. We advance a new concept of ‘return-cost’ efficiency as another important element in evaluating portfolio management, in addition to the mean-variance efficiency concept. Our index based on a non-parametric estimation is shown to be similar to the Sharpe index with multiple slopes (or factors). We have shown that all fund categories, except income funds, have similar average efficiency scores after controlling for economies of scale. Most funds operate in increasing returns to scale and seem to be successful in holding mean-variance efficient portfolios, but unsuccessful in allocating transaction costs efficiently, evidenced by excessive turnovers and loads.

    Notizen: We analyze firms’ investment and abandonment decisions when both output price and investment cost change stochastically. The model allows for and makes endogenous the abandonment decision, thereby incorporating irreversibility. We show that the investment trigger may be much higher than the standard net present value rule suggests even when a substantial portion of the investment cost may be recovered. Further, we argue that the correlation between output price and investment costs significantly affects the effect of irreversibility on investment behavior. Empirical implications are discussed with extensive numerical illustrations, along with an application to the banking industry.
