Summary In 4 patients with chronical potassium depletion of various origin we found a resting membrane potential of −99.6 (s=± 5.9) mV, a threshold current of 4.37 (s=±2.18) · 10−8 A and a latency time of 27.2 (s=±10.2) msec. In comparison to controls in healthy persons the resting membrane potential was increased by 16.0 mV [normal: −83.6 (s=±4.5) mV] (p〈0.001). The threshold current was statistically not significant changed. The latency time was increased by more than 100%, that means by 15.3 msec [normal: 11.8 (s=± 2.91) msec] (p〈0.001). In conditions of a general potassium depletion an increased resting membrane potential doesn't mean in any case an increase of the threshold current, or a decrease of the excitability. The increase of the latency time means a decrease of the ionic conductivity under conditions of a decreased extracellular potassium concentration in general potassium depletion. The decrease of the ionic conductivity can be an explanation for the fact, that in patients with chronical general potassium depletion despite an increased resting membrane potential a change of the excitablity needs not to exist.

Zusammenfassung Bei 4 Patienten mit ausgeprägtem chronischen allgemeinen Kaliummangel verschiedener Ätiologie fanden wir bei einem Membranruhepotential von —99,6 (s=±5,9) mV eine Schwellenreizstromstärke von 4,37 (s=±2,18) · 10−8 A und eine Latenzzeit von 27,2 (s=±10,2) msec. Gegenüber Kontrolluntersuchungen an gesunden Personen war das Ruhepotential um 16,0 mV [normal −83,6 (s=±4,5) mV] deutlich erhöht (p〈0,001). Die Schwellenreizstromstärke war statistisch nicht signifikant verändert. Die Latenzzeit war um mehr als das Doppelte, nämlich um 15,3 msec [normal 11,8 (s=±2,91) msec], erhöht (p〈0,001). Unter den Bedingungen eines allgemeinen Kaliummangels braucht also ein erhöhtes Membranruhepotential nicht zwangsläufig auch mit einer Erhöhung der Schwellenreizstromstärke, also einer Abnahme der Erregbarkeit verknüpft zu sein. Die Verlängerung der Latenzzeit bedeutet eine Abnahme der Ionenleitfähigkeit bei erniedrigter extracellulärer Kaliumkonzentration im allgemeinen Kaliummangel. Diese Abnahme der Ionenleitfähigkeit kann eine Erklärung dafür sein, warum bei Patienten mit chronischem allgemeinen Kaliummangel bei einem erhöhten Membranruhepotential keine Änderung der Erregbarkeit zu bestehen braucht.
