    Notes: Abstract. An internal reflection mode is introduced for scanning near-field optical microscopy (SNOM) with the tetrahedral tip. A beam of light is coupled into the tip and the light specularly reflected out of the tip is detected as a photosignal for SNOM. An auxiliary STM mode is used to control the distance during the scanning process and to record the topography of the sample simultaneously with the SNOM image. Images were obtained of different metallic samples which show a contrast in the order of 10% of the total reflected photosignal. In images of metallic samples an inverted contrast is consistently obtained compared to images previously obtained of comparable samples in a transmission mode. The contrast shows a pronounced dependence on the polarization of the incident beam with respect to the orientation of the edges of the tip. In the case of gold surfaces, the photosignal as a function of distance between the tip and the surface shows a pronounced peak in the near-field range of 0–20 nm which is tentatively attributed to the excitation of surface plasmons on the gold surface. The pronounced near-field effects and the strong contrast in the near-field images and the resolution well below 50 nm are an indication of a highly efficient coupling of the incident beam to a local excitation of the tip apex which is essential for the function of the tip as a probe for SNOM.

    Notes: Abstract  In recent years, significant improvements have been made in the management of neutropenia and thrombocytopenia and other potentially life-threatening complications of ablative chemotherapy. While these complications are of particular concern to physicians, patients receiving ablative therapy for bone marrow or blood stem cell transplants are often troubled by other side effects such as nausea, vomiting, diarrhea and mouth sores. The purpose of the study was to gain a better understanding of patients' experiences while undergoing a transplant. The same professional medical interviewer conducted in-depth interviews with 38 subjects (10 men, 28 women; mean age 46.9 years) who had received ablative therapy for bone marrow and/or peripheral blood stem cell transplants. Participants were consecutively identified through physician and patient referrals, cancer and BMT patient support groups, and newspaper advertisements. Twenty-eight patients (74%) received autologous stem cell transplants and 10 patients (26%) received allogeneic transplants. Participants reported mouth sores, nausea and vomiting, diarrhea, and fatigue as the most troubling side effects of their transplants. Mouth sores were selected as the single most debilitating side effect (42%), followed by nausea and vomiting (13%). Many patients mentioned that mouth sores made it difficult or impossible to eat (n=23), swallow (n=21), drink (n=17), and/or talk (n=8). Twenty patients reported pain in the mouth, throat, and/or esophagus. Two-thirds (66%) of patients reported receiving opioid analgesics, most frequently morphine, to relieve oral pain. For many, opioids caused incapacitating side effects, including hallucinations, a feeling of loss of control and a decrease in mental acuity. Patients receiving ablative chemotherapy identify oral mucositis as a significant cause of suffering and morbidity. Effective interventions to alleviate this complication are urgently needed.

    Notes: Abstract. Patterned bleaching of a photolabile monomolecular dye film by means of scanning near-field optical microscopy (SNOM) is demonstrated. After exposure, the written patterns were verified by SNOM with fluorescence detection. The adsorption of appropriate material to such near-field optically created patterns seems now feasible. The ultimate resolution limit of a monomolecular dye film for patterned bleaching by SNOM is discussed on the basis of a simple model.

    Notes: Abstract A series of numerical studies on the behaviour of magnetic fields and motions in a spherical body of an electrically conducting incompressible fluid have been carried out. The magnetic field was assumed to be maintained by a given electromotive force inside the body and to continue as a potential field in outer space. In view of the motion an external forcing was taken into account, and boundary conditions were considered which correspond to a stress-free surface. The stability of several steady states has been studied as well as the evolutions starting from unstable states. In this paper a configuration with a poloidal magnetic field and a differential rotation, both symmetric about the same axis, is considered. This configuration is stable only for sufficiently small Hartmann numbers but evolves, if disturbed, in the case of larger Hartmann numbers toward a non-axisymmetric state. In this case the well-known symmetrization effect of differential rotation in magnetic fields is destroyed.

    Notes: Zusammenfassung Es wird die Frage der enterogenen Infektion der Niere einer eingehenden Prüfung unterzogen. Es wurde von der Annahme ausgegangen, daß Stagnationsprozesse am Dick- und Mastdarm geeignet wären, eine Niereninfektion hervorzurufen. Da das Carcinom hierfür die besten Voraussetzungen zu bieten schien, wurde das klinische Material der Chirurgischen Universitätsklinik Würzburg in dieser Hinsicht ausgewertet. Dabei wurden bei 492 Fällen von Dickdarm- und Mastdarmcarcinom 380mal (=77%) eine Harninfektion gefunden. Bei 205 Rectumcarcinomen wurde außerdem festgestellt, daß die hierbei vorhandene Harninfektion 135mal (=66%) bereits präoperativ entstanden ist, womit bewiesen ist, daß die Infektion im wesentlichen nicht erst durch die Operation hervorgerufen wird. Bei 13 Dickdarm- oder Rectumcarcinomen wurde diese Feststellung durch kulturelle Urinuntersuchungen vor der Operation weiter erhärtet. Auf den schleichenden Charakter der enterogenen Infektion wird hingewiesen und damit ihre häufige klinische wie pathologisch-anatomische Nichtbeachtung erklärt. An Tierversuchen konnte durch Anlegen eines bilateralen Blindsackes am Colon ascendens bei allen Tieren eine Pyelonephritis erzeugt und damit die enterogene Entstehung einer Harnweginfektion bewiesen werden. Bezüglich des Infektionsweges kann auf Grund der histologischen Nierenveränderungen mit großer Wahrscheinlichkeit eine Entzündungsausbreitung auf dem Lymphweg vom Darm zur Niere im SinneFrankes angenommen werden.
