    Notes: Abstract  Removable partial dentures (RPD) can be retained using conical crowns or parallel-sided telescopic double crowns. The purpose of this study was to evaluate and compare the technical failure rate of the two retainer systems. One hundred seventeen dentures made by dentists of the medical school were included. Seventy-four RPD were retained with parallel-sided crowns (n=251) and 43 with conical crowns (n=160). Following the medical report follow-ups from January 1992 to December 1998, technical RPD failures were noted and analyzed. Technical problems occurred during the observation time in 48.8% of the conical retained dentures and 34.2% of the parallel-sided retained dentures. In both cases, loss of cementation was most frequently noted, while loss of the facings occurred only with conical crowns. Other technical failures did not depend on the type of retainer system used. These were most frequently problems with the denture base, e.g., fracture of artificial teeth or the metal framework. We conclude that there were different technical failures of both double crown retainer systems. These problems were not insignificant in number but treatable.

    Notes: Summary We examined renal responses to a pharmacological dosage of human atrial natriuretic peptide (hANP) and the potential interference of nifedipine administration with the effects of hANP on kidney function in healthy subjects and normoglycemic patients with type 1 diabetes mellitus. Ten healthy volunteers (age, 28±1 years) and ten patients (age, 33±2 years; diabetes duration; 14±3 years; HbAI 7.2%±0.2%) were studied. According to a double-blind, randomized, placebo-controlled trial design, three experiments were performed in each subject using the doubledummy technique: placebo only, hANP only, and nifedipine+hANP. As i.v. bolus injection 100 µg hANP was given; nifedipine was applied buccally, at a dose of 10 mg 90 min before and at a dose of 5 mg together with hANP injection. At base-line and in the placebo only experiment, patients did not differ from controls. In the hANP only experiment, in both groups hANP resulted in increased urinary volume and both sodium and chloride excretion (P〈0.05 vs placebo only experiment). In patients, hANP-induced increase in electrolyte excretion was greater than in controls (P〈0.05). In the nifedipine + hANP experiment, hANP-induced changes in renal indexes were enhanced in controls (P〈0.05 vs hANP only experiment) but not in patients. Thus, diuretic response to nifedipine + hANP in patients was decreased in comparison with controls (P〈0.05). In patients, however, nifedipine administration decreased the hANP-induced increase in urinary albumin excretion (P〈0.05 vs hANP only experiment). Creatinine clearance was uninfluenced throughout the experiments. There were similar decreases in blood pressure in patients and controls after nifedipine administration (P〈0.05 vs placebo only experiment). The increase in heart rate after nifedipine was more pronounced in patients than in controls (P〈0.05). Conversely, plasma renin activity was stimulated by nifedipine only in controls (P〈0.05 vs placebo only experiment). In this study hANP had no effect on heart rate, blood pressure, or plasma renin activity. There was a short-term increase in hANP levels in plasma after nifedipine administration in controls (P〈0.05 vs placebo only experiment) but not in patients. In contrast to a previous study, where renal responses to the same pharmacological dosage of hANP were decreased in patients with type 1 diabetes mellitus with HbAI exceeding the normal range, there is no impairment of renal responsiveness to an i.v. bolus of hANP in patients with HbAI within the normal range. Nifedipine and hANP have synergistic effects on kidney function in healthy subjects. It remains to be studied, however, by which mechanism(s) this synergism could be obscured in diabetes patients. Moreover, the increase in hANP levels after nifedipine administration exclusively in controls merits further investigation.

    Notes: Summary ¶ Purpose. In this prospective study the results of multimodal postoperative neuro-imaging were related to the survival of patients with high grade gliomas.  Methods. All 73 patients included underwent microsurgical tumour resection and had postoperative CT and transcranial sonography (TCS) examinations. In addition, 35 of the 73 patients received an early postoperative MRI. Patients were followed up for at least one year.  Findings. At the end of the 7 year study period 56 patients had died. The median survival time was 371 days. Survival rate was significantly higher in patients with anaplastic astrocytomas and inpatients displaying complete tumour resection on MRI (log-rank-test, p〈0.05) or a small postoperative residual tumour bulk on TCS (log-rank-test, p〈0.05). Cox proportional hazards model identified histological tumour grade, postoperative Karnofsky index, complete resection based on MRI and small postoperative residual tumour mass on TCS as independent predictors of survival.  Interpretation. This study demonstrates that early postoperative neuro-imaging has prognostic implications for the survival of patients with high grade gliomas. According to our results postoperative imaging with MRI and TCS is a valuable prognostic with regard to patient survival and should therefore be implemented in postoperative follow-up. It also helps to evaluate the efficacy of adjuvant therapy.

    Notes: Abstract Haemochromatosis is characterised by deposition of iron-containing pigment in various organs, but little is known about possible deposition in the brain and its clinical impact. We therefore investigated 14 patients with hereditary haemochromatosis with MRI, CT and transcranial ultrasound (TCS) and examined them neurologically. In six of the patients dense lesions were found within the lentiform nucleus on CT, all of whom displayed hyperechogenic lesions in the same area on TCS, as did one other patient. In these patients the relative signal intensities of the lentiform nucleus measured by MRI relaxometry were higher. No patient had clinical signs of basal ganglia disorders.

    Notes: Abstract The case of monocygotic twins discordant for a psychotic disorder is presented. An anomaly of the septum pellucidum, a so-called cavum veli interpositi was found in the psychotic twin while his brother showed no such anomaly. Previous studies have shown a higher prevalence of septum pellucidum anomalies in schizophrenic patients. Abnormalities of the septum pellucidum may be associated with disturbed neuronal development in distinct limbic brain areas which cannot yet be visualized yet by brain imaging techniques. The finding of the cavum veli interpositi in the psychotic twin could be incidental; however, it may indicate a dysgenic process in early brain development and, thus, play a significant role in the etiology of psychosis.

    Notes: Summary An intra vitam diagnosis of Baló's concentric sclerosis in a 39-year-old male is described. To our knowledge this is the first reported case of ante-mortem diagnosis based solely on magnetic resonance imaging (MRI) findings. MRI showed features unique to this process. After high-dose intravenous therapy with prednisone, the clinical symptoms improved, and MRI showed marked changes of the concentric lesions.

Computer- und Kernspintomographie erlauben eine wesentlich genauere Abgrenzung des Tumors innerhalb und ausserhalb des Knochens. Diese beiden Verfahren haben zusammen mit der Ultraschalluntersuchung die Diagnostik der Weichteilgeschwülste revolutioniert, da diese vorher auf Röntgenaufnahmen nur in den seltensten Fällen (Lipome, Verkalkungen) darzustellen waren.
    Notes: Summary Conventional plain film roentgenography is the basic examination of bone tumors. Modern digital roentgenography offers technical innovations but no fundamental new diagnostic dimensions. Computerized and magnetic resonance-tomography allow a far better delineation of the tumor spread (staging). These new imaging methods together with ultrasound have revolutionized the diagnosis of soft tissue neoplasms, not visible on plain film roentgenograms except lipomas and calcifications.
