    Notes: Although the eradication of Helicobacter pylori infection benefits patients with gastric or duodenal ulcers, the value of eradicating the infection in the patients with functional dyspepsia (FD) remains controversial.〈section xml:id="abs1-2"〉〈title type="main"〉Aims:To determine whether eradicating H. pylori can prevent the subsequent development of ulcers or relieve the symptoms of functional dyspepsia patients.〈section xml:id="abs1-3"〉〈title type="main"〉Methods:In a double-blind, placebo-controlled trial, 161 patients infected with H. pylori who had functional dyspepsia were randomly assigned to 7 days of treatment with a lansoprazole-based triple therapy or placebo and then followed for 1 year. The main outcome measures were the development of peptic ulcers and the resolution of symptoms.〈section xml:id="abs1-4"〉〈title type="main"〉Results: H. pylori was eradicated in 63 out of 81 patients (78%) in the treatment group and none of the 80 patients (0%) in the placebo group. During the follow-up period, two patients in the treatment group and six patients in the placebo group developed peptic ulcers at repeat endoscopy (2.5% vs. 7.5%; 95% CI: –12 to 2). The reduction in ulcer rates was statistically significant in the ‘ulcer-like’ sub-group (0% vs. 16.7%; 95% CI: –32 to –2), but not in the ‘dysmotility-like’ and ‘unclassifiable’ sub-groups. Regarding symptom response, the resolution rates of symptoms were similar between the treatment and placebo groups (58.0% vs. 55.0%, 95% CI: –12 to 18). Additionally, no significant differences existed in the symptom responses between the treatment and control arms in each of the dyspepsia sub-groups.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusions:Eradicating H. pylori can prevent the subsequent development of peptic ulcers in the patients with ‘ulcer-like’ functional dyspepsia. However, this approach does not significantly reduce the symptoms of functional dyspepsia patients.

    Description / Table of Contents: Summary The guideline “Joint Swelling” is addressed to primary care physicians – general practitioners, internists or orthopedists without special experience in rheumatology. It provides a framework for interviewing patients, as well as for physical, laboratory and imaging examinations and for selection of treatment appropriate to the level of primary care. Situations which call for urgent evaluation and criteria for referral to rheumatologists are described. The appendix contains comments on signs and symptoms to differentiate arthralgia from joint swelling and on the diagnostic value of a history of joint swelling without confirmation by the physician. Further recommendations for the evaluation of patient history and physical and technical examinations are given in a tabular form. The significance of laboratory and imaging procedures is discussed.
    Notes: Zusammenfassung Die Leitlinie „Gelenkschwellung” richtet sich an den Hausarzt – Allgemeinarzt oder hausärztlicher Internist – sowie an den nicht spezialisierten Orthopäden. Sie beschreibt die Anamnese, die problemorientierte klinische Untersuchung, die labormedizinische und bildgebende Diagnostik sowie therapeutische Maßnahmen, die auf der primärärztlichen Ebene einzuleiten sind. Situationen, die sofortiger Abklärung bedürfen und Überweisungsindikationen zum Spezialarzt werden genannt. Der Anhang enthält Erläuterungen zur Differenzierung der Symptome von Gelenkschmerz und Gelenkschwellung, zur Wertigkeit der anamnestischen Angabe einer früheren Gelenkschwellung ohne Nachweis durch den Arzt und zu Gelenkschwellungen aus mechanischer Ursache. In tabellarischer Form werden nähere Hinweise zur Anamnese und Untersuchung gegeben und die Bedeutung der labormedizischen und bildgebenden Diagnostik diskutiert.

    Notes: Summary ¶Objective. The literature contains investigations and discussion of the role of neutrophils and neutrophil-derived myeloperoxidase (MPO) in inflammatory processes, local ischaemia, and ischaemia-reperfusion injury models. Our aim was to determine whether the same roles existed for neutrophils and the system involving neutrophil-derived MPO in experimental subarachnoid haemorrhage (SAH) and associated ischaemia. Material and Method. Forty-eight adult New Zealand white rabbits were divided into six groups of eight. The first SAH model was applied to 16 animals. Eight of these rabbits were sacrificed after 48 hours (Group 1) and the remaining eight were killed after 96 hours (Group 2). The second SAH model applied to another 16 rabbits, which were sacrificed in two groups at the above time periods, forming Groups 3 and 4, respectively. There were two groups of 8 control animals, one group per SAH model, and these rabbits were sacrificed after 48 hours. We carried out histopathological studies using haematoxylin and eosin (H&E) stain, elastin stain, MPO immunohistochemistry, and determination of basilar artery cross-sectional diameter. We also did biochemical analysis of cerebral hemisphere and brainstem specimens, measuring tissue lipid peroxidase and MPO activity. Results were compared between the groups and with their related controls. Results. In contrast to previous experimental findings in local ischaemia and ischaemia-reperfusion models, we found no histopathological or biochemical evidence to suggest a role for neutrophils and neutrophil-derived MPO in relation to subarachnoid haemorrhage and resultant vasospasm. Although we confirmed the successful induction of significant vasospasm and observed the clinical evidences of subsequent ischaemia, there was no notable accumulation of neutrophils or activity of neutrophil-derived MPO in the tissues studied. This suggests that the biological process induced by SAH follows a different pattern from that seen in local ischaemia and ischaemia-reperfusion injury.

    Notes: Abstract  Escherichia coli strain HS3, metabolically engineered to have Met–, AHVr, IleL and AECr characteristics, produced 58.0 g/l of l-threonine, but it was neither salt-tolerant nor osmotolerant; and the growth and threonine production of the strain were severely inhibited both by the addition of NaCl with a concentration higher than 2% and by the presence of glucose with a concentration higher than 10%. Therefore, salt-tolerant mutants were isolated. The salt-tolerant mutants, HS454 and HS528 which were derived from strain HS3, were both tolerant to salt (2%) and hyperproductive. The growth and l-threonine production by the mutant strain HS454 were almost unaffected by a glucose concentration lower than 10%, but gradually reduced with increasing glucose concentration, up to 15%. However, the mutant strain HS528 showed slightly enhanced growth and l-threonine production with increasing glucose concentration, up to 10–12.5%. Strains HS454 and HS528 produced 69.8 g/l and 74.0 g/l of l-threonine, respectively in a 5-l jar fermentor.

    Notes: The formation of specific target states in Na2+ is investigated using phase-locked femtosecond pulse sequences. The pulse sequences generate customized vibrational wave packets whose motion can be interpreted using classical physics. It is shown that, if two vibrational wave packets are initially excited at either end of the vibrational coordinate, changing the initial phase difference between them can have a profound effect on the subsequent dynamics. In particular, the choice of phase differences (2π, π, −π/2 or π/2) has a dramatic effect on the dynamics around the times of the second and fourth order partial revivals. The results are interpreted quantitatively using knowledge of the phase differences between components of fractional revivals evolving from a single wave packet. Finally, we discuss a novel detection technique for monitoring vibrational wave packet dynamics in molecular cations, which combines phase-modulated detection methods borrowed from Rydberg electron wave packet experiments and zero-kinetic energy pulsed-field ionization borrowed from high-resolution spectroscopy. © 2000 American Institute of Physics.

    Notes: This study presents a new approach that can stabilize effectively l-ascorbic acid in water-in-oil-in-water (w/o/w) double emulsions. Basically, the behavior of l-ascorbic acid in the aqueous phase was observed, considering its molecular deformation. Then, it was found that the stability determined in the aqueous phase by high-performance liquid chromatography (HPLC) showed that the collapse of ionization of l-ascorbic acid played a crucial role in protecting the molecular deformation. Then, the stable aqueous system was incorporated into the internal aqueous phase of the double emulsions. From the PLC analysis, it was observed that the l-ascorbic acid in an appropriate system showed high molecular stability for a long time. Moreover, in the measurement of in vitro skin permeation, the l-ascorbic acid stabilized in this study showed considerable skin permeation ability, indicating its potential applicability in pharmaceutics and cosmetics.

    Notes: This investigation dealt with the role and impact of affect in the process of resistance. A total of 597 participants took part in the study in 4 phases spanning 6 weeks. Initial results indicated that the cognitive, affective-anger, and affective-happiness inoculation treatments all conferred resistance to persuasive attacks. Structural equation analyses were conducted on the cognitive, affective-anger, and affective-happiness experimental inoculation conditions in order to examine the process of resistance. The results across all 3 conditions revealed a direct path in which inoculation treatments directly induced resistance to persuasive attacks. However, indirect paths to resistance varied across the 3 experimental conditions. Cognitive inoculation treatments contributed to receiver threat and counterarguing output, which, in turn, enhanced resistance. Thus, the cognitive inoculation treatments triggered a process that is consistent with McGuire's theoretical explanation for resistance. By contrast, both affective-anger and affective-happiness inoculation treatments relied more heavily on elicited emotional responses. Finally, the results indicated that greater receiver involvement was positively associated with experienced anger and, therefore, indirectly contributed to resistance, whereas greater receiver self-efficacy tended to dampen resistance.

    Notes: High-resolution transmission electron microscopy in conjunction with autocorrelation function analysis have been applied to investigate the evolution of structural order in germanium ion-implanted amorphous silicon (a-Si) layers. A high density of Si nanocrystallites as small as 1 nm in size was detected in as-implanted a-Si layers. The density of embedded nanocrystalline Si was found to diminish in a-Si layers with annealing temperature first then increase. The results are discussed in the context of free energy change with annealing temperature. © 2002 American Institute of Physics.
