    Notes: Abstract. A case of calcific tendinitis of the pectoralis major insertion with cortical bone erosion is presented. Clinical and laboratory findings showed a significant inflammatory reaction. Both CT and MR images demonstrated the extent of the lesion providing additional information on the dimensions of inflammatory soft tissue and bone marrow reaction. Biopsy was performed and histology revealed the typical features of calcification, inflammation and giant cell reaction.

    Notes: Abstract. Posttraumatic bone-cyst formation is a known but rare process. Typical location is the distal radius, usually as a result of a greenstick fracture. The pathogenesis is unknown; however, subperiosteal migration of fat or hemorrhage with later absorption may cause cyst formation. We present the first reported case of MR imaging of this lesion. At MRI subperiosteal hemorrhage was found to be the cause of cyst formation.

    Description / Table of Contents: Summary The numerical variations of the foot — polydactyly and oligodactyly — are layed down and 48 cases are reported, showing that polydactyly at the foot predominates. The border-rays are attached mainly. Twofold and threefold the first toe is observed. Polydactyly on the foot appears mainly on both sides. The endogenous and exogenous genesis is discussed. — Oligodactyly of the foot is a rarity. 12 own causes are quoted. The IV. and V. ray are mainly concerned.
    Abstract: Résumé Polydactylie et Oligodactylie sont discutés, 48 cas propes sont allégués. — On demontra que Polydactylie au pied est dominant. Surtout le gros orteil et le V. rayon sont attenit. Polydactylie au pied se trouve surtout doubleface. — Oligodactylie du pied est rare. 12 cas propres sont allégués. Le IV. et le V. rayon sont surtout regardé.
    Notes: Zusammenfassung An Hand von 48 eigenen Patienten werden die numerischen Variationen des Fußes — Polydaktylie und Oligodaktylie — besprochen. Es wird festgestellt, daß am Fuß die Überschußbildungen überwiegen, wobei vorwiegend die Randstrahlen gedoppelt sind. Gemeinsame Doppelung des I. und V. Zehenstrahls sowie Polydaktylie der Binnenstrahlen werden im Vergleich zur Polydaktylie der einzelnen Randstrahlen als seltene Fehlbildung angesehen. Gleichzeitige numerische und metrische Überschußbildungen werden am Fuß im Bereich der Großzehe in Form der Triphalangie des I. Strahls bei Doppelung oder Dreifachbildung desselben beobachtet. Polydaktylie der Zehenstrahlen findet sich vorwiegend doppelseitig. Geschlechtsgebundenheit der Polydaktylie besteht nicht. Familiäre Häufung konnte in drei Fällen von Doppelung des V. Strahls nachgewiesen werden. Ursächlich muß sowohl die endogene wie die exogene Genese in Betracht gezogen werden. Oligodaktylie des Fußes wird verhältnismäßig selten beobachtet. Bei 12 eigenen Fällen wird vorwiegend Mangelbildung des V. Strahls sowie des IV. und V. Strahls, der fibulare Doppeldefekt, beobachtet. Oligodaktylien der übrigen Strahlen sind selten. Die Kombination der Oligodaktylie sowohl des V. wie des IV. und V. Strahls mit Fehlbildungen der proximalen Röhrenknochen wird häufig beobachtet. Der vorwiegende Befall eines Fußes oder eines Geschlechtes wird nicht nachgewiesen. Auf die Schwierigkeit der Einordnung einzelner Fälle mit Mangelbildungen wird hingewiesen.

    Notes: Zusammenfassung Es wird über eine familiäre Häufung postaxialer Polydaktylie an allen vier Extremitäten über vier Generationen berichtet. Nach Schilderung der morphologischen Gegebenheiten wird über die intrafamiliäre Variabilität in der Ausprägung der Finger- und Zehenstrahlen gesprochen und ihre genetischen Ursachen diskutiert. Das Auftreten eines zweieiigen Zwillingspaares innerhalb der Sippe wird besonders erwähnt. Die Kombination von postaxialer Polydaktylie und Zygodaktylie in der vierten Generation wird eingehend besprochen.

    Description / Table of Contents: Résumé Nous avons décrit une observation courante de la dysostose spondylo-métaphysaire (Type Kozlowski-Maroteaux-Spranger). Nous avons discuté les resultats cliniques et radiologiques. Ce qui est frappant, c'est l'insuffisance straturale, la scoliose, une platyspondylie, des ailes iliaques larges et saillantes et une alteration de la hanche. Les examens de laboratoire sont normaux. Du point de vue ophtalmologique, on peut prouver une hypéropie facultative. L'aspect clinique est hérité de façon autosomique.
    Abstract: Zusammenfassung Basierend auf der Erstbeschreibung der spondylo-metaphysären Dysostose durch Kozlowski, Maroteaux u. Spranger wird über eine familiäre Beobachtung des gleichen Krankheitsbildes berichtet. Dabei wird auf das klinische Erscheinungsbild, das mit Kleinwuchs, mit einer Skoliose sowie mit fakultativer Einschränkung der Ellbogengelenksbeweglichkeit einhergeht, eingegangen. Röntgenologisch besteht eine allgemeine Platyspondylie, die mit breit ausladenden Beckenschaufeln sowie horizontal gestellten Pfannendächern kombiniert ist. Die coxalen Femurenden zeigen kurze und breite Schenkelhälse. Die Hüftköpfe sind mit zunehmendem Alter einer Destruktion erlegen. Die serologischen Laboruntersuchungen ergeben normale Befunde. Die Mucopolysaccharidausscheidung im Urin ist normal. Keratansulfat ist im Urin nicht nachzuweisen. Ophthalmologisch findet sich fakultativ eine Hyperopie. Das Krankheitsbild wird autosomal-dominant vererbt.
    Notes: Summary The first observation of a spondylo-metaphyseal dysostosis (typ Kozlowski-Maroteaux-Spranger) in a family is discribed. The clinical and x-ray examinations are discussed. As typical signs there are statural insufficience, scoliosis, generalized platyspondyly, modifications of the pelvis horizontal acetabular roofs and abnormalities on the femoral neck. The laboratory examinations are normal. Hyperopia is traceable. The spondylo-metaphyseal dysostosis is inherited as an autosomal dominant.

    Notes: Abstract In a retrospective study, 22 patients treated surgically for solitary or multiple myeloma between 1980 and 1993 were analysed. The main complaint was pain. A fracture was observed in 7 cases and motor-sensory impaired neurology due to spinal compression in 3. Apart from incisional biopsies, tumour resections, reductions (with and without stabilization by osteosynthesis) and endoprotheses were performed either at the extremities or on the spine. In addition, radiation and chemotherapy were included in the therapeutical concept. Early mobilization was achieved in all cases, and the 5-year survival rate (Kaplan-Meier method) was 48%. The results presented in this study demonstrate that a variety of surgical interventions can be of importance in the treatment of myeloma of the bone, ranging from biopsy or even curative resections in selected cases to endoprosthetic replacement. Thus, good functional results can be achieved and maintained over often long survival times.

    Notes: Abstract Between 1989 and 1995, 326 hip arthroplasties were surgically revised in our clinic because of loosening. For evaluation of loosening, we used history, clinical examination, radiographs, bone scans and arthrography with digital subtraction technique. In 130 cases (40%), surgery was planned by plain radiographs. In 116 cases (89%) a correct diagnosis of loosened components was made. Of the 72 patients (22%) with an additional bone scan, 87% (63 patients) had a correct diagnosis. Fifty-four patients (17%) with different findings were examined by hip arthrography; in 83% (45 patients) the diagnosis was confirmed intraoperatively. In 93% (65 patients) of 70 patients (21%) with bone scans and arthrography, surgery confirmed the diagnosis. Sensitivity for loosening of the socket was 61% by bone scan and 85% by digital subtraction arthrography; specificity was 75% in each case. Sensitivity for loosening of the stem was 71% by bone scan and 65% by digital subtraction arthrography; specificity was 80% and 88%, respectively. Our results showed that in most cases of suspected loosening of total hip arthroplasties, analysis of plain radiograph series can lead to an accurate diagnosis in 85%–90%. In cases of uncertainty, we suggest the additional use of digital subtraction arthrography, which has a similarly high diagnostic quality. Because of the low sensitivity of nuclide bone scanning (about 60%) and its dependence on prosthetic material and fixation, we do not recommend primary scintigraphy. It can be helpful in problematic cases with additional questions like activity of ectopic bone formation.

    Notes: Abstract The purpose of the study was to determine the changes that might be detected using magnetic resonance imaging (MRI) on patients with chronic lateral epicondylitis of the elbow and to judge its value concerning the clinical treatment. Thirty-four patients with chronic lateral epicondylitis were included in a prospective study. All individuals underwent MRI of the elbow on a 0.2-T dedicated system. The MRI findings were interpreted by an independent radiologist without knowledge of the clinical findings. In 6 surgical cases an additional histological study was done. The biopsy of the extensor tendon was performed for correlation with the MRI. In 21 of 34 symptomatic patients, increased signal changes in T1- and T2-weighted images were seen. In a further 11 cases, the morphology and signal intensity were normal. The histopathological analysis of 6 surgical cases confirmed the preoperative MRI findings by showing either focal fibrous degenerative tendon tissue or microruptures of collagenous fibres. MRI in patients with chronic lateral epicondylitis can help to differentiate the disease and may be of use in clinical management, preoperative planning, and in the evaluation of the degree of degeneration at the common extensor tendon insertion.

    Notes: Abstract In a prospective single-blind study the contact media ultrasound gel, vaseline and castor oil were examined for their effect on surface pain caused by extracorporeal shock waves used for tendinosis calcarea (n = 25), radiohumeral epicondylitis (n = 23) and plantar heel spur ¶(n = 12). A total of 60 patients was divided into six groups. Using a Compact S shockwave source (Dornier MedTech), an energy flux density up to 0.12 mJ/mm2 was applied three times within 3 weeks. Independent of the diagnosis, there was a statistically significant influence of the contact medium on the intensity of application pain. In this comparison castor oil was best. For the diagnosis of tendinosis calcarea and plantar heel spur, castor oil was significantly better than the other two contact media, while for epicondylitis there was no significant difference. Castor oil was found to have an advantage over ultrasound jelly and vaseline in all indications used with regard to application pain. The positive effect of castor oil can be explained by its cavitation-free quality.
