    Notes: Summary A case of acute hemolytic streptococcal gangrene is presented. Typical signs of this disease are local pain, erythema, edema, formation of blisters, and gangrene. Malaise and fever are more unspecific. The special clinical feature of this postpartum case is discussed and compared with the literature. The therapy comprised extended incision of the vulva, application of antimicrobial agents, and exchange transfusion.

    Notes: Summary Endotoxin from E. coli was infused into the distally ligated common bile duct of rabbits under the static pressure of 25cm H2O. Fibrinogen, soluble fibrin monomer complexes, antithrombin III, leukocyte and platelet counts were estimated before, and 2, 4, and 6 h after endotoxin infusion. All parameters were found significantly changed 2 h after endotoxin infusion. While fibrinogen level, AT III, leukocyte and platelet counts decreased after the endotoxin infusion the amount of SFMC increased. The change of hematological parameters showed a pattern characteristic of disseminated intravascular coagulation (DIC). In accordance with this microclots in the glomeruli of the kidneys could be demonstrated in all endotoxin-treated animals by pathological study. The findings suggest that by endotoxin infusion into the common bile duct, as a focal origin, DIC can be produced.

    Description / Table of Contents: Summary In 20 patients with acute leukemia the thrombin-mediated fibrin monomer complexes (SFMC) were determined before and after remission induction chemotherapy for several weeks to assess the extent of hypercoagulability. Increased levels of SFMC were observed in patients with high leukocyte cell counts after induction chemotherapy which may reflect the release of thromboplastic material from destructed leukemic cells. Patients with low leukocyte counts showed only a moderate increase in SFMC. A low concentration of antithrombin III (〈20 mg/100 ml) or a drop from a previously normal value was a prognostic unfavorable sign. An elevated level of plasmin-mediated fibrin-split-products could not be related to the course of therapy.
    Notes: Zusammenfassung Bei 20 Patienten mit akuter Leukämie wurden über mehrere Wochen vor und nach Remissions-Induktions-Chemotherapie die Thrombin-induzierten, löslichen Fibrin-Monomerkomplexe (LFMK) bestimmt, um das Ausmaß einer möglichen Hyperkoagulabilität beurteilen zu können. Nach Induktions-Chemotherapie wurden bei Patienten mit hohen Leukozytenzahlen deutlich erhöhte LFMK-Spiegel beobachtet, während es bei Patienten mit niedrigen Leukozytenzahlen nur zu einem geringen Anstieg der LFMK kam. Eine niedrige Antithrombin III Konzentration (〈 20 mg/100 ml) oder der Abfall von einem normalen auf einen niedrigen Wert war ein prognostisch ungünstiges Zeichen. Erhöhte Spiegel der Plasmin-induzierten Fibrin/Fibrinogen Spaltprodukte standen in keiner Beziehung zur Therapie.

    Description / Table of Contents: Summary Soluble fibrin monomer complexes (SFMC) were determined in users of different oral contraceptives. Quantitative gel filtration of β-alanin precipitated plasma samples yielded the relative (per cent of total fibrinogen content) and absolute (mg per 100 ml plasma) amount of SFMC. Increased levels of SFMC were observed in users of oral contraceptives, when compared to age, parity and socioeconomic background matched control groups. This increase was more pronounced in users of Neogynon (0.05 mg ethinyl estradiol, 0.25 mg norgestrel) than in the ones using Microgynon (0.03 mg ethinyl estradiol, 0.15 mg norgestrel). Users of Ovulen (0.1 mg mestranol, 1 mg ethynodiol diacetate) in addition to the rise of SFMC levels exhibited elevated fibrinogen levels in plasma. The data presented indicate a limited state of hypercoagulability in users of oral contraceptives.
    Notes: Zusammenfassung Lösliche Fibrinmonomerkomplexe (LFMK) wurden bei Frauen bestimmt, die verschiedene Ovulationshemmer einnahmen. Plasmaproben wurden mit β-Alanin gefällt und durch Gelfiltration der relative Gehalt an LFMK (Prozent des Gesamtfibrinogens) und der absolute Gehalt (mg pro 100 ml Plasma) bestimmt. Es wurden Frauen, die Ovulationshemmer einnahmen, mit Kontrollgruppen verglichen, die hinsichtlich Alters, Zahl der Schwangerschaften und sozioökonomischen Milieus übereinstimmten. Es konnte ein signifikant erhöhter Spiegel von LFMK gegenüber den Kontrollgruppen festgestellt werden. Dieser Anstieg war bei der Gruppe, die Neogynon (0,05mg Äthinylöstradiol, 0,25 mg D-Norgestrel) einnahmen, größer als bei einer Gruppe die Microgynon (0,03 mg Äthinylöstradiol, 0,15 mg D-Norgestrel) einnahmen. Bei der Gruppe, die Ovulen (0,1 mg Mestranol, 1 mg Ethynodioldiacetat) nahmen, konnte zusätzlich zu dem Anstieg der LFMK ein erhöhter Fibrinogenspiegel nachgewiesen werden. Die Befunde zeigen, daß die Einnahme von Ovulationshemmern zu einer begrenzten Hyperkoagulabilität führt.

    Description / Table of Contents: Zusammenfassung Die permeabilitätssteigernden Eigenschaften von Fibrin(ogen)spaltprodukten durch Plasmin (p-FDP) und ihre klinische Relevanz im Mikroemboliesyndrom veranlaßten uns, Leukocyten-Elastase Abbauprodukte (e-FDP) zu testen, da deren biologische Aktivität noch weitgehend unbekannt ist. Dabei erwiesen sich die X, Y, D und E-Fragmente der p-FDP als weniger wirksam als die dialysierten p-FDP (MG 〈 20000) oder e-FDP (MG 〈 40000). e-FDP mit einem MG 〈 1000 zeigten keine Effekte. Im Unterschied dazu sind biologische Wirkungen eines Undeka- und Pentapeptids von p-FDP beschrieben.
    Notes: Summary Plasmin-induced fibrin(ogen) degradation products (p-fdp) possess numerous biological activities with clinical relevance to the microembolism syndrome. However, biological effects of leucocyte elastase induced fibrin(ogen) degradation products (e-fdp) have not been described. In a rat skin model, permeability effect of p-fdp and e-fdp were compared. X, Y, D and E fragments of p-fdp enhanced the permeability to a lesser extent than the dialyzed p-fdp (MW 〈 20,000) and e-fdp (MW 〈 40,000). e-fdp with MW 〈 1,000 were inactive, whereas the effective fragments of p-fdp are an undecapeptide and a pentapeptide according to literature.
