    Notes: Summary A new technique for the stabilization of metastatic cervical instability of the axis and/or neighboring vertebrae at lower levels is described. By a combination of the transarticular screw fixation C1/2 (Magerl) with the hook plate technique (Magerl) (or facultatively with a 1/3 tubular or 3.5 mm dynamic compression plate) from a posterior approach, the risks and stresses on the patient of a transoral or a combined extended technique are avoided, creating a proven biomechanically stable situation. The new technique is particularly helpful in those patients with a rapid progression of their malignant disease in whom local tumor growth is not expected to compress the spinal cord, and palliative stabilization of the unstable upper cervical spine can avoid neurological deficits or alleviate pain syndromes at a minimized morbidity due to surgery. The new technique has been successfully applied in a limited clinical series of four patients with metastasis of the cervical spine, resulting in substantial improvement of the general condition and cervical pain syndrome and stability of the assemblage during the observation period (4–9 months).

Diese stellt eine absolute Indikation zur Operation dar. Zusammen mit einer Sammelstatistik der Deutschen AO wurden 245 distale intraarticuläre Humerusfrakturen ausgewertet. Verfahrenswahl. Operation in Bauchlage des Patienten, dorsaler Zugang (60 %), Olecranonosteotomie (52 %), Rekonstruktion mit verschiedenen Implantaten. Bei schwerer Weichteilverletzung temporär Fixateur externe. Sofortige kontrollierte Krankengymnastik aus der Gipsschiene (70 %). Ergebnisse. 29,5 % sehr gut, 39,6 % gut, 19,4 mäßig, 11,5 % schlecht.

The remaining disability can be controlled in two-thirds of the cases to such an extent that no alterations are necessary in private or professional life. Of the results, 29.5 % could be classified as very good, an additional 39.6 % as good. 19.4 % as moderate, and 11.5 % as poor.

    Description / Table of Contents: Zusammenfassung Distale intraarticuläre Humerusfrakturen erfordern im allgemeinen eine operative Behandlung. Das funktionelle Ergebnis ist abhängig von der anatomischen Rekonstruktion der Gelenkflächen und von der frühzeitigen kontrollierten Nachbehandlung. Das Gesamtergebnis wurde bewertet mit 23,8% sehr gut, 36,0% gut, 22,3% mäßig und 17,9% schlecht. Hierbei ist zu berücksichtigen, daß 18% der Patienten polytraumatisiert waren und in 27% eine offene Gelenktrümmerfraktur vorgelegen hatte.
    Notes: Summary Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).
