    Description / Table of Contents: Zusammenfassung Die lymphomatoide Granulomatose (LYG) wird als besonderes Krankheitsbild angesehen, das primär die Lungen und gelegentlich den oberen Respirationstrakt befällt. Die typischen histopathologischen Veränderungen zeigen eine nichtneoplastische lymphoproliferative Erkrankung, verbunden mit Nekrosen im infiltrierten Gewebe und mit angiodestruktiven Prozessen. Die Autoren beschreiben einen Fall, in dem die für die lymphomatoide Granulomatose typischen Veränderungen zuerst in der quergestreiften Muskulatur auftraten. In diesem Fall fanden sich multiple tumoröse Muskelinfiltrate und Infiltrate in Pharynx, Larynx, den paranasalen Sinus und in der Orbita vor der Lungeninfiltration, die 6 Monate nach dem ersten Muskelinfiltrat und 2 Monate vor dem Tod nachgewiesen wurde. Da eine primär muskuläre Manifestation der lymphomatoiden Granulomatose bis jetzt nicht beschrieben wurde, ist dieser Fall wegen seiner ungewöhnlichen Manifestation und der ausgesprochenen Therapieresistenz bemerkenswert. Die klinischen und experimentellen Daten sprechen für eine gestörte T-Zellfunktion.
    Notes: Summary Lymphomatoid granulomatosis (LYG) has been recognized as a peculiar disease which primarily affects the lungs and occasionally the upper respiratory tract. Typical histopathologic lesions present features of a nonneoplastic lymphoproliferative disorder associated with necroses of the infiltrated tissues and with angiodestructive processes. The authors observed a peculiar case in which lesions typical for LYG first developed in the striated muscles. In this case multiple tumorous muscular infiltrates and affections of the pharynx, larynx, paranasal sinuses, and orbita preceded the infiltration of the lung, which was first recognized 6 months after the first muscular infiltrate and 2 months before death. Because primary muscular manifestation of LYG has never been reported, this case is remarkable both for its unusual manifestation and for its striking irresponseveness towards therapy. Clinical and experimental data suggest impairment of T-cell functions.

    Notes: Abstract Penile induration, a disease of connective tissue, requires the precise delineation and differentiation of inflammatory changes (plaques) for accurate therapy. Seventy two patients with clinically suspected Induratio penis plastica (IPP) underwent ultrasound examination between 1984 and 1991. In 37 patients (aged 18 to 80) the diagnosis was proven by ultrasound. Out of this group 27 patients were examined with magnetic resonance imaging (MRI), 15 of them with additional intravenous application of adolinium DTPA (Gd-DTPA). All examinations were performed within four weeks. The results showed, that ultrasound was able to detect plaques and changes of the tunica albuginea in all 37 cases, but MRI was more sensitive in the detection of possible inflammatory change (T2-prolongation and moderate Gd-DTPA enhancement in gradient-echo sequences. MRI revealed inflammatory changes in 22 of 27 examined patients while ultrasound detected only nine such cases out of 37. Ultrasound is the method of choice in diagnosing IPP, due to the overall sensitivity to plaques and changes in the tunica. On the other hand MRI offers valuable additional information with respect to inflammatory changes particularly after Gd-DTPA. Therefore MRI has the potential to improve therapeutic planning and should be used in monitoring therapeutic effects.

    Notes: Abstract Between 1984 and 1990 312 patients underwent surgery for habitual or recurrent shoulder dislocation. Out then, 65 had a post-operative CT examination. This retrospective study was launched to demonstrate the value of CT in post-operative shoulder imaging. In most cases, CT was done using standardised techniques without contrast medium. Except in 4 cases, an intra-articular double-contrast technique was used. Evaluation was focused on 41 cases after implantation of a wedged bone graft in cases of primary or secondary flat glenoid or widening of a small glenoid with cortical consoles and bone block. The results demonstrate the effectiveness of standardised CT technique in that all necessary information concerning stabilisation of the shoulder joint, sufficient implant of bone grafts and assessment of correct inclination after osteotomy were obtained.

    Notes: Abstract We examined 50 asymptomatic patients 6–60 months after percutaneous nephrolithotomy (PCNL) by high-resolution sonography and additional colour-coded sonography and compared our results with the preoperative sonograms and the operative records. No lesions of larger vessels were detected, either arteriovenous fistulate or pseudoaneurysms. Avascular band-shaped hyperechoic parenchymal stripes were found in 17 of 50 patients (35%). As these did not appear in the preoperative sonograms, and as the stripes were in the same orientation as the neprrostomy canal, we believe them to be scar tissue.

    Notes: Abstract Sonoenorphotogic criteria of male urethral external sphincter under normal and pathologic conditions were evaluated by direct visualization with endoluminal US and correlated with normal anatomy. Between October 1992 and August 1993 18 patients were examined prospectively. We used a 12.5 MHz interventional US machine eduipped with a Sonicat 9-French catheter system. In all cases the anatomic structure of the external male urethral sphincter was visualized and demonstrated not in the expected circular configuration, but rather in an “omega sign” of the muscle and fibers, functioning with retraction of the ventral and lateral parts of the muscle, surrounding the urethra. Disorders of muscular function were revealed in 8 cases, and 6 cases demonstrated scarring and thinning of the muscle. Endoluminal-US offers the possibility of direct visualization not only of the male external sphincter, but also of the contracting mechanism in cross-sectional view. An exact description of sonoanatorny and the pathoenorphologic correlation is more accurate. The method therefore has the benefit of better access to further therapeutic considerations.

    Notes: Abstract We report the case of a 44-year-old male presenting with deviation of the penis during erection. Upon physical and clinical examination the patient did not have the typical findings of Peyronie's disease, therefore he was admitted for further evaluation by conventional sonography, duplex Doppler ultrasound, endourethral ultrasound and magnetic resonance imaging (MRI). Endourethral ultrasound and MRI provided evidence of Peyronie's disease with atypical involvement of the corpus spongiosum and, in addition, demonstrated nonpalpable plaques in the septum of the penis.

    Notes: Abstract In order to identify a high-risk group for the development of arterial hypertension after extracorporeal shock wave nephrolithotripsy (ESWL), we correlated the increase in renovascular resistance induced by ESWL in 79 normotensive patients with changes in arterial blood pressure occurring 20 (+-3) months after therapy. Renal vascular resistance was measured as resistive index (RI) by duplex Doppler sonography in both kidneys. Mean RI before treatment was 0.620±0.035 (SD). After treatment, we observed a significant and age-dependent increase in the RI in the treated kidney to 0.673 ± 0.06. Of the patients with a post-ESWL RI〉 0.690, 39% developed hypertension. Posttherapy RI values surpassing 0.690 showed a 0.8 sensitivity and a 0.7 specificity in predicting arterial hypertension. Intrarenal Doppler ultrasound (US) is useful to find the high-risk group for arterial hypertension after ESWL.

    Description / Table of Contents: Summary In the workup of patients suffering from spinal trauma the radiologist is eager to make a proper diagnosis asking only for few additional examinations. This can be done by a functional and biomechanical approach in reading standard radiographs. With the knowledge of the kind of injury expected the whole extent of posttraumatic changes in the spine can be delineated exactly. The goal is to make a diagnosis that answers questions of stability so the traumatologist can be aided in therapy.
    Notes: Zusammenfassung Von dem in der Traumatologie tätigen Radiologen wird gefordert, rasch und mit wenigen Zusatzuntersuchungen eine umfassende Diagnose von Wirbelsäulenverletzungen zu erstellen. Um dieser Aufgabe gerecht zu werden bedarf es einer funktionellen Analyse der Standardröntgenbilder. Diese sollten nach den zu erwartenden Traumafolgen je nach Verletzungsmuster beurteilt werden und sind mittels individuell angepaßten Zusatzuntersuchungen zu ergänzen. Ziel ist, die Verletzung mit allen Auswirkungen auf ihre Stabilität umfassend zu beschreiben, um für den Traumatologen wichtige prätherapeutische Informationen zu erstellen.

    Description / Table of Contents: Summary Sonography of the shoulder joint is a well-established technique in the hands of the experienced examiner, when using a standardized protocol. It has proved invaluable in assessing pathological soft tissue changes, especially after trauma. The static evaluation of anatomy and dynamic assessment of function are especially helpful in both preoperative staging and postoperative follow-up. The normal anatomy, examination techniques, including our own variations, and pathological conditions are discussed. The findings and various classifications of impingement syndrome, rotator cuff injuries, biceps tendon lesions and inflammatory changes are examined. Review of the major articles in the literature shows excellent correlation with our results, the overall sensitivity in the case of rotator cuff lesions being over 90 %. A well-performed ultrasound examination in most cases obviates the need for the more invasive arthroscopy and the more cumbersome and expensive MRI examinations.
    Notes: Zusammenfassung Der Stellenwert der Schultersonographie unter standardisierten Untersuchungsbedingungen und entsprechender Erfahrung des Untersuchers ist unbestritten. Insbesondere im Bereich der Traumatologie ist die Wertigkeit bei Rotatorenmanschettenläsionen mit entsprechendem operativem Korrelat statistisch gut abgesichert. Die Overall-Sensitivität bei Rotatorenläsion liegt heute bei ca. 95 %. Partielle und Totalrupturen der Rotatorenmanschette sowie das Impingementsyndrom fallen unter den Sammelbegriff Subakromialsyndrome und sind sonographisch sowohl in statistischer als auch dynamischer Untersuchungstechnik gut beurteilbar. Eine wesentliche Bedeutung hat die sonographische Normalwertbestimmung von Schulterweichteilen als metrisches Fundament zur Bewertung pathologischer Veränderungen. Ein definiertes Staging (prä- wie postoperativ) ermöglicht eine exakte Operationsplanung sowie eine genaue Therapiekontrolle. Die Beurteilung der Gleitlager als indirekter Hinweis auf Traumen bzw. bei entsprechender Klinik als Anhaltspunkt entzündlicher Veränderungen mit exsudativ proliferativen Gelenkergüssen oder im Rahmen der Tendinitis calcarea ist heute sonographischer Untersuchungsstandard. Die Indikation für andere Untersuchungsverfahren wie Arthroskopie, Kontrast-CT oder MRT ist dadurch zurückgedrängt.
