    Description / Table of Contents: Zusammenfassung Die Feinstruktur des Naevus iridis beim Menschen wurde mittels des Raster- und Transmissions-Elektronenmikroskops untersucht. Zur Vorderkammer hin finden sich in der Hauptsache abgerundete Zellen mit feinhöckeriger Oberfläche. Flache Deckzellen und spindelförmige Zelltypen sind selten. Das Cytoplasma der Naevuszellen findet sich dicht mit Pigmentgranula besetzt. Es weist außerdem einen ausgedehnten Golgi-Komplex mit Centrosomen, Mitochondrien, freie Ribosomen und Filamente auf. An der Zellmembran werden Pinocytosevesikel und charakteristische Membranspezialisierungen beobachtet. Die Bindegewebszellen im Naevusgewebe besitzen große Granula-Komplexe. Außerdem kommen normale Irismelanocyten vor. Die Naevuszelle ist ein hoch differenzierter Zelltyp, der strukturell dem Irismelanocyt nahesteht.
    Notes: Summary The fine structure of naevus iridis of the human iris was studied by scanning and transmission electron microscopy. Mainly rounded cells with a raspberrylike surface were found towards the anterior chamber. Flat covering cells and spindlelike cell types occur rarely. The cytoplasm of the naevus cells is densely filled with pigment granules. There is an extensive Golgi complex with centrosomes, mitochondria, free ribosomes and filaments. Pinocytotic vesicles and characteristic membrane specializations were observed at the cell membrane. The connective tissue cells of the naevus possess extensive accumulations of pigment granules. There are also melanocytes of normal appearance. The naevus cell is a further differentiated cell type which structurally resembles the iris melanocyte.

    Notes: Summary Often the diagnosis “light chain nephropathy” is not made before renal failure has developed. In the past these patients had a poor prognosis. Special features of this disease and improvement of renal function by plasma exchange therapy are shown in a case report. A 51-year-old man admitted to hospital with renal failure, proteinuria and hypercalcaemia was treated by Ca-free dialysis and membrane plasma separation (MPS). Renal biopsy studied by light and electron microscopy showed mainly tubular protein casts. It is assumed that this nephropathy is caused by a direct toxic effect of the kappa light chains on function and structure of tubuli. Proteinuria was markedly lowered and renal failure was made partly reversible by prompt and effective MPS. Thus, MPS may improve the prognosis of this stage of disease.

    Description / Table of Contents: Summary A detailed report is given of a transfusion reaction of the hemorrhagichemolytic type due to the rare antibody anti-Cellano. The search for compatible blood for the patient-she had the blood group formula A1 ccD Ee KK-was further complicated by the development of a secondary antibody of the specificity anti-C (C major). This secondary antibody for a short time had the same titre (1: 512) as the anti-Cellano but showed both a different mode of reaction and a different stability. The necessity to use the optimal techniques for the serologic compatibility tests and to work only with fresh drawn blood samples when investigating sensitized patients is stressed. Among the therapeutical and organizational consequences the availability of samples of rare blood groups for the identification of irregular antibodies and of donors of this rare blood type is thought to be of special importance.
    Notes: Zusammenfassung Es wird ausführlich über einen hämolytischen, hämorrhagischen Transfusionszwischenfall berichtet, als dessen Ursache der seltene Antikörper Anti-Cellano diagnostiziert wurde. Die Suche nach kompatiblen Blutkonserven für die Patientin mit der Blutgruppenformel A1 ccD. Ee KK wurde noch kompliziert durch das nachträgliche Auftreten eines sekundären Antikörpers der Spezifität Anti-C (C-major) mit kurzfristig gleicher Titerhöhe (1: 512), aber unterschiedlicher Reaktionsweise und unterschiedlichem Stabilitätsverhalten. In der Reihe der therapeutischen und organisatorischen Konsequenzen wird neben der Forderung nach optimaler Technik der serologischen Verträglichkeitsprobe und für diese nach stets frischen Blutproben bei sensibilisierten Patienten die Notwendigkeit betont, seltene Blutmuster zur Identifizierung irregulärer Antikörper und entsprechende Blutspender dieser seltenen Formel verfügbar zu haben.

    Notes: Summary In 10 patients with end stage renal disease on regular haemodialysis the plasma concentrations and dialyzer clearance of isosorbide dinitrate (ISDN) were determined after an oral dose of a retarded release formulation of 60 mg ISDN. The maximal plasma concentration of ISDN 2–7 h after oral administration was higher (14 ng/ml) than has been reported in healthy volunteers. The haemodialyzer clearance of ISDN was 92.4 ml/min at a blood flow of 200 ml/min and dialysate flow of 500 ml/min. During a 5-h haemodialysis an average of 0.3 mg ISDN was removed from the patient's circulation, representing about 0.5% of the administered dose and about 3% of the available drug in the circulation. No influence of haemodialysis on the plasma level of ISDN was found.

    Notes: Summary The influence of prospective HLA-DR matching on the graft survival rate was investigated in a multicenter analysis of 85 transplants. Simultaneously in a retrospective analysis of graft outcome the importance of matching for MT-antigens MT1, MT2 and MT3 as a newly defined B-cell alloantigen system was evaluated. HLA-DR antigens and MT-specificities were determined on B-cells enriched by nylon-wool filtration using locally well characterised HLA-DR antisera and the antiserum set of the 8th International Histocompatibility Workshop (“discase set”) which allowed the definition of the HLA-DR specificities HLA-DR 1–9 and of the MT-antigens MT 1–3. HLA-DR matching showed a significantly improved graft outcome only in HLA-DR identical donor-recipient combinations. In 11 of 60 patients with one HLA-DR compatibility additional matching for two MT-antigens, however, improved the two year graft survival rate from 60% to 91%. Altogether 17 patients were matched for two MT-specificities with their kidney donor and showed a superior prognosis of 94% at two years compared to 53% or 17% of recipients with one or zero MT compatibility. Graft outcome in this patient group was also superior to that of HLA-DR identical or HLA-AB identical grafts. These data suggested that the MT-system rather than the HLA-DR antigens may be of critical importance in cadaver kidney transplantation. In addition a favorable influence of pretransplant blood transfusions on less HLA-DR matched grafts was confirmed.
