Summary The conditions considered to be prerequisite to any systemic cancer therapy comprise a specific training in medical oncology with detailled knowledge of the heterogenous aspects of tumor biology as well as the pharmacokinetics and-dynamics of the cytostatic agents. Active participation in regular interdisciplinary rounds and, for reasons of quality control in clinical trials, are necessary to offer ones patients the optimal choice of treatment options, be it in curative, adjuvant, palliative or merely supportive intent. If the physician together with the oncological team master all the details and pitfalls of treatment protocols, they can save the patients from unnecessary burden and help to improve their time without tumor induced symptoms and/or treatment related toxicity. Besides cancer nursing the psychosocial guidance of the patient and his relatives throughout the progressing illness and at times beyond death are inalienable. The following paper details some of these absolute requirements for any form of cytostatic chemotherapy, be it on an in- or outpatient basis. Die bescheidenen Fortschritte, die die klinische Onkologie auf dem Gebiet der systemischen Therapie maligner gynäkologischer Tumoren und des Mammakarzinoms gemacht hat, umfaßt im wesentlichen 3 Gebiete: Zum ersten die Entwicklung wirksamer Medikamente, Endokrinologika und Zytostatika, zum zweiten die Pharmakokinetik und Pharmakodynamik, die uns eine optimale Nutzung und einen relativ gezielten und schonenden Einsatz, dieser z.T. hochtoxischen Substanzen erlauben und zum dritten die supportive Therapie, also Maßnahmen zur Besserung des Befindens und der Leistungsbreite, also der Lebensqualität ...innerhalb der von der Biologie des individuellen Tumors definierten und begrenzten Überlebensdauer. Daß ich als medizinischer Onkologe dieseklinischen Aspekte in den Vordergrund stelle, hängt auch mit der chronologischen Entwicklung der Krebsforschung zusammen: Sie nahm in den fünfziger Jahren ihren Ursprung in der zunächst empirischen Suche nach effektiven zytostatischen Substanzen, führte dann über die Erforschung der Tumorbiologie zu der aufregenden Entdeckung der auto- und parakrinen Regulationsmechanismen und schließlich in die Molekularbiologie des Tumorzell-Genoms. In der letzten Dekade hat sich die theoretische Onkologie von der klinischen Empirie freigemacht, ist ihr, weit vorausgeeilt und schafft neueGrundlagen, auf die wir unsere aktuellen und zukünftigen Therapieentscheide gründen können. Damit eröffnen sich endlich auch wieder neue Aspekte, die uns aus dem gegen-wärtigen Plateau therapeutischer Frustationen herauszubringen versprechen.
