    Notes: Abstract: Objective and design. To determine and compare the proteoglycan degradative properties of neutrophils, macrophages and synoviocytes in cultures of articular cartilage.¶Material of subjects. Bovine articular cartilage was aseptically isolated from metacarpopharyngeal joints.Neutrophils and macropahges were isolated from normal human blood and bovine synovial fibroblasts were isolated from explant cultures before being incubated with the cartilage.¶Treatment: Neutrophils, macrophages or synovial fibroblasts (1 × 106-8 × 106) were incubated with 35SO4 labelled cartilage for 2.5-72 h.¶Methods. Cartilage degradation was measured as a loss of 35SO4 into the cartilage medium as a percentage of the total labelled proteoglycan in the cartilage slice. Statistical significances were determined using a 2-tailed unpaired Student's t-test.¶Results. Neutrophils rapidly degraded articular cartilage. After 2.5 hours of culture, neutrophils degraded cartilage proteoglycan up to 28 times more than either macrophages or synovial fibroblasts.¶Conclusions. Neutrophils induce rapid damage to articular cartilage proteoglycan, whereas in comparison, macrophages and synovial fibroblasts degrade articular cartilage proteoglycans poorly. These findings indicate that at least under conditions where the influence of cellular-cellular interactions and soluble mediator action are excluded, adhesion of neutrophils to articular cartilage is sufficient to stimulate rapid and marked cartilage degradation compared to the other two cell types.¶

    Notes: A modified solid-state precipitation process for the formation of stoichiometric A-15 V3Ga is presented which results in high values of Tc=15.0 K and Hc2(4.2)=22 T, excellent phase homogeneity, and a high critical current Jc(4.2 K)=3×105–6×104 A/cm2 over the field range 0–18 T. We find that grain boundary pinning is dominant, producing a very high specific pinning force Qmax=6.4×104 dyn/cm2. The nonparamagnetically limited Hc2 is needed to explain our high flux-pinning results.

    Notes: Enhanced magnetically isotropic properties are observed in melt-spun flakes of the Nd2Fe14B(Si) type. The magnetic enhancement, referred to as HIREM, includes a remanent magnetization equal to 8–9.3 kG (with saturation magnetization measured between 15.2 and 15.7 kG), and an energy product up to 19 MGOe. Both of these properties are significantly above the conventional limits permissible for noninteracting magnetic grains. Microanalysis in an analytical electron microscope (AEM) indicates that there are two requirements for the HIREM phenomenon to arise: (1) a substantial absence of an intergranular phase between the 2-14-1 crystallites, and (2) a specific grain size range. These results support a model for the enhancement resulting from intergrain magnetic exchange coupling.

    Notes: We have fabricated a macroscopically isotropic permanent magnet material (Ovonic Hi-RemTM) which significantly exceeds conventionally understood performance limits for a material with saturation magnetization Ms =16 kG. Enhanced magnetic properties, including a remanent magnetization Mr =9–10 kG or more, and a maximum energy product in excess of 20 MGOe, are observed without preferred orientation. This material has been fabricated by rapid solidification of an alloy structurally and compositionally similar to Nd2Fe14B, but which depends on critical alloying additions and process parameters to obtain optimum magnetic performance.

    Notes: Aims : To determine the serum and intrahepatic levels of T-helper-1-associated chemokines in patients with chronic hepatitis C before, during and after peginterferon plus ribavirin combination therapy and to search for correlations with baseline characteristics of hepatitis C virus-related chronic liver disease and type of therapeutic response.Methods : Serum chemokine levels were determined by enzyme-linked immunosorbent assays and intrahepatic chemokine messenger RNA and protein levels were tested by ribonuclease protection assay and immunohistochemistry.Results : Serum and intrahepatic chemokine levels were elevated in all patients with chronic hepatitis C and showed a marked decrease in patients who obtained a virological response vs. non-responders. Increased serum interferon-γ-inducible protein-10 levels at baseline in genotype 1-infected patients were significantly associated with greater degrees of intrahepatic inflammation and fibrosis (P = 0.0046 and P = 0.02, respectively) and with virological non-response (P = 0.01). In patients with genotype 1, basal serum interferon-γ-inducible protein-10 levels greater than 299 pg/mL identified 80% of non-responders and lower than 299 pg/mL identified 63% of responders.Conclusions : Circulating and intrahepatic T-helper-1-associated chemokines are abnormally elevated in patients with chronic hepatitis C. Increased baseline serum interferon-γ-inducible protein-10 levels in genotype 1-infected patients are associated with virological non-response to peginterferon plus ribavirin combination therapy.

    Notes: Abstract During the period 1972–1982, only 10 of the 105 patients under 20 years of age who underwent thyroid surgery (9.5%) had thyroid cancer. In these 10 cases total thyroidectomy was the treatment of choice. Lymphadenectomy was perfomed in 6 cases following intraoperative histological examination. In all cases, a total body scan was done 30 days after surgical excision. Three patients required complementary radiometabolic therapy. In three cases, lymph node metastases (found during follow-up visits at various times after the first procedure) were removed. All 10 patients are presently alive and without signs of disease. The authors, in their confirmation of the favorable prognosis of thyroid cancer in children and adolescents, stress the advantages of using near-total thyroidectomy and radiometabolic therapy in controlling the disease.

    Notes: Abstract Pulmonary complications after orthotopic liver transplant (OLT) are frequent, involving high morbidity and mortality. We have determined the pulmonary complication incidence in 187 patients submitted to OLT at the General University Hospital “Gregorio Marañón” in the last 4 years, analyzing the type of infection, evolution, diagnostic and therapeutic measures and their influence on OLT mortality. A total of 120 patients had pulmonary complications, the most frequent being pleural effusion (61.94 %), pneumonia (43.36 %), and pneumothorax (11.5 %). Serious pulmonary hypertension was diagnosed by invasive methods in two patients at the time of surgery (unidentified before OLT); both died at early post postoperative times. Pleural effusion was noted in 70 patients, 31.42 % of them requiring thoracic tube drainage, complications developing in 22.72 %. Thirteen patients were diagnosed of pneumothorax, the most frequent etiologies being percutaneous liver biopsy, thoracic tube drainage for pleural effusion, and postoperative complications in 41.6, 33.3, and 23.3 %, respectively. Pneumonia was diagnosed in the 1st month after OLT in 45 patients. Tests to diagnose and identify the etiological agent were made in 71.1 % of diagnosed pneumonia patients, identification being obtained in 62.5 %. Telescope catheter culture identified the agent in 48 %, fiber optic bronchoscopy in 50 %, and lung or pleural biopsy in 100 %. Respiratory insufficiency was noted in 64 patients (34.22 % of transplanted patients). Factors involved in their development were pneumonia (42.18 %), graft dysfunction (39.06 %, pleural effusion (34.37 %), sepsis (28.18 %), and poor nutritional status (7.81 %). Fifty patients (41.66 %) died, pulmonary pathology being the determinant factor in 28.8 %. Patient mortality with respiratory insufficiency was greater, especially in those with three factors involved the development of respiratory insufficiency.

    Notes: Abstract Acute rejection (AR) is a frequent complication following liver transplantation (LT). ICAM-1 may be involved in its pathogenesis. High doses of glucocorticoids are the standard treatment in these patients. The aim of this study was to describe corticoid effects on ICAM-1 tissue expression in liver biopsies of patients with LT and AR. The study included liver biopsies performed before and after treatment in 12 patients with LT and proven AR. In 10 patients AR was reversible and in 2, was steroid resistant. For immunohistochemistry, an indirect immunoperoxidase technique was used. Each histology section was semiquantitatively evaluated as follows: 0: 〈 10 % staining, 1: 10–25 %, 2: 25–50 %, 3: 〉 50 %. The control group comprised nine patients with LT and normal liver biopsies. In pre-treatment liver biopsy samples, ICAM-1 was markedly expressed on sinusoidal cells (2.41 ± 0.66), and there was also expression on periportal (0.66 ± 0.65) and perivenular hepatocytes (0.83 ± 0.57). By contrast, in the liver tissue from the control group, sinusoidal ICAM-1 reactivity was significantly lower (0.88 ± 0.33; P 〈 0.05), and hepatocytes showed no reliable ICAM-1 expression. After steroid treatment the intensity of ICAM-1 decreased significantly in sinusoids (1.5 ± 0.67; P 〈 0.05) and in perivenular hepatocytes (0.25 ± 0.86; P 〈 0.05). Additionally, we also observed a decreased ICAM-1 reactivity in portal hepatocytes (0.25 ± 0.62), but these differences did not reach statistical significance. Remarkably, after treatment, hepatocytes did not show ICAM-1 reactivity in resolved AR, but in corticoid-resistant patients AR did not change or increase. In conclusion, in patients with LT and AR, ICAM-1 was expressed in hepatocytes and with more intensity in sinusoid cells. Additionally, a down-regulation of the ICAM-1 tissue expression after corticoid treatment may exist, although in corticoid-resistant AR no modulation on ICAM-1 tissue expression was observed.
