    Description / Table of Contents: Summary Our knowledge on the composition of general hospital diets still relates virtually exclusively to calculated values derived from food tables. To find out how relevant these are for the estimation of potassium and sodium contents of the general diet data were chemically analysed and compared to table values. The chemical analysis of daily general diet portions resulted in a mean value of 1.86 g potassium/day in contrast to the calculated mean value of 2.72 g/day. As far as the sodium contents of the general diets are concerned, the picture was reversed (calculated: 3.16 g/day, analysed: 5.20 g/day). These results suggest that only chemical analysis of hospital diets guarantees an adequate supply of potassium and sodium.
    Notes: Zusammenfassung Unser Wissen über den Nährstoffgehalt der Klinikkost beruht fast ausschließlich auf Nahrungsmitteltabellen. Um herauszufinden, wie relevant diese für den Kalium- und Natriumgehalt der Klinik-Vollkost sind, wurden analytische Daten ermittelt und den berechneten gegenübergestellt. Beim Kalium lag der chemisch ermittelte Wert mit 1,86 g/Tag signifikant niedriger als der Tabellenwert (2,72 g/ Tag). Beim Natrium war das Verhältnis umgekehrt (Analysenwert: 5,20 g/Tag, errechneter Wert: 3,16 g/Tag). Diese Befunde sprechen dafür, daß nur eine chemische Analyse der Klinikkost ausreichende Information zur Kalium- und Natriumzufuhr liefert.

    Notes: Summary A study of 110 patients with membranoproliferative glomerulonephritis (MPGN) revealed that this disease appears in a simple form and in a lobular form. Both forms are characterized by a differently severe proliferation of mesangial cells and by a mostly extensiv thickening of the glomerular basement membrane. The proliferation of mesangial cells and thickening of the glomerular basement membrane is more severe in the lobular form of MPGN. Dense deposit lesions were observed in association with both the simple form and the lobular form of MGPN. An analysis of the morphological and clinical features of MPGN in patients with dense deposits in the glomerular capillary walls failed to indicate that dense deposits represent a unique disease entity.

    Description / Table of Contents: Zusammenfassung Die vorliegende Arbeit beschäftigt sich mit der Frage, wieweit auch bei Patienten mit Niereninsuffizienz (die regelmäßig dialysiert werden) der Serumtransferrinspiegel als Maß für den Eiweißernährungszustand verwertbar ist. In einem kontrollierten Versuch wurde bei einer ausgesuchten Gruppe von chronisch hämodialysierten Patienten mit erniedrigtem Serumtransferrinspiegel das Nahrungseiweiß unter im übrigen konstant gehaltenen Bedingungen erhöht. Es kam bei allen Probanden zu einem signifikanten Anstieg der Serumtransferrinwerte. Dies spricht dafür, daß der Serumtransferrinspiegel unter bestimmten Einschränkungen (z. B. schwere Leberschädigung) ein empfindlicher biochemischer Indicator zur Erfassung eines auch subklinischen Eiweißmangels ist und bei chronisch hämodialysierten Patienten eine optimale Dosierung der Eiweißzufuhr erlaubt.
    Notes: Summary The subject of this paper is to establish inhowfar the serum transferrin level may be used as an indicator of protein malnutrition in patients with terminal renal insufficiency (regular dialysis patients). In a controlled prospective study such patients, on a low protein diet with low transferrin levels, were placed on a high protein isocaloric diet. A significant increase (normalisation) of the serum transferrin levels was observed except in one patient having a severe hepatic disease. The results suggest that in dialysis patients the serum transferrin level is a sensitive indicator of protein malnutrition.

    Description / Table of Contents: Zusammenfassung Bei 71 Patienten mit chronischer Nierenerkrankung verschiedener Stadien fand sich eine lineare enge Korrelation zwischen der Konzentration des „wahren Kreatinins“ im Serum und der des 3-Methyl-Histidins (r=0.79). In ersten orientierenden Untersuchungen wird wahrscheinlich gemacht, daß der Ausscheidungsmechanismus des 3-Methyl-Histidins sich von dem des Histidins grundsätzlich unterscheidet. Histidin wird glomerulär filtriert und 5–7% der filtrierten Menge erscheinen im Urin, 3-Methyl-Histidin wird filtriert und zusätzlich sezerniert. Die 3-Methyl-Histidin-Clearance liegt beim Gesunden dreimal höher als die endogene Kreatininclearance, bei starker Einschränkung der Nierenfunktion fällt sie unter den Wert der Kreatininclearance ab.
    Notes: Summary In 71 patients with different stages of renal disease, a close linear correlation was found between the concentration of “true creatinine” in the serum and that of 3-methyl-histidine (r=0.79). First orientational investigations indicate that the excretion mechanism of 3-methyl-histidine is fundamentally different from that of histidine. Histidine is filtered glomerularly, and 5–7% of the filtered volume appears in the urine; 3-methyl-histidine is filtered and secreted as well. In healthy subjects, the clearance of 3-methyl-histidine is three times higher than the endogenous creatinine clearance, and it falls below the creatinine clearance when renal function is severely impaired.
