    Description / Table of Contents: Abstract. A 32-year-old white woman presented with persistent swelling of the right upper eyelid for diagnosis and therapy. History and physical examination revealed a faint nevus flammeus on the right side of her face and neck and bilateral blepharochalasis, both of which can produce unilateral periorbital swelling. A biopsy taken for histology was diagnostic in this case, since it showed lymphoedema secondary to lymphangiectasias associated with the patient's port-wine stain. Many other possible causes of unilateral lid swelling have to be considered and excluded before a definite diagnosis can be established. The differential diagnosis is discussed.
    Notes: Zusammenfassung. Es wird über eine 32jährige Patientin berichtet, die sich zur Abklärung und Therapie einer chronisch persistierenden Schwellung des rechten Oberlides vorstellte. Klinisch fanden sich ein blasser Naevus flammeus der rechten Gesichts- und Halspartie und eine beidseitige Blepharochalasis. Beide Erkrankungen können das klinische Symptom einer einseitigen Lidschwellung hervorrufen, jedoch sind andere Ursachen differentialdiagnostisch abzuklären und auszuschließen. Im vorliegenden Fall wurde durch den histologischen Nachweis ektatischer Lymphgefäße und eines unspezifischen Ödems die Diagnose eines Lymphödems des rechten Oberlides bei einer seit Geburt bestehenden Minimalvariante eines Naevus flammeus gestellt. Die verschiedenen Differentialdiagnosen von Lidschwellungen werden diskutiert.

    Notes: Objectives:  Occupational skin diseases (OSDs) account for a large number of occupational diseases in Europe. Population-based epidemiological studies concerning certain occupations (e.g. Health care workers), however, are missing.Aim of our study was to analyse the reports of OSD in Health care workers (HCW) in the register of OSDs of Northern Bavaria and to assess the annual incidence in this occupational group in relationship to the total employed population in Northern Bavaria. Further, we investigated the spectrum of sensitizations with and without occupational relevance in this group.Methods:  A total of 5285 cases of OSD were assessed prospectively and registered from 1990–1999. Data of total employment were provided by the German Federal Employment office.Results:  A total of 3097 cases were confirmed as OSD in 24 occupational groups, of which 482 were confirmed in the group of HCW. The average annual incidence of OSD in this group accounted for 7.3 per 10,000 workers. In 1990 the incidences in this occupational group accounted for 11.4%, in 1999 for 5.0% per 10,000 workers with highest incidences in younger age groups. In the evaluated health care workers from the register of OSD in Northern Bavaria 54%(n = 260) had irritant and 51%(n = 244) had allergic contact dermatitis including 19%(n = 89) who suffered from both. Of the 482 HCW 13% had occupational relevant Type IV allergy to glutaraldehyde, 12% to thiurams, 6% to nickel (II) sulfate, 4% to formaldehyde or fragances, respectively, 2% to thimerosal, potassium (II) chloride, chloromethyl-isothiazolone and p-phenylendiamine (free base) each.Conclusions:  HCW belong to the occupational groups at exceedingly high risk for OSD. Higher frequencies of OSD occur in younger age groups. Irritant and allergic contact dermatitis are equally common causes of OSD in HCW. Sensitization to certain allergens (e.g. glutaraldehyde, thiurams and latex) are most often of occupational relevance, whereas others although frequently found (e.g. sensitization to nickel-II-sulfate, cobalt-II-chloride, fragance-mix) rarely are of occupational relevance.

    Notes: An 8-year-old boy (case 1) was seen in September 2000 with pruritic papules on his left upper arm, 1 week after having received a so-called henna painting in Egypt (〈link href="#f14841"〉Fig. 1a). An 18-year-old female student (case 2) was seen in August 2000 with pruritic papular skin eruptions on her lower back, 2 weeks after having obtained a so-called temporary henna tattoo in Ibiza (〈link href="#f14841"〉Fig. 1d). The dermis showed monomorphous lichenoid lymphoid infiltrates, few histiocytes and eosinophils, partially erythrocyte extravasate and obliterated capillaries (〈link href="#f14841"〉Fig. 1e).In both patients patch test series (standard series〈link href="#fn14841"〉*, textile dyes〈link href="#fn14842"〉**, aromatic para-substituted amino-compounds〈link href="#fn14843"〉***, cosmetics and skin care products〈link href="#fn14844"〉****) were performed according to the German Contact Dermatitis Research Group. Additionally, procaine (pure), sulfamethoxazole (pure), saccharine (pure) and cyclamate (pure) were tested. In the 8-year-old patient, after 48 h and 72 h a +++-reaction (i.e. erythema, infiltration and confluent vesicles according to the International Contact Dermatitis Research Group) occured to p-phenylenediamine, p-toluylenediamine, disperse orange 3, disperse yellow 3, p-aminoazobenzol and p-aminophenol; a ++-reaction (i.e. erythema, infiltration, papules and vesicles) occured to IPPD, 4,4′diaminodiphenyl methane; and a +-reaction (i.e. erythema, infiltration, papules) to tert. butyl hydroquinone (〈link href="#f14841"〉Fig. 1c). The 18-year old patient exhibited a +++-reaction to p-phenylenediamine, p-toluylenediamine, disperse orange 3 and p-aminoazobenzol after 48 h and 72 h.Topical steroids were utilized for therapeutic intervention during the initial pruritic phase of skin lesions. In case 1, prednicarbat 0.25% cream was topically applied twice a day for 6 weeks. In patient 2, clobetasol-17-propionat 0.05% cream was used once a day for 2 weeks. When the discomfort ceased, this therapy was discontinued and a steroid-free ointment (Unguentum leniens) was applied twice daily.In June 2001, in both patients, the skin lesions were invariably present. The infiltrate decreased on further follow-up. At the 12-months follow-up visit the infiltrate had markedly decreased in both patients. In case 2 a slight infiltration was still palpable, in case 1 (the boy) a hypopigmentation remained (〈link href="#f14841"〉Fig. 1b).Although fully informed and aware of the persistent sensitivity to p-phenylenediamine and cross-reactive para-substituted amino-compounds as well as their occurrence, the 18-year-old patient reported to have recently dyed her eyebrows and lashes in black, followed by acute peri-ocular eczema. Henna was continuously used as a red hair dye without adverse reactions.

    Description / Table of Contents: Summary The simultaneous occurrence of lupus erythematosus (LE)- and lichen ruber (LP)-like symptoms is called LE/LP-overlap syndrome (LE/LP-OS). It is defined by concomitant clinical, histologic and immunhistologic features of both diseases. To date, 47 cases of this rare dermatosis have been reported with marked differences in the skin lesions: They are either of intermediate appearance between LE and LP (type I=intermediate type), or show a co-existence of LE- and LP-specific lesions (type II=polar type). To determine the frequency and characteristics of the LE/LP-OS we reviewed our LE-patients from 1984–1995. 5 cases were diagnosed. The frequency of LE/LP-OS in our patients is higher than generally assumed. Due to its variable clinical, histological and immunhistological appearance and the lack of unequivocal pathognomonic signs, this overlap-dermatosis may be underdiagnosed. Since therapeutic consequences result from the diagnosis, criteria are suggested to faciliate the recognition of the LE/LP-OS.
    Notes: Zusammenfassung Das simultane Vorkommen von Lupus-erythematodes-(LE-) und Lichen-ruber-planus-(LP-) artigen Symptomen wird als LE/LP-Overlap-Syndrom (LE/LP-OS) bezeichnet. Es wird definiert durch das gleichzeitige Vorkommen klinischer, histologischer und immunhistologischer Charakteristika beider Krankheitsbilder. In der Literatur sind bisher 47 Patienten mit dieser seltenen Erkrankung beschrieben, wobei die klinischen Erscheinungsbilder heterogen sind: Die einzelnen Hauterscheinungen zeigen ein zwischen LE und LP intermediäres Erscheinungsbild (Typ I=intermediärer Typ) oder aber ein Nebeneinander von LE- und LP-typischen Effloreszenzen (Typ II=polarer Typ). Zur Bestimmung der Häufigkeit und Charakteristika des LE/LP-OS untersuchten wir retrospektiv unser LE-Patientengut von 1984–1995: In 5 Fällen wurde die Diagnose eines LE/LP-OS gestellt. Das LE/LP-OS ist demnach im untersuchten Kollektiv häufiger als bisher angenommen. Aufgrund der großen Variationsbreite der klinischen, histologischen und immunhistologischen Befunde und Fehlen pathognomonischer Merkmale des LE/LP-OS, wird die Diagnose möglicherweise zu selten gestellt. Da sich jedoch aus der Diagnose therapeutische Konsequenzen ergeben, werden Kriterien vorgestellt, die Erkennen und Abgrenzung des LE/LP-OS erleichtern.

    Description / Table of Contents: Summary. We report on a 31-year-old white woman, who was bitten in her right calf by a "spitting cobra" (Neia nigricollis) during a safari in Tansania. Minor initial systemic symptoms such as nausea and vomiting were followed by severe oedematous swelling of the extremity after 2 – 3 h and demarcation of a 2.75×2.75 in. area of necrotic skin. The patient returned to her home country, where 8 days after the snake-bite necrosectomy was performed. Antibiotics, antiinflammatory agents and local therapy with hydrocolloidal wound dressings were administered. With this therapy the lesion healed completely with minor scarring within 5 months. A new Salmonella strain was isolated from the ground of the ulcer.
    Notes: Zusammenfassung. Eine 31jährige Frau wurde während einer Safari in Tansania von einer Speikobra (Neia nigricollis) in die rechte Wade gebissen. Nach leichter systemischer Symptomatik mit Übelkeit und Erbrechen kam es innerhalb von 2 – 3 h zu starker ödematöser Schwellung der Gliedmaße und Demarkation eines 7×7 cm großen nekrotischen Hautareals. Nach Rückreise wurden 8 Tage nach der Bißverletzung eine Nekrosektomie, antibiotische und antiphlogistische Therapie sowie lokale Behandlung mit hydrokolloidalen Wundverbänden durchgeführt, was innerhalb von 5 Monaten zur narbigen Abheilung der Läsion führte. Aus dem Ulkusgrund konnte eine neue, bislang nicht charakterisierte Salmonellen-Spezies isoliert werden.

    Description / Table of Contents: Summary Papillary cystadenoma of the minor salivary glands is a rare benign tumor that clinically resembles a mucous cyst. We demonstrate its histologic features and differential diagnosis by a case report. Benign and malignant neoplasms of the minor salivary glands are not well acknowledged in the dermatologic literature, but should be considered in the differential diagnosis for mucous cysts.
    Notes: Zusammenfassung Das papilläre Zystadenom der kleinen Speicheldrüsen ist ein seltener benigner Tumor, der klinisch der Schleimretentionszyste gleicht. Anhand einer Kasuistik werden histologischer Befund und Differentialdiagnosen dieses Tumors dargestellt. Die im dermatologischen Schrifttum wenig beachteten Neubildungen der kleinen Speicheldrüsen sollten als Differentialdiagnose der häufig auftretenden echten und falschen Schleimretentionszysten nicht unberücksichtigt bleiben.

    Description / Table of Contents: Summary A 19-year old caucasian patient suffered from ulceration and scaring of his fingers since age two. During childhood, fibrosing contractures on the phalanges developed. Since puberty, his eunuch-like stature, bird-like face and exophthalmos had become under obvious. His hair was extraordinarily fine; his pubic hair rare. Radiological examination revealed ankylosis and osteoporosis of the phalanges and carpals. Angiography showed occlusion of multiple digital and interdigital arteries. Furthermore, a bilateral posterior cataract, restrictive respiratory disease, impaired glucose tolerance, hyperuricemia, proteinuria and primary hypogonadism were diagnosed. These findings are characteristic for the premature aging syndromes. The cardial symptoms of tall stature, bird-like face, pseudoexophthalmos, skleroderma-like and poikiloderma-like cutaneous lesions, scarce hair growth, early diabetes mellitus and arteriosclerosis led to the diagnosis of metageria.
    Notes: Zusammenfassung Bei einem 19jährigen Mann entwickelten sich seit dem 2. Lebensjahr Ulzerationen und Narben an Fingern und Zehen. Im Laufe der Kindheit bildeten sich sekundäre Beugekontrakturen der Phalangen aus. Seit der Pubertät besteht ein eunuchoider Habitus mit Vogelgesicht und Pseudoexophthalmus. Das Haupthaar ist fein und die primäre Geschlechtsbehaarung gering ausgeprägt. An den Phalangen und Handwurzelknochen konnten radiologisch Ankylosen und eine Osteoporose gesichert sowie multiple Gefäßabbrüche v.a. der Interdigital- und Digitalarterien nachgewiesen werden. Die Durchuntersuchung ergab eine Cataracta polaris posterior beidseits, eine restriktive Ventilationsstörung, einen pathologischen Glukosetoleranztest, eine Hyperurikämie, eine erhebliche Proteinurie sowie einen hypergonadotropen Hypogonadismus. Anhand oben genannter Symptome und Befunde konnte der Patient der Krankheitsgruppe der Progeriesyndrome zugeordnet werden. Hierbei kommt besonders die Metagerie mit den Symptomen Überlänge, Vogelgesicht mit Pseudoexophthalmus, sklero- und poikilodermieartige Hautveränderungen, spärliche Behaarung, frühzeitig auftretender Diabetes mellitus und Arteriosklerose in Betracht. Daher wurde im vorliegenden Fall die Diagnose der Metagerie gestellt.

    Description / Table of Contents: Summary Skin diseases, the psyche and psychological changes are often intertwined, especially in patients presenting for expert dermatologic opinion. In many cases an additional evaluation provided by psychotherapeutic medicine may be necessary. This resource may help with the diagnosis, explanation of the problem and estimation of the degree of disability. The different legal guidelines of the various evaluation boards must be considered. The role of psychotherapeutic evaluation is demonstrated through case examples. The evaluation of new possibly occupationally-related disorders such as multiple chemical sensitivity and mobbing is considered.
    Notes: Zusammenfassung Zwischen Haut und Psyche sowie psychischer Alteration bestehen bei vielen Dermatosen wechselseitige Zusammenhänge, die auch in Begutachtungssituationen zutagetreten können. In bestimmten Fällen kann daher eine Zusatzbegutachtung durch die psychotherapeutische Medizin erforderlich werden, insbesondere wenn daraus neue Erkenntnisse zur Aufklärung, Diagnosesicherung oder Einschätzung der Minderung der Erwerbsfähigkeit (MdE) resultieren. Die unterschiedlichen rechtlichen Grundlagen der verschiedenen Rehabilitationsträger sind dabei zu berücksichtigen. Die unterschiedlichen Indikationen zur Zusatzbegutachtung durch die psychotherapeutische Medizin werden anhand von konkreten Gutachtensbeispielen dargestellt. Die Wertung neuer möglicherweise berufsassoziierter Erkrankungen wie z.B. „multiple chemical sensitivity” und Mobbing wird diskutiert.
