    Notes: Abstract Combined hyperlipidaemia and the presence of small and dense LDL particles in the circulation are associated with an increased risk of myocardial infarction. The effect of pravastatin on plasma lipoproteins and on LDL size has been evaluated in a single-blind, randomised, placebo-controlled study in 24 patients with combined hyperlipidaemia; 12 patients on pravastatin and 12 on placebo. After 16 weeks on pravastatin, plasma total (−15%) and LDL (−23%) cholesterol and apo B (−13%) levels were significantly reduced and apo AI (+6%) had increased. LDL size (measured by gradient gel electrophoresis) had not changed. No adverse effect was observed. The study suggests that, in combined hyperlipidaemia, LDL size is not affected by variation in LDL receptor activity.

    Notes: In a series of 30 patients affected by severe anorexia nervosa (AN) we examined hair samples to detect the prevalence of acquired pili torti (APT). True APT were not detected but in two cases (6.6%) twisted hair was observed. The first case was a 24-year-old woman with secondary amenorrhoea for 9 years, whose body mass index (BMI) was 12.2 kg/m2. She also had severe skin xerosis and hypertrichosis. The second case was a 24-year-old woman with secondary amenorrhoea for 1 year, whose BMI was 11.3 kg/m2. She also had severe skin and lip xerosis, severe effluvium, cystic acne, acrocyanosis, perimylolysis (severe erosion of the dentition) and scars due to cigarette burns on her forearms. Reviewing the literature we noticed that the largest series of pili torti − congenital and acquired − were published by authors from countries such as Israel and Egypt. We therefore hypothesize that, under the same conditions, a genetic factor may predispose to this hair shaft defect.

    Notes: Abstract Hypertension and microalbuminuria are predictors of cardiovascular mortality in type 2 (non-insulin-dependent) diabetes independently of other conventional risk factors. The presence of high triglyceride levels with small and/or dense low density lipoprotein particles is associated with cardiovascular disease. The aim of this study was to analyse the plasma lipids, Na+/Li+ countertransport (a genetic marker of hypertension) and microalbuminuria in type 2 diabetic patients. Plasma lipids were determined in 15 normotensive normoalbuminuric (H−M−), 32 hypertensive normoalbuminuric (H+M−) and 22 hypertensive microalbuminuric (H+M+) type 2 diabetic patients and in 20 sex-and age-matched non-diabetic subjects. Plasma cholesterol was significantly higher in H+M+ patients than in controls (226±38 vs 192±38 mg/dl, mean ±SD). Plasma triglycerides were significantly higher in H+M+ patients than in either controls or H−M− patients (192±117 vs 104±59 and 115±52 respectively). The Na+/Li+ countertransport activity in red blood cells was significantly higher in H+M− and H+M+ patients than in controls, and in the type 2 diabetic patients it was directly related to plasma triglycerides (r=0.53,P〈0.0001) and inversely to high density lipoprotein (HDL) cholesterol (r=−0.43,P〈0.0001). Microalbuminuria, hypertension and elevated Na+/Li+ countertransport activity are thus associated with high triglyceride and low HDL cholesterol levels in type 2 diabetic patients. This atherogenic lipoprotein pattern might at least partially explain the association of microalbuminuria with cardiovascular disease in type 2 diabetes.

    Notes: Abstract Patients with type 1 (insulin-dependent) diabetes mellitus in good metabolic control usually have normal plasma lipid levels yet they have an increased incidence of vascular complications. Abnormalities in the distribution and composition of lipoprotein subfractions might in part be responsible for the macroangiopathy seen in type 1 diabetes mellitus. The plasma lipids, lipoproteins and apolipoproteins were studied in 9 type 1 diabetic patients during conventional insulin therapy and in 14 healthy controls. Plasma lipoproteins were analysed by ultracentrifugation in a zonal rotor to evaluate their concentrations and flotation properties and for compositional analysis. In diabetic patients the mean glycosylated haemoglobin (HbA1c) was 9.44±1.02% and the plasma lipid concentrations were not significantly different from healthy controls. The very low density lipoprotein (VLDL) subclass cholesterol concentrations were no different in diabetic patients and control subjects, but the VLDL cholesterol/triglyceride ratio was significantly lower in diabetic patients than in control subjects (0.34±0.05 vs 0.85±0.14; p〈0.05). The flotation rate of LDL2, the major component of low density lipoprotein (LDL) was lower in the diabetic patients compared with the control subjects. The cholesterol concentrations of intermediate density lipoprotein and LDL3, the minor component of LDL, were significantly higher (0.17±0.03 and 0.83±0.14 mmol/l respectively) in diabetic patients than in control subjects (0.05±0.02 and 0.24±0.08 mmol/l). The flotation properties and cholesterol concentrations of the high density lipoprotein (HDL) subclass, and the protein-lipid composition of LDL2, HDL2 and HDL3, were no different in diabetic patients and control subjects. Diabetic patients had lower apoprotein AII and higher CII and E levels than control subjects. the plasma lipoproteins in type 1 diabetes mellitus are characterized by increased intermediate density lipoprotein and LDL3 concentrations and by abnormal LDL2 flotation properties. These lipoprotein abnormalities might have a role in atherogenesis in type 1 diabetic patients since similar alterations were associated in some recent epidemiological studies with an increased incidence of cardiovascular disease in non-diabetic patients.

    Description / Table of Contents: Zusammenfassung Über hohe Spiegel von immunoreaktivem Calcitonin (iCT) im Blut von Heroinsüchtigen wurde in vorhergehenden Studien berichtet. Da bekannt ist, daß zahlreiche Formen von Calcitonin imBlut und in den Geweben existieren, war es das Ziel der vorliegenden Studie, die immunologische Naturdes calcitoninähnlichen immunreaktiven Materials zu untersuchen, welches im Blut dieser Personengruppe gefunden wird. Wir untersuchten 25 Süchtige,welche seit mehr als 1 Jahr Heroin nahmen und während eines Entgiftungsprogramms mehr als zwei Wochen hospitalisiertwaren. Blutproben wurden zu Beginn des Programms genommen (als die Probanden noch heroinbelastetwaren) und nach 5 bzw. 12 Tagen der Abstinenz. 25 gesunde Versuchspersonen dienten als Kontrollgruppe. Wirbenutzten zwei kommerzielle RIA Kits, welche gegen dasselbe Referenzmaterial (WHO 70-234)calibriert waren, jedoch unterschiedliche Antiseren beinhalten. Ein Antiserum stimmte substanziell mit vorhergehenden Beobachtungen überein, nach denen die Calcitonin-Spiegel während des Gebrauchs von Heroin erhöht sind,das andere jedoch schien jede Veränderung der Hormon-Konzentrationen auszuschließen. Dieses Ergebnis deutetdarauf hin, daß das “calcitoninähnliche Material”, welches bei Heroinsüchtigen gefunden wird, einige Epitopeenthält, welche analog dem Calcitonin-Standard sind und durch das erste Antiserum erfaßt werden. Das“calcitoninähnliche Material” scheint jedoch andere Epitope nicht aufzuweisen, welche imCalcitonin-Standard vorhanden sind und welche durch das zweite Antiserum detektiert werden. Daher scheintdiese Substanz sich vom Standard-Calcitonin 1–32 zu unterscheiden. Es wird diskutiert, daß möglicherweise einCalcitonin-Analogon (Precursor oder Metabolit) in die biochemischen Veränderungen während des chronischen Opiatmißbrauchsinvolviert ist.
    Notes: Summary High levels of immunoreactive calcitonin (iCT) in the blood of heroin addicts were previously reported. As it is well known that multiple forms of calcitonin exist in the blood and in tissues, the purpose of the present study was to investigate the immunological nature of the CT-like immunoreactive material found in the blood of these subjects. We investigated 25 addicts, who had been using heroin for more than one year and were hospitalized for a 2 week detoxication program. Blood samples were drawn at the start of the program (when the subjects were still on heroin) and after 5 and 12 days of abstinence from heroin. Twenty-five healthy subjects served as controls. We used 2 commercial RIA kits, calibrated against the same reference material (WHO 70-234), but employing different antisera. One antiserum substantially confirmed the previous findings of increased levels of calcitonin during heroin use, but the other seemed to exclude any change in the hormone concentrations. This suggests that the “calcitonin like” material found in heroin addicts contains some epitopes similar to those found in the calcitonin standard which are detected by the first antiserum. However it lacks other epitopes which are also present in calcitonin standard and which are recognized by the second antiserum. Therefore, this substance seems to be different from the standard human calcitonin 1–32. A possible involvement of a calcitonin analogue (precursor or metabolite) in the biochemical changes occurring during chronic opiate use is suggested.
