Summary Urodynamic data from patients with stress incontinence were assessed before, and one year after, a modified Marshall-Marchetti-Krantz (MMK) procedure. A comparison was made between patients cured and those not cured by the operation. After the operation the maximal urethral closure pressure was significantly raised in both groups (P = 0.0418). The functional urethral length showed only an insignificant increase. Prior to surgery both functional urethral length and maximal urethral closure pressure were significantly higher in those patients who were cured by the operation compared with those who suffered from recurrent incontinence (P = 0.00116 andP = 0.00090, respectively). These high preoperative values of urethral closure pressure and functional urethral length seem to be important prognostic factors. A comparison with previously published data from patients operated on by anterior colporrhaphy [35] or the Pereyra suspension [36] showed that urethral closure pressure was highest after the MMK procedure. Therefore the MMK operation seems to be specially suitable for incontinent patients with low maximal urethral closure pressure.

Zusammenfassung Die urodynamischen Meßwerte von 88 Patientinnen präoperativ und ein Jahr nach Zysto-Urethropexie nach Marshall-Marchetti-Krantz (MMK) wurden in einer retrospektiven Studie ausgewertet. Bei der Datenanalyse wurden zwei Gruppen gebildet: 56 Frauen, die durch die Operation geheilt waren und 32 Patientinnen, bei denen die Harninkontinenz weiterbestand. In beiden Gruppen wurde der maximale Urethra-Verschlußdruck durch die Operation signifikant erhöht (P = 0,0418). Auffallend war, daß bei den geheilten Frauen bereits präoperativ die funktionelle Urethra länger (P = 0,00116) und der maximale Urethraverschlußdruck höher (P = 0,0090) war als bei den erneut rezidivinkontinenten Patientinnen. Im Vergleich mit früheren Analysen urodynamischer Daten nach vorderer Kolporrhaphie [35] und nach urethro-vesikaler Suspension [36] erzielt die MMK-Operation den höchsten urethralen Verschlußdruck. Deswegen ist die MMK-Operation besonders bei Patientinnen mit einem niedrigen urethralen Verschlußdruck indiziert.
