    Description / Table of Contents: Zusammenfassung Es wird über zwei Fälle von chronischer Quecksilbervergiftung nach externer Anwendung von Bleichcremes zur Behandlung von Sommersprossen berichtet. Über 20–25 Jahre waren 3–10%ige quecksilberpräzipitathaltige Cremes zweimal täglich benutzt worden. Attackenartige Kopfschmerzen, Atembeklemmung und abdominale kolikartige Schmerzen hatten zu mehrfachen Noteinweisungen sowie exploratorischen chirurgischen Eingriffen geführt. Die Diagnose Mercurialismus wurde klinisch aufgrund des schiefergrauen Hautkolorits im Gesicht bei zahlreichen Sommersprossen und der Medikamentenanamnese gestellt sowie elektronenmikroskopisch aus Hautbiopsien gesichert. Die Quecksilberausscheidung im Urin war auf 15,4 µg-% bzw. 5,0 µg-% Gesamt-Hg (Norm 2,0 µg-%) erhöht. Unter einer D-Penicillamin-Therapie stieg die Quecksilberausscheidung im Urin bis auf 25 µg-% an. Weder das schiefergraue Hautkolorit noch die durch die chronische Quecksilbervergiftung hervorgerufenen neurasthenischen Beschwerden wurden durch die Therapie wesentlich beeinflußt.
    Notes: Summary In two female patients chronic mercurialism following topical application of skin bleachers for the treatment of freckles was diagnosed. Over 20 to 25 years 3 to 10% mercurial ointments were applied twice daily. Recurrent attacks of headache, dyspnoea and abdominal colic-like cramps had lead repeatedly to emergency hospitalisations, multiple medical check-ups and surgical exploratory procedures. The diagnosis of mercurialism was made clinically because of a slate-grayish skin hyperpigmentation in the presence of freckles, and the drug-history. Metallic deposits in facial biopsies were demonstrated by electron microscopy. On admission 15.4 µg% and 5.0 µg% total Hg respectively were found in the two patients (normal values up to 2.0 µg%). Following D-penicillamine treatment the mercury excretion via the urine could be elevated up to 25 µg%. Neither the slate-gray facial discoloration nor the neurasthenic complaints were affected by this therapeutic trial. In one patient, a maculo-papular drug-induced skin rash to D-penicillamine developed. Persistent unsolved neurological complaints and cramp-like abdominal pains should remind that percutaneous mercury intoxication through intact skin following skin bleachers is still possible today.

    Notes: Die Einführung des monochromatischen Schmalspektrum-UVB 311 nm und neuer Kombinationstherapien haben in den letzten Jahren die Behandlung der Psoriasis und anderer entzündlicher Hautkrankheiten weitgehend beeinflußt. Hier werden photobiologische und therapeutische Charakteristika der derzeit verfügbaren Breitband-UVB- und Schmalspektrum-UVB 311 nm-Therapie besprochen und auch die Kombinationstherapien berücksichtigt, die in den letzten Jahren die Möglichkeiten der UVB-Phototherapie erweitert haben.

    Notes: Summary Fingernail growth was studied in 28 patients who were receiving systemic treatment with the aromatic retinoid, Ro 10-9359 known as Tigason (etretinate; AR). A group of 15 patients with psoriasis was compared with a group of 13 patients with other dermatoses which are known to have a good response to AR therapy. In the second group, oral therapy with AR appeared to have no effect on the daily growth rate of fingernails (99±4 μm). With AR treatment, the nail-plate growth rate in the psoriatics increased to 132±5 μm.

    Notes: Summary UV-A-induced skin pigmentation was investigated morphologically in semithin and thin sections from 11 volunteers, using different irradiation modalities (single doses of 10,50 and 100 J/cm2). Visible skin pigmentation was observed in all but two probands, and erythema in two; pronounced pigmentation was present after repeated irradiation only. Contralateral non-irradiated, UV-B-irradiated and suntanned skin specimens were used as controls. There was an increase in the number of clear cells in the basal layer (x 1.6) and particularly of large clear cells (x 1.7) after repeated irradiation. Also, the number of melanosomes in melanocytic dendrites (x 2.8) increased after repeated irradiation. The number, size and shape of the melanosome complexes in both basal and suprabasal keratinocytes remained unchanged, even when a distinction was made between central and peripheral location. In contrast, suntanned skin showed an increase in melanosome complexes in basal (x 5.8) and suprabasal (x 3.7) keratinocytes. It is concluded that UV-A-induced skin pigmentation differs in some ways from UV-B or sun-induced melanogenesis, and that the clinical grade of tanning cannot accurately be determined by ultrastructural methods.

    Notes: Summary A new approach studying the characteristics of the stratum corneum is presented: the electrophoretic mobility of corneocytes by laser Doppler spectroscopy. The detergent scrub technique was used for harvesting corneocytes from three body regions (forehead, palm, and sole) of normal persons (n=20) under casual conditions and after thorough defattening of the skin with 70% isopropyl alcohol or petrol. Similarly, cells from the forehead, shoulder, and palm were obtained from 22 acne patients treated with isotretinoin (13-cis retinoic acid) 0.5–0.7 mg/kg body weight (b.wt.)/day for 12–16 weeks, and in patients receiving arotinoid (Ro 15-0778) 192 mg (n=5) or 500 mg (n=5) per kg/b.wt./day for 6 weeks (forehead and shoulder). In another experiment, cell suspensions with a pH ranging from 5.0–7.3 were evaluated. Measurements were performed by dynamic laser light scattering. This laser application allows exact electrophoretic mobility measurements in a short time (3 min). When cells pass the laser beam, the scattered light is frequency-shifted due to the optical Doppler effect. These frequency shifts are analyzed by the heterodyne light beating technique. The analog signal of the photodetector is converted into a power spectrum by Fourier analysis. This power spectrum represents the spectrum of electrophoretic cell mobility distribution. Results showed different electrophoretic mobility values for corneocytes dependent on the topographic region: forehead 1.18±0.16, palm 1.10±0.14, and sole 0.83±0.10 (means±2 SEM) μm cm/Vs. Defattening with isopropyl alcohol decreased the mobility values to 0.90±0.09 (p≤0.01), 0.95±0.10, and 0.77±0.10 μm cm/Vs respectively. Defattening with petrol yielded similar effects as with isoproply alcohol. During treatment with isotretinoin acid, the mobility was consistently reduced, i.e., forehead from 1.18±0.18 to 0.92±0.12 (means ±2 SEM) μm cm/Vs (12th week,p≤0.01). The arotinoid was much less effective than isotretinoin in this respect. It is assumed that the electrophoretic mobility of corneocytes mainly depends on stratum corneum lipids. This is consistent with the results of the topical defattening experiments and the systemic treatment.

    Notes: Abstract One of the primary events in the pathogenesis of acne vulgaris is abnormal follicular keratinization. Since oral isotretinoin therapy reduces follicular hyperkeratinization in acne, our study has been designed to determine whether epidermal lipid composition of the epithelium of sebaceous follicles is affected by isotretinoin treatment. Noninflamed early comedones obtained from ten patients with nodulocystic acne before and after the 6th week of isotretinoin therapy (mean daily dose 0.7 mg/kg b.wt.) were used as probes of the hyperkeratinizing follicular epithelium. Comedonal lipids were analyzed by high-performance thin-layer chromatography. Oral isotretinoin caused a decrease of the comedonal glyceride fraction by 36% (P〈0.01), whereas free sterols and total ceramides increased by 34% (P〈0.10) and 19%, respectively. The changes of comedonal lipids were associated with a significant elevation of the free sterols/cholesterol sulfate ratio of 86% from pretreatment levels (P〈0.05). The isotretinoin-induced changes of the comedonal lipid composition in direction to a pattern of epidermal lipids of normal desquamating stratum corneum are discussed as a possible comedolytic mechanism of oral isotretinoin treatment.

    Notes: Acne inversa is a recurrent, suppurative disease manifested by abscesses, fistulas, and scarring. Once considered to be a disease of the apocrine glands, it is actually a defect of follicular epithelium. Thus, the term hidradenitis suppurativa is a misnomer and should be abandoned. In cases of familial acne inversa, the pattern of transmission and number of affected individuals are consistent with autosomal dominant inheritance. Aetiological factors such as hyperandrogenism, obesity, smoking and chemical irritants are not consistently associated with the affection. Bacterial involvement is not a primary event in acne inversa, but is secondary to the disease process. Potential complications include dermal contraction, local or systemic infection due to the spread of microorganisms, systemic amyloidosis, arthropathy, and squamous cell carcinoma. As spontaneous resolution is rare and progressive disability is the rule, early definitive surgical intervention is advisable. The surgical procedure of choice in most cases is wide local excision and healing by secondary intention. Pharmacotherapeutic drugs, including synthetic retinoids and antiandrogens, do not prevent progression of the disease.
