    Description / Table of Contents: Zusammenfassung Eine hormonale Kontrazeption bewirkt Veränderungen des T3-und T4-Tests, die auf eine erhöhte Bindungskapazität des thyroxin-bindenden Globulins (TBG) schließen lassen. Mit Hilfe der Laurell-Elektrophorese und einem von den Behringwerken entwickelten Anti-TBG-Serum ist jetzt die quantitative Bestimmung des TBG, des wichtigsten Trägerproteins der Schilddrüsenhormone, ohne größeren Aufwand möglich. Untersucht wurden insgesamt 137 Seren gesunder Frauen, die verschiedenartige hormonale Kontrazeptiva über mindestens 3 Monate einnahmen. Die hormonale Kontrazeption führt generell zu einem signifikanten Anstieg der TBG-Konzentration im Serum. Dabei erzielen die Präparate zum Teil sehr unterschiedliche Konzentrationserhöhungen. Präparate mit einer betont östrogenen Wirksamkeit bewirken im Mittel einen weit höheren Anstieg der TBG-Serum-Konzentration als die mehr gestagen-betonten Präparate. Der unterschiedliche Anstieg ist statistisch hoch signifikant. Die Vermehrung des TBG-Gehaltes im Serum unter hormonaler Kontrazeption ist demnach östrogenbedingt, dagegen ist das Ausmaß der TBG-Vermehrung von der Art des beteiligten Progestagens abhängig.
    Notes: Summary The use of oral contraceptives results in an increase in thyroid-binding-capacity in conformity with a change of results by T3/T4-tests. Using Laurell-electrophoresis and an anti-serum produced by Behringwerke the determination of the thyroid-binding-globulin (TBG) in serum is easy to handle. TBG was determined in serum from 137 women using oral contraceptives for more than 3 months. The apllication of oral contraceptives is followed by a statistically significant increase in the TBG level in serum. Thereby oral contraceptives change the TBG-level in a different degree. Preparations with a marked estrogen potency on the average effect a higher TBG-concentration than those with progesterone activity derived by menses-delay-test. The difference of TBG-level is statistically significant. According by the increase of TBG-concentration in serum is caused by estrogen, the extent of the increase is modified by the progesterone component.

    Description / Table of Contents: Zusammenfassung Bei der Mutter eines an juveniler Leberzirrhose bei Alpha-1-Antitrypsinmangel erkrankten Kindes wurde in ihrer erneuten Schwangerschaft der fetale Phänotyp Pi MZ durch Isoelektrofokussierung des Fruchtwassers festgestellt. Die quantitativen Werte des Alpha-1-Antitrypsins entsprachen denen von Kontrollfruchtwasserproben. Bei der Entbindung bestätigte eine Nabelschnurblutuntersuchung diesen Befund. Somit konnte erstmalig pränatal das Vorliegen eines schweren Alpha-1-Antitrypsinmangels Pi Z ausgeschlossen werden. In diesem Zusammenhang wird auf die klinische Bedeutung des Alpha-1-Antitrypsinmangels, seine Häufigkeit in der europäischen und nordamerikanischen Bevölkerung und die Prognose bei Vorliegen des Phänotyps Pi Z hingewiesen.
    Notes: Summary A mother had a child with cirrhosis of the liver and alpha-1-antitrypsin deficiency. In a subsequent pregnancy the fetal phenotype Pi MZ was detected by isoelectrofocussing in the amniotic fluid. Quantitative assay of alpha-1-antitrypsin gave results in the normal range. Umbilical vein blood analysis confirmed the antenatal findings. In this case it has been possibel to roole out the disease before birth. In this context the clinical importance of alpha-1-antitrypsin deficiency is stressed, its frequency in the European and North-American population and the prognosis with phenotype Pi Z.

    Notes: Summary The alphafetoprotein (AFP) concentration in maternal serum was determined before and after chorionic villus sampling (CVS). A significant increase of 20% or more in the pre-CVS level was noted immediately after sampling in 59% of 837 pregnancies indicating some degree of feto-maternal haemorrhage. The increase in the AFP concentration in maternal serum was correlated with the weight of the tissue sample but not with the number of sampling attempts. A correlation of AFP increase and frequency of spontaneous abortions following CVS was suggested only in the group with an AFP increase of more than 100% or with a continuing rise in the first hour following CVS. CVS in early pregnancy obviously did not interfere with maternal serum AFP screening for neural tube defects in the second trimester. Although AFP measurement before and after CVS seems to have no immediate diagnostic application, in the research phase of CVS it may help to identify those procedures that are the least traumatic.

    Notes: Abstract Serum samples from 320 women with chromosomally normal fetuses and from 50 women with fetuses affected by Down's syndrome were assayed retrospectively for human chorionic gonadotropin (hCG), pregnancy-specificβ 1 glycoprotein (SP1), alpha fetoprotein (AFP), and unconjugated estriol (uE3) between 14 and 21 weeks of gestation. Nonparametric discriminant analysis was applied to calculate Down syndrome risks on the basis of various combinations of serum parameters. At a risk threshold that falsely identifies 5% of controls as being affected, 46 to 48% of Down syndrome pregnancies were detected by combinations of hCG/AFP, hCG/AFP/uE3, and hCG/AFP/uE3/SP1 respectively. HCG, AFP, and uE3 were assayed in 652 serum samples from women who underwent amniocentesis because of maternal age (≥35 years in this prospective study). 49% of women with euploid fetal karyotype, 8 of 10 pregnancies with Down's syndrome, and 3 pregnancies with sex chromosomal anomalies were identified as being at an increased risk (〉1:380).
