Zusammenfassung Die posttraumatische Knocheninfektion kann Folge einer offenen oder einer primär geschlossenen aber operativ versorgten Fraktur sein. Die operative Behandlung - Eröffnung des osteomyelitischen Herdes, Ausräumung von nekrotischem Knochengewebe, Belassung oder Entfernung von Osteosynthese-material, Spülbehandlung, Totalausräumung mit Anlagerung von Eigenspongiosa -ist Aufgabe der stationären Behandlung. Sind diese Behandlungsmaßnahmen abgeschlossen und besteht als Restzustand eine nach außen drainierende Fistel, ist der Zeitpunkt gekommen, den Patienten in die ambulante Osteomyelitis-Sprechstunde zu entlassen. Die nun bestehende chronische Infektion bedarf einer langdauernden, kontinuierlichen Überwachung. Der Wegfall der Gefährdung durch Hospitalkeime ermöglicht eine sinnvoll geplante Chemotherapie, die unter den konservativen Maßnahmen im Vordergrund steht. Laufende kurzfristige bakteriologische Überprüfung des Keimwechsels und der Resistenzlage sowie die Beurteilung von Fortbestand oder Ausheilung der Knocheninfektion sind Aufgaben der Langzeitüberwachung in einer Osteomyelitis-Sprech-stunde. Diese hat die Abkürzung und sinnvolle Ergänzung des stationären Aufenthaltes zum Ziel. Eine Karteikarte für eine Osteomyelitis-Langzeitüberwachung soll anhand eines Schemas im Diapositiv besprochen werden.

Summary A post-traumatic bone infection can be the result of an open or primarily closed but operatively treated fracture. The surgical treatment, consisting of the opening of the osteomyelitis focus, the removal of necrotic bone tissue, the retention or elimination of osteosynthesis material, wash outs, total clearance and implantation of autogenous spongiosa, is a task that has to be carried out in hospital. Once these therapeutic measures have been completed and only an externally draining fistula remains, the time has come to discharge the patient and keep him under the supervision of a special “osteomyelitis consulting hour” for out-patients. The now present chronic infection requires continuous long-term supervision. The elimination of the threat of hospital bacteria permits an adequately planned chemotherapy which is the most important feature of the conservative treatment. Continuous controls of bacterial changes and of resistance carried out at short intervals and assessment of persistence or healing of the infection in the bones are the tasks of the long-term supervision undertaken by the “osteomyelitis consulting hour”. Its aim is the shortening and supplementation of the treatment in hospital. With the aid of a slide a special index card to be used in the long-term supervision of osteomyelitis is discussed.
