    Notes: Abstract Strains from type culture collections and clinical isolates belonging to theAeromonas andPseudomonas genera were identified with conventional tests. Production of extracellular enzymes and haemolysins were detected by simple plate agar methods. The following enzymes were found to be of special value for a rapid and simple classification of certain species in both genera: protease (casein and gelatin agar), lecithinase (lecithin agar), and deoxyribonuclease (DNA agar). Elastase, staphylolytic enzyme, lipase, ribonuclease, amylase, and egg yolk reaction were other enzymes studied. However, these tests were not positive for more than 90% of any species.A. hydrophila, A. salmonicida, andP. aeruginosa were haemolytic on agar containing rabbit erythrocytes.

    Notes: Abstract 347 strains from human infections were identified by gas-liquid chromatography of metabolic products and by conventional tests. Simple agar-plate assays were used to analyze the ability to form extracellular proteins. More than 90% of all strains were hemolytic on agar containing rabbit erythrocytes and all were gelatinase producers. All strains ofC. bifermentans, C. sordelli, andC. sporogenes were also caseinolytic on skimmed-milk agar, but strains ofC. perfringens, C. novyi types A and B were not. Less than 10% ofC. perfringens strains were producers of elastase and staphylolytic enzyme and all other species were non-producers. AllC. perfringens, C. novyii, C. bifermentans, andC. sordelli were lecithinase producers, butC. sporogenes was not. All strains ofC. sporogenes formed deoxyribonuclease, while a varying number of the other species showed a positive reaction.

    Notes: Abstract Bronchopulmonary dysplasia (BPD) is a chronic lung disease often occurring in ventilator-treated very low birth weight infants. The aetiology of BPD is multifactorial and pulmonary immaturity, high oxygen concentrations, peak inspiratory pressure levels and large tidal volumes during prolonged mechanical ventilation are important factors. We measured in tracheobronchial aspirate fluid (TAF) the concentrations of the pro-inflammatory cytokines tumour necrosis factor α, interleukin-1β (IL-1β), IL-6, IL-8, and IL-1 receptor antagonist in infants requiring artificial ventilation for BPD (n = 17) or respiratory distress syndrome (RDS) (n = 15) or postoperatively after surgery (n = 15). The median levels of all studied cytokines in TAF were higher in infants with BPD without local or systemic corticosteroid treatment compared to the median TAF levels of BPD neonates treated with corticosteroids (P = 0.06–P 〈 0.01). The neonates with BPD not treated with corticosteroids also showed higher levels of the five studied cytokines in TAF compared to infants on short-time ventilator treatment (P 〈 0.01–P 〈 0.001) and compared to neonates with RDS (P = 0.07–P 〈 0.001). The corticosteroid treated neonates with BPD had TAF cytokine levels approaching those of the control neonates. Conclusion Tumour necrosis factors α, IL-1β, IL6, IL8 and IL1ra were markedly elevated in tracheobronchial aspirate fluids from neonates with bronchopulmonary dysplasia. Corticoid treatment seemed to reduce these levels.

    Notes: Pleiotropic mutations (xcp) affecting secretion of proteins in Pseudomonas aeruginosa have been previously characterized and mapped at 0 min, 55 min and 65 min. Genomic libraries of this organism have been constructed and the genes xcp-5 and xcp-54, located at the 55 min region, were cloned using the adjacent met allele as a marker, and complementation of xcp strains. From our linkage and cloning analysis, the most probable gene order in this region appears to be pyrD., xcp-5/xcp-54/met-9011/oru-314/trpF/leu-10. Restriction mapping and transposon (Tn 1725) insertion mutagenesis demonstrated that: (i) the overall size of DNA necessary for xcp expression was 9 kb, (ii) the two loci are not adjacent on the chromosome, and (iii) the two loci are expressed independently. The xcp-5 gene has been subcloned on a 4 kb Eco RI fragment.

    Notes: Abstract An agar dilution method was used to determine thein vitro sensitivity of differentPseudomonas andAeromonas species to sulphonamide, tetracycline, colistin, gentamicin, tobramycin, ampicillin, carbenicillin, and cephaiothin.P. aeruginosa was generally sensitive to carbenicillin, colistin, tobramycin, and gentamicin.P. putida andP. fluorescens were generally resistant toΒ-lactam antibiotics but sensitive to gentamicin and tobramycin.P. cepacia andP. maltophilia were mostly resistant to colistin, gentamicin and tobramycin. OtherPseudomonas species were usually more sensitive to the antibiotics tested. Strains ofAeromonas hydrophila were mostly resistant toΒ-lactam antibiotics but sensitive to other antibiotics, while strains ofA. shigelloides were very sensitive to all antibiotics tested.

    Notes: Abstract The relationship between urine interleukin-6 (IL-6) and interleukin-8 (IL-8)/creatinine quotients and99mTc-dimercaptosuccinic acid (DMSA) scintigraphy, performed within 10 days of acute first-time pyelonephritis and after 1 year, was studied in 41 children. The urine IL-6 and IL-8/creatinine quotients were also related to the urineN-acetyl-β-D-glucosaminidase (NAG) and albumin/creatinine quotients. Presence of DMSA uptake defects, reflecting local inflammation, in children in the acute phase of pyelonephritis, were associated with elevated urine IL-6/creatinine quotients (median 27 pg/μmol); in children without DMSA changes there was no increase in quotients (median non-detectable) (P〈0.05). Persistent DMSA changes at the 1-year follow-up, probably reflecting renal scarring, were only seen in children with increased urine IL-6/creatinine quotients in the acute phase (P〈0.01). No correlation was found between urine IL-8 and DMSA uptake defects. Vesicoureteral reflux (VUR) at 6–8 weeks did not correlate with the urine cytokine levels in the acute phase. The urine excretion of NAG and albumin, reflecting renal dysfunction, was associated with values of both urine IL-6 and IL-8/creatinine quotients, but not with DMSA defects or VUR. Thus, the initial urine IL-6/creatinine quotients might be used as an indicator of risk for persistent renal damage in acute pyelonephritis.

    Notes: Abstract The efficacy of the antimicrobial removal device in facilitating isolation of bacteria from blood containing antibiotics was evaluated in a multicenter study. Blood specimens from 143 patients who had culture-proven septicemia and had been on antibiotic treatment for at least 24 hours were recultured with and without the aid of the device. Septicemia persisted in 33 of the patients and use of the device improved the isolation rates: 63 % of the gram-positive bacteria and 32 % of the gram-negative bacteria were isolated only with the aid of the device. Use of the device was an advantage in 10 of 13 cases where the antibiotic concentration in the bottles exceeded the MIC for the infecting microorganism but in only 5 of 19 cases where the concentration was lower than the MIC. It is concluded that the antimicrobial removal device may be useful in patients already on antibiotic treatment, but that its use is limited by the cost and the amount of work involved.

    Notes: Abstract The case history of 88 patients with blood cultures positive forEscherichia coli and evidence of systemic disease was reviewed, and theEscherichia coli blood isolates tested for P-fimbriation. Fifty-five strains (63 %) were P-fimbriated. Patients with a positive urine culture had a higher incidence of P-fimbriatedEscherichia coli strains (53/75, 71 %) than patients with a negative urine culture (2/13, 15%). Patients with no predisposing factors, such as instrumentation of the urinary tract or a chronic disease, had a significantly higher frequency of P-fimbriated strains (31/36, 86%) compared to patients without such underlying factors (p 〈 0.01). There was no significant difference in clinical parameters or in frequency of P-fimbriated strains between patients with one positive blood culture and those with two or more positive cultures. The high incidence of P-fimbriatedEscherichia coli strains in these patients is thought to depend on the ability of such bacteria to cause acute pyelonephritis, which in many cases precedesEscherichia coli bacteremia.
