    Description / Table of Contents: Summary Multiple hamartoma and neoplasia syndrome, or Cowden's disease, is a genodermatosis inherited as an autosomal dominant trait with variable expressivity. We describe a woman with classic mucocutaneous features of the syndrome: facial papules, acral keratoses, oral mucosal papillomatosis, oral polyps and scrotal tongue. As a child she had strabismus. Her family history is positive for the disease, and a sister died of malignant melanoma. The check-up revealed chronic lymphocytic thyroiditis. A checklist for use in screening patients with Cowden's disease is proposed.
    Notes: Zusammenfassung Berichtet wird über eine 42jährige Patientin mit einem Syndrom der multiplen Hamartome und Neoplasien. Die mukokutanen Läsionen bestanden aus fazialen Papeln, akralen Keratosen, Mundschleimhautpapillomen mit einem oralen Polypen und einer Lingua scrotalis. Das Leiden zeigt als häufigste Assoziation Schilddrüsenerkrankungen; in unserem Fall fand sich eine Hashimoto-Thyreoiditis. Die Familienanamnese für Hautveränderungen war positiv; die Mutter und eine Schwester sollen ähnliche Hauterscheinungen aufweisen. Darüber hinaus ist eine Schwester an einem malignem Melanom verstorben. Ein bösartiger Tumor, insbesondere ein Mamma- oder Schilddrüsenkarzinom bei der Patientin selbst war nicht gegeben. Das Cowden-Syndrom kann mit malignen Tumoren assoziiert sein. wobei die Hautveränderungen vor der 2. Dekade auftreten und zur Frühdiagnose beitragen können.

    Description / Table of Contents: Summary Melatonin (N-acetyl-5-methoxytryptamine) is a hormone with multiple functions in humans, produced by the pineal gland and stimulated by β-adrenergic receptors. Serum melatonin levels exhibit a circadian rhythm with low levels during the day, rise in the evening and maximum levels at night between 2 and 4 a.m.. Melatonin participates in the regulation of several physiological processes such as seasonal biological rhythm, daily sleep induction, aging and modulation of immunobiological defence reactions. Furthermore, melatonin has a highly lipophilic molecular structure facilitating penetration of cell membranes and serving as an extra- and intracellular free radical scavenger. Melatonin seems to quench mainly hydroxyl radicals, the most damaging of all free radicals. Melatonin may play a role in the etiology and treatment of several dermatoses e.g. atopic eczema, psoriasis and malignant melanoma. The influence of melatonin on hair growth is another aspect. Topical application of melatonin inhibits the development of UV-erythema. Penetration through skin after topical application and oral bioavailability auxit further investigations on the pharmacokinetic and pharmacodynamic actions of melatonin.
    Notes: Zusammenfassung Melatonin (N-Acetyl-5-Methoxytryptamin) ist ein unter dem Einflußβ-adrenerger Rezeptoren von der Glandula pinealis produziertes und sezerniertes Hormon mit vielfältigem Wirkungsspektrum. Die Ausschüttung des Hormons folgt einem zirkadianen Rhythmus mit niedrigen, gleichmäßigen Spiegeln tagsüber und einem abendlichen Ansteigen und nächtlichen Peak zwischen 2.00 und 4.00 Uhr. Melatonin beeinflußt den saisonalen Biorhythmus, den Schlaf-Wach-Rhythmus, den Alterungsprozeß und moduliert immunbiologische Abwehrfunktionen. Zusätzlich besitzt Melatonin unabhängig von einem Rezeptorsystem durch seine starken lipophilen Eigenschaften die Fähigkeit, frei durch Zellmembranen zu diffundieren und im extra- wie intrazellulären Raum als Radikalfänger zu wirken. Die Affinität zu dem stark schädigenden Hydroxylradikal ist besonders groß. In der Dermatologie ergeben sich für Melatonin Perspektiven bei einigen Hauterkrankungen wie zum Beispiel dem atopischen Ekzem, der Psoriasis und dem malignen Melanom. Der Einfluß von Melatonin auf das Haarwachstum ist ein weiterer Aspekt. In topischer Anwendung hat Melatonin eine suppressive Wirkung auf die Ausbildung eines UV-Erythems. Über die Penetrationseigenschaften durch die Haut und die orale Verfügbarkeit liegen einige Erkenntnisse vor, die durch weitere Untersuchungen der Pharmakokinetik und Pharmakodynamik ergänzt werden können.

    Notes: Protective creams (PCs) play their part in the prevention of occupational contact dermatitis, even though efficacy data are frequently lacking. 4 different commercially-available PCs were evaluated against a set of 4 standard irritants (10%, sodium lauryl sulfate (SLS) 1% sodium hydroxide (NaOH), 30% lactic acid (LA) and undiluted toluene (TOL) in the repetitive irritation test (RIT) in humans described by Frosch and Kurte. 20 subjects were tested on the paravertebral skin on the mid-back. Irritation was assessed by visual scoring, transepidermal water loss (TEWL) as an indicator of epidermal barrier function, and colorimetry as a parameter of inflammation. Very different protective effects of the PCs on irritation by chemical substances were detectable. All products were very effective against SLS irritation. No PC provided significant protection against toluene. 3 products showed a partially protective effect against all ionic irritants, while the 4ih showed less protection against SLS and NaOH, and even amplification of inflammation by TOL. Considering the range of PC effects from good protection 10 increased irritation, depending on the irritant, the need for careful selection of PCs for specific workplaces is stressed.

    Notes: 2 workers at an aircraft factory were employed in a plasma sparying unit. Soon after they were equipped with new semisynthetic working suits, they started to complain of pruritic eruptions after heavy exposure to metal dust. They noted that the dust was not as easily blown off the clothes by pressurized air as previously. Clinical findings consisted of discrete macular and papular lesions, partly follicular, on the ventral and medial thighs. Atopy score, IgE level and a standard series of prick tests ruled out atopic disposition. Patch tests revealed no reactions. A diagnosis of occupational airborne irritant contact dermatitis from metal dust was therefore made. To elucidate the role of the working suit, extensive physical investigations of the physical properties of the textile were performed. Microscopic pictures at low magnification showed more dust particles on the semisynthetic working suit, compared to the former pure cotton suit. This impression could not be confirmed by particle count because of too uneven particle distribution at higher magnification. Hairiness of the 2 textiles was low and ruled out irritation by the semisynthetic textile itself. No difference in electrostatic properties between the 2 working suits could be established either. Both textiles showed high static electrical propensity. When use of the semisynthetic overall was discontinued, the patients reported no recurrence. The 2 reported cases show that the physical properties of the textiles of working suits can be of interest and should be considered in the management of dust-induced irritant contact dermatitis. Extended physical examinations of textile failed to explain the clinical observation; further investigations might establish physical characteristics of textiles for such adhesion qualities.

    Notes: Products intended for individuals in contact with strongly adhering dirt often contain grit. Various clinical test methods have been developed for evaluating the potential of personal washing products to induce skin irritation. In the present study, differences in the irritant effects of washing products containing naturally-derived grit and synthetic grit were investigated in a forearm wash test. The forearms of 16 test subjects were washed in a total of 18 treatments (4 per day for 4 days, with 2 treatments on the 5th day). Treatment consisted of continuous washing for 2 min by a technician, who gently slid his fingertips with the lather up and down the forearm. Non-invasive instrumental measurements of skin barrier function were performed. Repetitive washing for 1 week lead to increased TEWL values, skin redness and decreased stratum corneum hydration. Results indicate differences in irritancy potential due to different types of grit, their surface and concentration. It is concluded that the repeated wash test seems to be adequate for rating personal washing products that contain grit.
