    Notizen: Abstract Phosphorylcholine (PC) antibodies in serum from patients with pulmonary infection, and from normal individuals, were studied. Anti-PC antibodies were detectable in the serum from normal individuals at mean concentrations of 320 μg/ ml for the IgG class and 110 μg/ml for the IgM class. Concentrations of anti-PC antibodies which were higher than normal for both the IgG and IgM classes were observed in the serum in pulmonary infection (1,440 μg/ml and 210 μg/ml, respectively). Despite the significant difference in the concentration of anti-PC antibodies, the PC-specific B cell precursor frequency in the peripheral blood lymphocytes showed no difference between normal individuals and the patients with pulmonary infection, or between the acute phase and the chronic phase in a single patient with chronic pulmonary infection. Serologically, the purified IgG anti-PC antibody did not share the cross-reactive idiotype of TEPC 15, which is the most common idiotype of the murine anti-PC antibodies. However, the purified IgM anti-PC antibody expressed a very weak cross-reactive idiotype of TEPC 15. It appears from these studies that human anti-PC antibodies may play an important biological role in pulmonary infection by microorganisms which possess a PC determinant.

    Notizen: Summary We present the unusual case of 16-year-old girl who developed intractable convulsions five days after the onset of a cold. Meningeal signs, lymphopenia, proteinuria, and lupus anticoagulant were also present. Treatment with anticonvulsants, antituberculous agents, and adenine arabinoside were ineffective. The initiation of methylprednisolone pulse therapy immediately resolved convulsions and fever. The diagnosis, suggested by the clinical course and the marked improvement of the meningoencephalitis by pulse therapy, was an encephalitic form of acute disseminated encephalomyelitis. Clinical and laboratory findings indicated that an immune disorder may have triggered an abnormal response to a viral infection leading to this patient's neurologic disorder.

    Notizen: Abstract A 34-year-old Japanese woman developed spiking fever, splenomegaly, arthritis, neutrophilia, hyperferritinaemia (22517 ng/ml), elevated C-reactive protein (9.1 mg/ml) and severe thrombocytopenia (1.7×104/μl). The patient had depressed antithrombin III activity and abnormally high concentrations of both fibrin degradation products and thrombin-antithrombin complexes. This condition was resistant to high-dose prednisolone therapy (120 mg/day) and non-steroidal anti-inflammatory drugs. We initiated oral methotrexate therapy (7.5 mg/week, orally) with a favourable outcome. The patient's spiking fever subsided on the first day of methotrexate administration. Elevated levels of ferritin and C-reactive protein in the sera rapidly normalised. Methotrexate rapidly improved the disease state which suggested that methotrexate act via modulation of cytokine production or secretion.

    Notizen: Abstract The psychiatric and cognitive condition of a patient with lupus psychosis was evaluated. Using a device that detects the corneal reflection of infrared light, the patterns of eye tracking movements were recorded before the onset of lupus psychosis, after remission, and again 1 year later. Electroencephalographic findings and cerebrospinal fluid levels of both interferon α and interleukin-6 were also obtained longitudinally. Electroencephalographic findings and clinical signs were correlated to the levels of interferon α in cerebrospinal fluid. Analysis of exploratory eye movements revealed marked decreases in the number of eye fixations, mean eye-scanning length and total eye-scanning length. Even though the lupus psychosis resolved and the electroencephalographic findings became normal, the eye movement patterns showed remaining deterioration. It was concluded that analysis of exploratory eye movements in patients with systemic lupus erythematosus may be useful in diagnosing lupus psychosis, and may also present a diagnostic clue to subclinical lupus psychosis.

    Notizen: Summary To add to our knowledge of collagen diseases, plasma fibronectin (FN) in patients with systemic lupus erythematosus (SLE) has been measured, and it was determined that the plasma FN value in those with SLE was 454±36 μg/ml, is significantly higher than the FN value in normal subjects (234±21 μg/ml. Further, the plasma FN value of patients with active SLE was significantly higher (591±46 μg/ml than that of patients with non-active SLE (287±31 μg/ml. The plasma FN value of SLE patients was also seen to be associated with the peripheral blood platelet count and with the dose level of the corticosteroid hormone administered to patients. In active SLE patients, it was similarly found that the plasma FN value had a significant correlation with the peripheral blood lymphocyte count and with the dose level of the corticosteroid hormone given to patients. Since the plasma FN value is known to be high in untreated SLE patients, it was felt that the increase of the FN value in SLE patients is not due to the effect of the corticosteroid but to the disease itself.
