Zusammenfassung Postoperative (p.o.) Adhäsionen werden möglicherweise durch eine eingriffsbedingte Depression der intraperitonealen Fibrinolyse begünstigt. An Patienten wurde deshalb die biochemische Wirkung eines p.o. lokal applizierten Fibrinolytikums (Varidase, Lederle, Wolfratshausen) in Peritonealsekret und Blut dosis- und zeitabhängig untersucht. Fibrinolyseparameter (Plasminogen, α2 Antiplasmin, α2 M-Plasminkomplexe, jeweils chromogene Substratassays) sowie Plasminogenaktivatoren und spontane Fibrinolyse (Fibrinplattentests) zeigten 8 h p.o. ein Wirkungsmaximum nach 4 Ampullen (ä 100 000 LE. Streptokinase u. 25 000-100 000 LE. Streptodornase), bei einer maximalen Wirkungsdauer von 48 h. Noch höhere Dosierungen erzeugten gelegentlich Anzeichen einer systemischen Hyperfibrinolyse (auf Fibrinplatten). Wundheilungsstörungen traten nicht auf.

Summary Postoperative (p.o.) adhesions are most likely induced by the depression of intraperitoneal fibrinolysis provoked by the surgical procedure. Therefore, the dose and time-dependent biochemical effect of a locally applied fibrinolytic agent (Varidase, Cyanamid Lederle, FRG) was studied in patients' peritoneal fluid and plasma. The parameters of fibrinolysis (plasminogen, α2-antiplasmin and α2 M-plasmin complexes with chromogenic substrates, plasminogen activators and spontaneous fibrinolytic activity on fibrin plates) indicate a maximal effect 8 h p.o. after 4 ampoules (100,000 IU streptokinase and 25,000 IU streptodomase). Elevated fibrinolysis lasted as long as 48 h. Higher amounts eventually induced systemic hyperfibrinolysis (on fibrin plates). Wound healing was not impaired.
