    Notizen: Several halogenated anaesthetic agents have been associated with hepatotoxicity. We report a case of fulminant, fatal hepatic necrosis after uneventful isoflurane anaesthesia in a patient without previous liver disease, who may have been sensitised by previous exposure to enflurane. Although no anti-trifluoroacetyl antibodies could be detected in the patient's serum, isoflurane hepatotoxicity seems very likely to be the reason for fulminant hepatic failure in this patient.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung. Von 63 Patienten, welche eine tiefe anteriore Rectumresektion erhielten, wurden 39 mit einer direkten coloanalen Anastomose (CAA), 24 unter Zwischenschaltung eine Colon-J-Pouches als colonpouchanale Anastomose (CPA) rekonstruiert. Lokalseptische Komplikationen traten in 20,5 % aller Patienten nach CAA gegenüber 12,5 % nach CPA auf, die Stuhlfrequenz war nach zusätzlicher Pouchanlage 3,3 pro 24 h verglichen mit 5,2 pro 24 Std nach direkter coloanaler Anastomose (p = 0,053). Die postoperative Kontinenz war in der Pouchgruppe tendentiell besser (n. s.), die Analmanometrie zeigte lediglich einen signifikanten postoperativen Abfall des Ruhedruckes nach direkter coloanaler Anastomose. Die Ergebnisse sprechen für die Anlage eines primären Colonpouches, da die lokalseptische Komplikationsrate nach colonpouchanaler Anastomose niedriger und das funktionelle Ergebnis tendentiell günstiger ist.
    Notizen: Summary. Thirty-nine of 63 patients undergoing deep anterior rectal resection received a straight coloanal anastomosis (CAA); the remaining 24 patients additionally had a colon-j-pouch (CPA) constructed. After pouch-anal anastomosis, local septic complications occurred in 12.5 % of patients compared to 20.5 % after coloanal anastomosis. Stool frequency after pouch-anal anastomosis was 3.3 per 24 h compared to 5.2 per 24 h after straight anastomosis within the first year after ileostomy closure (P = 0.053). Continence was slightly better in the pouch group (n. s.), and anal manometry showed a significant postoperative decrease only in resting pressure after straight coloanal anastomosis (P 〈 0.001). This study supports the construction of a colon-j-pouch after deep rectal resection, as the pouch-anal anastomosis has fewer local septic complications and seems to improve functional outcome.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung. Wir berichten über den Fall einer subacromialen Schulterluxation, die durch die Kombination von einer cranialen Glenoidfraktur und einer Acromionfraktur möglich wurde. Der Patient hatte sich diese ungewöhnliche Kombinationsverletzung im Rahmen eines Handballspiels zugezogen. Erst die weiterführende Diagnostik mit einem CT der Schulter zeigte das ganze Ausmaß der Verletzung. Es erfolgte die osteosynthetische Versorgung der Glenoidfraktur nach Reposition der Schulter. Das postoperative Ergebnis zeigte eine gute anatomische Rekonstruktion der glenoidalen Gelenkfläche. Der Patient hat nach Abschluß unserer Behandlung eine gute funktionelle Wiederherstellung der Beweglichkeit im Schultergelenk mit Abduktion bis 180 ° und ist wieder voll berufstätig.
    Notizen: Summary. We report on a case of subacromial shoulder dislocation, which resulted from a combination of a cranial glenoid fracture and an acromial fracture. The patient sustained this rare injury while playing handball. The whole implication of the injury was revealed only after extensive workup with CT scan of the shoulder. After reduction of the shoulder dislocation, the glenoid fracture was stabilized by osteosynthesis. The postoperative results showed anatomical reconstruction of the glenoid joint surface. After completion of therapy, the patient has achieved good functional results with a full range of motion (abduction 180 °) and has been able to work full-time again.

    Notizen: Abstract PURPOSE: Restorative proctocolectomy is a standard procedure in the surgical treatment of ulcerative colitis and familial adenomatous polyposis. The radical removal of the colorectum with construction of an ileostomy often results in high stoma losses. These may lead to changes in the electrolyte and acid-base balance and to alterations in renal and suprarenal gland function. METHODS: In this study 33 patients who received an ileoanal pouch before and after proctocolectomy were investigated at different time intervals for electrolyte changes, alteration of the acid-base balance, kidney function, and hormonal changes of the suprarenal glands. Measurements were performed before proctocolectomy, ten days after proctocolectomy with ileal pouch-anal anastomosis under protective loop ileostomy, before ileostomy closure, and 6 to 12 months after ileostomy closure. Neither acute renal failure nor other vital complications were observed. RESULTS: Statistical analysis showed a significant decrease of urine pH to 5.4±0.22 (before ileostomy closure) and metabolic acidosis (pH 7.32±0.04; base excess −1.3±5.6 (before ileostomy closure)). Likewise, we found a decrease in renal clearance to 86 ml/minute (before ileostomy closure) without signs of tubular damage. The most important change during the phase with ileostomy was a functional secondary hyperaldosteronism with aldosterone levels of 63.2±70.8 ng/dl (before ileostomy closure). In comparison with preoperative levels, there was a ten-fold increase in mineralocorticoid adrenal activity. Additionally, during the period with protective ileostomy, the hepatic synthesis of aldosterone-18-glucuronide was only slightly increased, and the cortisol/cortisone ratio was extremely decreased. CONCLUSIONS: These results show that restorative proctocolectomy with ileal pouch-anal anastomosis and protective loop ileostomy significantly influences fluid, electrolyte, and acid-base balance. Functional secondary hyperaldosteronism is of central importance for subsequent renal recompensation. Approximately one-half year after ileostomy closure, the endogenous hormones with mineralocorticoid effects returned to normal levels.

    Notizen: Abstract γ-ray spectroscopic techniques have been applied to measure properties of excited states (E *〈2MeV) in210Pb,212Po, and213At after populating these neutron rich (N=128) nuclei via18O induced few-nucleon transfer reactions on208Pb and209Bi targets. In212Po an isomeric state is located atE *=1,477 keV with a halflife oft 1/2=14.7±0.3 ns. This state is interpreted to be the 8+ yrast level which decays to the ground state via the measured $$8^ + \left( {121.1 keV} \right)6^ + \left( {223.3 keV} \right)4^ + \left( {405.1 keV} \right)2^ + \left( {727.8 keV} \right)0^ + $$ γ-cascade.α-decay (E α≈10.2 MeV) of the isomer is also observed. In210Pb the time spectra for members of the 4+ (297.8keV) 2+ (799.4keV)0+ γ-cascade show a delayed component with a halflife oft 1/2=152±13 ns which is attributed to the known 8+ yrast state atE *=1.27 MeV. For213At first results on theγ-decay of excited states are presented.

    Notizen: Abstract The hyperfine Splitting of the 80.6-keVγ transition in Er166 has been measured in erbium metal between 4.2 °K and 40 °K using the Mössbauer effect. There is evidence for a unique magnetic field and electric fieldgradient at all nuclei in erbium metal. The magnetic field decreases from (7.55±0.20)·106 Oe at 4.2 °K to (6.10±0.40)·106 Oe at 40 °K. Extrapolation to 0 °K yieldsH (0 °K)=(7.60±0.20)·106 Oe. The quadrupole interaction energy for the 80.6-keV state iseQ V Z′Z′/4=(0.95±0.20)·10−6 eV at 4.2 °K. These results are discussed and compared with other measurements.

    Notizen: Abstract: We present the first energy and angle resolved measurements of e+e−-pairs emitted from heavy nuclei (Z≥ 40) at rest by internal pair conversion (IPC) of transitions with energies of less than 2 MeV as well as recent theoretical results using the DWBA method, which takes full account of relativistic effects, magnetic substates and finite size of the nucleus. The 1.76 MeV E0 transition in 90Zr (90Sr source) and the 1.77 MeV M1 transition in 207Pb (207Bi source) have been investigated experimentally using the essentially improved setup at the double-ORANGE β-spectrometer of GSI. The measurements prove the capability of the setup to cleanly identify the IPC pairs in the presence of five orders of magnitude higher β− and γ background from the same source and to yield essentially background-free sum spectra despite the large background. Using the ability of the ORANGE setup to directly determine the opening angle of the e+e−–pairs (Θe+e−), the angular correlation of the emitted pairs was measured within the range covered experimentally (40°≤Θe+e−≤ 180°). In the 90Zr case the correlation could be deduced for a wide range of energy differences E Δ of the pairs (−530 keV ≤E Δ≤ 530 keV). The 90Zr results are in good agreement with recent theory. The angular correlation deduced for the M1 transition in 207Pb is in strong disagreement with theoretical predictions derived within the Born approximation and shows almost isotropic character. This is again in agreement with the new theoretical results.

    Notizen: Abstract The 2S 1/2-2P 1/2 Lamb-shift in hydrogenlike Sulfur was measured with a field quench technique. A high velocity beam of S15+-ions (v/c=0.086) prepared in the metastable 2S 1/2 state crosses a homogeneous magnetic field of about 1.8 Tesla. The strong electric field of 4·107 V/m in the rest frame of the ions mixes the longlived 2S 1/2-state (τ=7.18 nsec) with the short lived 2P 1/2-state (τ=2.4·10−5 ns). From the measured lifetime of the thus quenched 2S 1/2-state a Lamb-shift value of 25.14±0.24 THz is derived and compared with theoretical predictions.
