    Notes: Apparently normal, and lesional skin from patients with atopic eczema were investigated immunohistochemically with anti-HLA-DR, -CD1a and -IgE antisera. A CD1a + intercellular pattern was observed in uninvolved skin in the majority of the patients whereas an HLA-DR +/CD1a+ network, mostly localized in basal and supra-basal areas, was shown in lesional skin of virtually all of them. Moreover, an HLA-DR +/CD1a + IgE + intercellular pattern was observed in some of the patients only and was predominantly localized in those areas characterized by lymphocyte exocytosis, spongiosis or vesicle formation. Whether keratinocytes are able to synthesize CD1a antigen and FcɛR or if these molecules are only produced and shed by CD1a +/IgE + epidermal dendritic cells remains unclear.

    Notes: Thirty-seven men (36 homosexual or bisexual and one heterosexual) with epidemic Kaposi's sarcoma and underlying HIV infection were followed up over a period of up to 32 months. Fourteen patients (38%) died, with a median survival time of 7.2 months after the diagnosis of AIDS. Seventeen patients (46%) presented with one or more opportunistic infections, mostly Pneumocystis carinii pneumonia. Eighteen patients (49%) had lymphadenopathy syndrome according to the definition of the CDC. Using the Laubenstein-classification of Kaposi's sarcoma, all patients either remained stable or deteriorated, improvement was never observed. Absolute T4 lymphocyte counts and the T4/T8 ratio were not related to the disease stage. With the onset of B symptoms (systemic symptoms), however, the absolute T4 numbers and the T4/T8 ratio markedly decreased. Delayed type hypersensitivity also showed no relationship to the clinical stages of Kaposi's sarcoma. Thus, the clinical progression of Kaposi's sarcoma lesions seems to be largely independent of the immunological parameters investigated. However, the onset of B symptoms was observed to be related to changes in immune status.

    Notes: We have quantified Langerhans cells (LC) in cryosections of normal human skin and lesional skin from patients with atopic eczema and psoriasis vulgaris using six different methods. The results from the different methods varied considerably and were sometimes contradictory, for example when LC numbers in psoriatic skin were compared with those in normal skin. Thus, in addition to the staining technique used and the selection of the dendritic cell type to be counted, the enumeration method used can also influence the quantitation of LC in normal and pathological skin.

    Notes: Six patients suffering from anaphylactoid reactions after infusion of pasteurized plasma (PP) or human serum albumin (HSA) were investigated. Clinical symptoms ranged from urticaria and hypotension to cardiac arrest. Immunoglobulin levels, especially of IgA, were normal, as were concentrations of complement factors C3, C4 and factor B. In skin and lymphocyte transformation tests patients, with the exception of one severely allergic to protein, did not react to the monomeric pure HSA. Five out of six patients reacted against HSA aggregates and three patients to the HSA modified by caprylate added as stabilizer during commercial HSA production. It is concluded that the anaphylactoid reactions developing after PP or HSA infusion result from a non-specific reaction to protein aggregates and in some cases possibly from a specific immune response to the caprylate-modified HSA.

    Notes: In fourteen patients with food allergy, intragastral provocation under endoscopical control (IPEC) was performed. In all patients positive immediate-type reactions of the gastric mucosa were observed consisting of oedema, erythema and petechial bleeding. Microscopically, mast cell degranulation was observed and measured by mast cell counts using the o-phthaldialdehyde technique. Concomitantly, tissue histamine content in gastric mucosa decreased significantly after allergen provocation, while there was no change in normal volunteers. Plasma histamine concentration increased in most patients; the increases were most evident in four patients showing mild systemic reactions (urticaria and bronchospasm). The technique described might prove to be useful in establishing the diagnosis in doubtful cases of food allergy.

    Notes: A patient with a 12-year history of a relapsing bullous dermatosis is presented. Unusual clinical features included urticarial erythema, conspicuous mottled hyper- and depigmentation, lichenification and ichthyosiform hyperkeratosis. Serum immunoglobulin E levels were elevated. Histological examination showing deposits of amyloid in the uppermost dermis confirmed the diagnosis of bullous amyloidosis. Ultrastructurally, blister formation occurred at the level of the lamina lucida. The amyloid did not react with a panel of antibodies directed against amyloid fibril proteins. No underlying systemic disease was found. The bullous eruption responded to prednisolone therapy.

    Notes: A new, standardized, ready-to-apply patch test, the TRUE Test™, has been evaluated on 698 consecutive patients with suspected contact dermatitis. The patients were tested with 12 different allergens. Simultaneously, the same 12 allergens in pet. (Trolab®) were applied symmetrically to the opposite side of the upper back using the conventional Finn Chamber® technique. There were positive test reactions to all 12 allergens lusted in the patient group. The concordance of positive reactions between the TRUE Test™ and the Finn Chamber® test was 67%; 13% of all positive reactions were recorded only for the TRUE Test™ and 20% only for the Finn Chamber® method. The frequency of questionable and irritant reactions was of the same low order of magnitude for both tests methods; such reactions were recorded an around 2% of all test patches.

    Description / Table of Contents: Zusammenfassung Anhand der anamnestischen Daten von 50 Patienten mit chronischer posttraumatischer Osteomyelitis wird eine Punktebewertung zur Beurteilung des Schweregrades der Erkrankung entwickelt, die neben der Gesamtdauer der Erkrankung den prozentualen Anteil der stationären Behandlung sowie die Anzahl der chirurgischen Eingriffe berücksichtigt. Zur Objektivierung des klinischen Verlaufes wird eine zweite Skala („München-Murnau-Skala“) vorgestellt, welche die äußeren Wundverhältnisse, die röntgenologischen Befunde sowie die Blutkörperchensenkungsgeschwindigkeit erfaßt. Anhand einer Autovaccinetherapie bei 18 Patienten wird die Objektivierung des Therapieerfolges mit Hilfe dieser Skala gezeigt.
    Notes: Summary From the anamnestic data of 50 patients suffering from chronic posttraumatic osteomyelitis a severity scoring system was developed, based on the duration of the disease, the time of hospitalization and the number of surgical interventions. The clinical course of 18 patients, undergoing an auto-vaccination treatment, was registered by another scoring system (“Munich-Murnau-Scale”) regarding wound morphology, bone radiology and erythrocyte sedimentation rate.

    Description / Table of Contents: Zusammenfassung Die chemische Modifikation von Standardgammaglobulin durch enzymatische Spaltung (Pepsin) oder Stabilisierung (β-Propiolacton) beeinflußt Elimination, Fragmentierung und Organverteilung des intravenös applizierten Gammaglobulins. Untersuchungen mit radioaktiv markierten allogenen und xenogenen Gammaglobulin-Präparaten an 36 Hunden zeigten, daß das pepsingespaltene Gammaglobulin am schnellsten fragmentiert und eliminiert wurde. Dabei fand sich für die Konzentration aller Gammaglobuline in der Haut ein wesentlich langsamerer Abfall als im Blut. Die höchsten Hautkonzentrationen nach 10 Tagen fanden sich fürβ-Propiolacton-Gammaglobulin mit 6,2 ± 1,6 µg/g Haut bei einer Blutkonzentration von 7,9 ± 0,9 µg/ml.
    Notes: Summary Chemical modification of standard gammaglobulin with enzyme treatment (pepsin) or stabilization (ß-propiolactone) is able to influence elimination, fragmentation and organ distribution of intravenously administered gammaglobulins as shown in 36 dogs after i.v. application of allogenic and xenogenic gammaglobulin preparations. Pepsin-gammaglobulin was eliminated and fragmented most rapidly. Gammaglobulin concentrations of all preparations in the skin showed as slower decrease than comparable blood concentrations. The highest skin concentrations 10 days after i.v. application were found for β-propiolactone gammaglobulin with 6.2 ± 1.6 µg/g compared to a blood level of 7.9 ±0.9 µg/ml.
