Notes: Abstract Three patients with maple syrup urine disease were treated during the acute neonatal stage. Multiple exchange transfusions proved to be a satisfactory means of achieving rapid clinical and biochemical improvement during this phase. On the other hand, evidence is provided suggesting that in addition to exchange transfusions, a high calorie intake above 150 Cal/kg body weight/day is necessary to lower the plasma concentration of the branched chain amino acids to near-normal levels. As long as this calorie intake was not provided, further exchange transfusions failed to lower the plasma leucine concentration to below 17 mg/100 ml in one patient. It is assumed that this high calorie intake is necessary to prevent the breakdown of endogenous protein. Treatment of acute episodes in maple syrup urine disease should therefore not only eliminate the elevated alpha-keto acids and branched chain amino acids quickly (i.e. by multiple exchange transfusions or peritoneal dialysis), but in addition should provide a high calorie intake.

Notes: Summary The disposition kinetics of bezafibrate, a newly developed drug of great lipid-lowering potency, were investigated in normal male subjects. Five male volunteers received14C-labelled bezafibrate orally in solution, and a further 10 were given the same dose (300 mg) of un-labelled drug as tablets. The concentration of bezafibrate in serum and urine from the latter was determined by GC, and in the former total radioactivity in serum, urine and feces was followed for 48 h, and urinary excretion products were analysed by TLC and GC-MS. Rapid absorption from the gastrointestinal tract led to peak serum levels 30 min and 2 h after administration of solution and tablets, respectively. Since approximately 95% of the administered14C-bezafibrate was excreted in urine within 48 h, and almost all the remainder was detected in feces, absorption can be regarded as complete after administration in solution. The relative optimal bioavailability from the tablets was also complete, since in both cases approximately 50% of the administered dose was detected as unchanged bezafibrate in urine within 24 h by GC in the tablet study, and by TLC in the solution study. Of the decomposition products, more than 20% of the dose was present as glucuronides and the remainder consisted of several more polar compounds, one of which was identified as a hydroxyderivative of bezafibrate. Since the apparent halflife of bezafibrate in serum was 2.1 h, this new drug possesses favourable pharmacokinetic features: rapid and complete absorption, even from tablets, combined with a conveniently short half-life, and clearance which is half renal (56 ml/min) and half metabolic (43 ml/min), giving a total clearance of 99 ml/min.

Notes: Summary Although multicentric glioblastomas are relatively rare, their occurrence is not an extreme rarity. A multicentric lesion should always be thought of when the symptoms cannot be related to a single focus and when the special investigations do not confirm a single lesion. In our opinion the brain scintigram and the computer tomogram offer the best diagnostic possibilities at present. If the preoperative diagnosis has been made, then an indication for operation can be justified only if multiple tumours can be removed through one approach and, what is even more important, at one sitting. The cases demonstrated prove that this procedure can be successful. If, on the other hand, only one of several tumours is excised, then a fatal outcome is almost certain.

Notes: Summary A glioblastoma was induced in BD-II rats by weeklyi.v. injections of methyl-nitrosourea and transplanted intracerebrally in numerous passages. The take rate was 100%. For the experiments with CCNU, solid pieces of tumour were implanted into the right parietal region. The cytostatically treated animals lived significantly longer. The untreated tumour rats had an average survival time of 18 days (counted from the time of transplantation), and the rats treated with CCNU had an average survival time of 28 days. The median values were 19 and 28 days.

Notes: In a family with perceptive, non-progressive hearing loss several of the members suffered from atopic dermatitis. The proband had a severe atopic dermatitis and an extremely high IgE value. Some of the family members suffered from atopic dermatitis, others from deafness, and some from both diseases. The co-existence of these two disorders has been previously described in two families. Atopic dermatitis and perceptive, non-progressive congenital deafness might be genetically associated.

Notes: Dynamical diffraction of thermal neutrons is treated for a plane single-crystal plate of magnetic neutron-absorbing material. It is shown that the results of the theory of dynamical X-ray diffraction can be used in the case of neutron diffraction, when the Fourier transforms of the polarizability are replaced by the Fourier transforms of the interaction potential divided by the kinetic energy of the neutrons. The Fourier transforms of the magnetic part of the interaction potential are calculated expicitly. In order to take the nuclear part of the interaction into account the method of the Fermi pseudopotential is used. The results are applied to the case of symmetrical Bragg reflection. There are two solutions for the intensity of the reflected beam as a function of the angle of neutron incidence, corresponding to the two possible directions of the neutron spin, parallel or antiparallel to the magnetic field in the crystal. It is shown that the positions of the maxima and the widths of the curves illustrating the intensity dependence on the angle of neutron incidence are different for the two possible spin directions and depend on the direction of the magnetization in the crystal. The total intensity of the reflected beam for the diffraction of unpolarized neutrons is obtained by averaging the intensities for the two possible spin directions.

Description / Table of Contents: Summary 1. Long-term recordings of redox-potential in marine sediments performed under laboratory conditions show that the drift of the measured potential after inserting the electrode cannot be neglected. 2. Cleaning and reinserting an electrode considerably changes the measured potential. This change may reach several hundred mV, depending on the position of the electrode and the composition of the sediment. 3. It takes several days until the potentials observed before cleaning are obtained again. 4. The overall Eh-profile in the sediment column is not changed by successive experiments. An electrode at a certain position in the sediment normally will return to the same potential if enough time is allowed for stabilization. 5. Quick measurements of single Eh-values in systems such as marine sediments must be handled with caution. 6. The measured shape of the redox-profile strongly depends on the time allowed for stabilization. 7. The position of the redox-potential-discontinuity defined as the point of inflection of the curve Eh versus depth seems to be the only meaningful parameter derived from quick measurements.
