Zusammenfassung Die Abstoßung homologer Gewebstransplantate beruht auf immunologischen Abwehrvorgängen, an denen neben humoralen Antikörpern in erster Linie zellbedingte Immunmechanismen beteiligt sind. Sie richten sich gegen Zelloberflächenantigene. Beim Menschen kommen neben den Blutgruppenfaktoren im wesentlichen die sog. HLA-Gruppen sämtlicher Blut- und Gewebszellen in Frage, deren Bestimmung bei Spender und Empfanger vor Organtransplantation in einem Typing-Zentrum erfolgen muß. Auch bei einer vollkommenen Übereinstimmung der nachgewiesenen HLA-Gruppen zwischen Spender und Empfänger müssen immunsuppressive Maßnahmen angewandt werden, um das Einheilen des allogenen Transplantates zu gewährleisten. Hierzu gehören Corticoide, Cystostatica, Antilymphocytengammaglobuline und Röntgenbestrahlung. Neue Konzepte, welche Möglichkeiten für das Einheilen von Organtransplantaten ohne immunsuppressive Dauertherapie eröffnen sollen, werden derzeit im Tierexperiment geprüft; sie laufen im Ergebnis auf eine Immuntoleranz hinaus. Jedoch sind die Ergebnisse derzeit noch nicht auf die Klinik übertragbar.

Summary Homograft rejection is based on an immunological mechanism in which, in addition to humoral antibodies, cell-based immune mechanisms are primarily involved. They are directed against cell-surface antigens. In humans, apart from the blood-group factors (ABO blood-group compatibility), the so-called HL-A systems of all blood and tissue cells are involved and their typing in the donor and in the recipient before organ-transplants has to be undertaken by a “typing centre”. But even when complete matching of identified HL-A systems in donor and recipient has been achieved, immuno-suppressive measures have to be carried out to ensure acceptance of the allogenous transplant. These consist of cortico-steroids, cytostatic drugs, anti-lymphocytic gamma-globulins and X-rays. New concepts which may lead to the acceptance of organ transplants without immuno-suppressive therapy of indefinite duration are at present being tested in animal trials. The results point towards an immune tolerance (adaptation). However, the results of these studies cannot yet be applied in clinical work.
