    Notizen: Summary The safety of drugs in hepatic porphyrias has largely been established by clinical experience, which is very limited in the case of antineoplastic agents. We administered three cycles of polychemotherapy consisting of daunorubicin, cytarabine and 6-thioguanine, and modified supportive care to a 33-year-old Turkish woman suffering from acute myelogenous leukemia. The urinary excretion of total porphyrins, porphobilinogen, and aminolevulinic acid was continuously monitored. Excreation of these metabolites was permanently elevated, but the values were comparatively low during cytotoxic therapy while peak values were recorded at the onset of fever during bone marrow aplasia; yet there were no clinical signs of porphyritic attacks at that time. A few potentially unsafe drugs were tolerated without an increase in porphyrin excretion. Although the susceptibility to drugs is highly variable in patients with hepatic porphyrias, the treatment of malignancy in these patients seems justified as long as porphyrin excretion under therapy is not grossly elevated over baseline values and appropriately modified supportive care is administered.

    Notizen: Summary Retrograde persufflation of ischemic kidneys of dogs via the renal vein with gaseous O2 was evaluated as a method of kidney preservation (retrograde aerobic ischemia) and compared to conventional ischemic storage without additional measures (anaerobic ischemia). Renal preservationin situ lasted 4 hrs at 6°C. A contralateral nephrectomy was performed before the preservation period. For retrograde aerobic ischemia the persufflation pressure was 30 mm Hg, the gas flowed off via opened capsular vessels. While dogs which had undergone renal anaerobic ischemia showed a marked temporary post-ischemic increase in renal blood flow and in the systemic blood pressure, dogs submitted to renal retrograde aerobic ischemia did not exhibit a storage induced loss of the renal vascular autoregulation. During and after retrograde aerobic ischemia the renal tissue levels of ATP, ADP, and AMP remained within normal ranges; ATP and the sum of adenine nucleotides decreased markedly during anaerobic ischemia. In both the groups a normal renal function was resumed again; but the recovery time of various parameters of renal function (clearance from inulin and PAH, urea excretion, serum concentrations of urea and creatinine) was shorter after retrograde aerobic ischemia than after anaerobic ischemia. Kidneys subjected to retrograde aerobic ischemia exhibited a better functional adaption to the contralateral nephrectomy during the 3 weeks of post-ischemic observation than kidneys stored in anaerobic ischemia.

    Notizen: Summary In search for new techniques of renal preservation, kidneys were persufflatedin situ with gaseous O2 at a pressure of 90–100 mm Hg via their vascular system (orthograde aerobic ischemia). The results on post-ischemic function and blood flow, and on the changes in the renal tissue levels of the adenine nucleotides were compared to those obtained from kidneys ischemically stored without additional measures (anaerobic ischemia). The kidneys were stored for 4 hrs at 6°C. A contralateral nephrectomy was performed before the preservation period. All dogs subjected to renal anaerobic ischemia survived. After orthograde aerobic ischemia, 3 out of 4 dogs died from uremia within 8 days, 1 animal survived. Although the aerobic ischemic preservation resulted in the maintenance of a metabolic status characteristic for aerobiosis, post-ischemic renal function recovered too slowly and insufficiently on the average. Renal blood flow was within normal ranges after anaerobic ischemia, it was reduced to about one third of this after orthograde aerobic ischemia.

    Notizen: Summary In three consecutive pilot studies the effect of recombinant human granulocyte/macrophage-colony-stimulating factor (rhGM-CSF) on haematopoetic recovery after chemotherapy in patients with small-cell lung cancer was investigated. In study I, 20 patients received AIO chemotherapy (A, Adriamycin 25 mg/m2 on days 1+2; I, ifosfamide 2 g/m2 on days 1–5; O, vincristine 2 mg on day 1) at 4-week intervals either with or without rhGM-CSF (250 μg/m2 sc) from day 8 until recovery of leucocytes. Neither the degree nor the duration of myelosuppression was markedly influenced by rhGM-CSF. Suggesting that these disappointing results were caused by the late onset of GM-CSF application, in the following study we shortened chemotherapy to 3 days and started with GM-CSF on day 4. The main objective of this study was to test whether the earlier administration of GM-CSF allowed treatment intervals to be reduced or the dose to be escalated. After 10 patients had received a starting dose of AIO (A, 50 mg/m2 on day 1; I, 2 g/m2 on days 1–3; 0,2 mg on day 1) alternating with cisplatin (90 mg/m2 on day 1) and etoposide (150 mg/m2 on days 1–3), the dose of ifosfamide and etoposide was escalated to 2.5 g/m2 on days 1–3 and 200 mg/m2 on days 1–3 in the next 10 patients. Treatment was given at 2-week intervals when leucocytes were 〉 3500/mm3 and thrombocytes were 〉 100 000 mm3 on day 14. At each dose level patients were randomized to receive either rhGM-CSF 250 μg/m2 s.c. on days 4–12 or no GM-CSF. In this study, rhGM-CSF markedly shortened the duration of leukopenia. Reinstitution of chemotherapy on day 15 was possible at dose level 1 in 1/4 patients without and in 3/4 patients with GM-CSF, and at dose level 2 in 0/5 patients without and in 5/5 patients with GM-CSF. However, the degree of myelosuppression was not improved by GMCSF. In a third study we tried to apply rhGM-CSF simultaneously with chemotherapy. After 3 patients had received GM-CSF starting on day 1 concurrent to AIO chemotherapy, we noticed an increase of myelosuppression with prolonged neutropenia and thrombocytopenia and stopped this investigation. Considering all patients included in these three consecutive pilot studies, there is no difference in response rates and survival between patients with and without rhGM-CSF treatment. Optimal scheduled rhGM-CSF application shortens the period of myelosuppression and allows reinstitution of chemotherapy at 2-week intervals. Whether these modifications are able to improve the overall treatment results, has to be determined in future clinical trials.

    Beschreibung / Inhaltsverzeichnis: The precipitation of diphosphate from solutions of secondary alkali diphosphates by means of Ba2+ yields the acid salt Ba7H2P8O28·6H2O. With increasing alkalinity of the solutions, the hydrogen ion content of the Ba salt precipitate decreases. Precipitation from Na4-diphosphate solutions yields the neutral Ba2P2O7·1,5 H2O. From X-ray investigations (no structural differences between the two precipitates being found) it in concluded that the “neutral” Ba diphosphate has a phase range from Ba:H = 2:0 to 7:2.
    Notizen: Ba2+ bildet-im Unterschied zu vielen anderen zweiwertigen Metallen-bei der Fällung aus Lösungen sekundärer Diphosphate ein saures Diphosphat, Ba7H2P8O28·6H2O. Mit steigender Alkalität der Fällungslösung nimmt die Acidität der Ba-Verbindung ab. Das neutrale Na-Diphosphat entsteht nur bei der Fällung aus quaternären Diphosphat-Lösungen. Bemerkenswerterweise besteht kein struktureller Unterschied zwischen den sauren Verbindungen und dem quaternären Salz, was zu der Annahme führt, daß das quaternäre Ba-Diphosphat eine Phasenbreite zwischen Ba:H = 2:0 und 7:2 besitzt.
