    Notes: Carcinogenesis in the human colon is associated with a marked increase in the tissue content of the urokinase-type plasmmogen activator (u-PA). This study was performed to determine the type of cells responsible for the u-PA increase in carcinomas of the colon and in their precursor lesions, the adenomas, by immunohistological evaluation applying monoclonal antibody 3689 directed to the β-chain of u-PA.Normal intestinal mucosa (n= 17) showed hardly any staining of u-PA, but some lamina propria cells were faintly positive. Carcinomas (n= 17) and adenomas (n= 16) showed a considerable and comparable staining intensity of u-PA in neoplastic columnar epithelial cells, and this staining was found to be diffuse and cytoplasmic. In a majority of the neoplastic tissues the u-PA staining was found to be patchy and not related to known risk markers of malignancy such as dysplasia in the adenomas, or to prognostic determinants such as Dukes' classification or differentiation in the carcinomas. The observation of strong u-PA positive lamina propria cells in adenomas but infrequently observed in normal mucosa and carcinomas was noteworthy. u-PA staining intensity of the tissue sections was found to correlate well with the u-PA antigen level in the tissue extracts determined by ELISA (r= 0.52, P= 0.0001) but poorly with the u-PA activity determined enzymatically (r= 0.28, P= 0.05).In conclusion, the u-PA increase in neoplasia of the human colon can be attributed to an increased diffuse cytoplasmic content of u-PA in neoplastic columnar epithelial cells.

    Notes: Aims: Ductal carcinoma in situ (DCIS) is a pre-invasive form of mammary carcinoma with no microscopic evidence of cancer cell invasion through the basement membrane. However, for initiation of invasion, tumour cells have to acquire and focus proteolytic activity on to the cell surface in order to infiltrate the surrounding extracellular matrix. The receptor (uPA-R or CD87) for the serine protease urokinase-type plasminogen activator (uPA) plays a central role in invasion and metastasis. This study was performed to determine and localize m-RNA and protein of uPA-R in ductal carcinoma in situ of the breast.〈section xml:id="abs1-1"〉〈title type="main"〉Methods and resultsWe analysed uPA-R mRNA and protein expression by in-situ hybridization and immunohistochemistry, respectively, in 50 formalin-fixed, paraffin-embedded specimens of DCIS. Three different antibodies were used to stain cell-associated uPA-R; chicken polyclonal antibody (pAb) HU277 and monoclonal antibodies (mAb) IID7 and 3936. In all cases, myoepithelial and stromal cells reacted with either antibody. Especially, reaction of macrophage-like cells with mAb 3936 resulted in a well-marked and bright staining. Applying mAb IID7, in 46 of the 50 breast specimens tumour cells showed a positive immunoreaction. Likewise pAb HU277 stained tumour cells in 40 of the 50 cases, whereas mAb 3936 reacted with only 24 of the 50 tissue sections. Endothelial cells were marked by both mAb IID7 and pAb HU277 (46/50 and 35/50, respectively); mAb 3936 did not label at all. All of the cell types stained by mAb IID7 and pAb HU277 also displayed reactivity with uPA-R mRNA-specific antisense oligonucleotides in in-situ hybridization.〈section xml:id="abs1-2"〉〈title type="main"〉ConclusionsOur results reveal the presence of the tumour invasion-related receptor for the protease uPA not only in invasive ductal breast carcinoma but also in different types of DCIS.

    Notes: Abstract. Stromelysin (ST)-3 is considered to be a marker of invasion and preinvasive lesions to indicate the likelihood of subsequent invasion. The expression of ST-3 has not been systematically studied in ovarian neoplasms. We studied 47 ovarian carcinomas and 49 ovarian tumours of low malignant potential (LMP) to see whether the expression of ST-3 correlated with any histopathologic features and, in the LMP tumours, whether its expression might be a prognostic indicator. All of the primary tumours and available metastases or implants were studied using immunohistochemistry (IHC) for ST-3 and, in 52 selected lesions, in situ hybridisation (ISH) using a cDNA probe. Expression of ST-3 was seen in 42 of 47 (89%) of the carcinomas and in 16 of 49 (33%) of the LMP cases. A significantly higher percentage of carcinomas than LMP tumours (P〈0.00001) expressed ST-3 in the stroma adjacent to the tumour, with a correlation to increasing FIGO (International Federation of Gynecology and Obstetrics) tumour stage. ST-3 was expressed in the surrounding stroma of 1 of 8 LMP implants and 46 of 55 (84%) of the carcinoma metastases. The single LMP patient who died of tumour recurrence had ST-3 expression, but a significant prognostic impact was not found. The infrequent expression of ST-3 in LMP compared with carcinoma metastases appears to be more consistent with a peritoneal "field effect" than spread from the ovarian LMP tumour.

    Notes: Summary Increased sialic acid levels reflecting tumor burden are found on the surface of T-lymphocytes and in the plasma of patients with carcinoma of the mammary gland. The data of the determinations of sialic acid content and distribution on T-cells, using microanalytical methods such as HPLC and a colorimetric test, show that the total sialic acid content is increased by about 60% and that nearly 80–90% of the sialic acids consist of Nacetyl-9-O-acetyl-neuraminic acid, in comparison to the healthy controls (not containing O-acetylated neuraminic acid). Investigations on lymphocytes of malignant melanoma patients show similar changes of sialic acid content and distribution on the cell surface. Increased sialic acid levels are also found in the plasma of patients with cancer but no O-acetylated derivative can be found. Furthermore the examinations show that the separation of the T-lymphocytes from the total lymphocyte fraction is not required. Determination of sialic acids in the total lymphocyte fraction can be a simplification in carrying out further diagnostic investigations. A high level of sialic acids as “antirecognition factor” seems to be not only a marker of tumor cells but also an attribute of T-lymphocytes, involved in the defence against the malignoma (malignant melanoma, breast cancer). Considering the possible contribution of sialic acid to the immunoregulatory protective mechanism during the first stage of pregnancy, sialic acid content and distribution on T-cells of pregnant women are investigated. Both an increase and a change in the distribution of sialic acids can be excluded.

    Description / Table of Contents: Zusammenfassung Glomeruläre Kbrinniederschläge, das Hauptmerkmal der generalisierten Shwartzman-Reaktion, kann bei Kaninchen und Ratten durch eine kontinuierliche intravenöse Infusion von Endotoxinen ausgelöst werden. Im Vergleich zu virginellen Kaninchen führte die Infusion von Endotoxin beim trächtigen Tier in einer kürzeren Zeit und quantitativ vermehrt zu Fibrinniederschlägen in der Niere. Dies weist erneut auf die stärkere Empfindlichkeit von trächtigen Tieren gegenüber Gerinnungsvorgängen hin. Die Infusion von Endotoxoid mit geringer pyrogener Aktivität resultierte nicht in glomerulären Fibrinniederschlägen, obwohl ein Abfall der Thrombozyten entstand. Die Ergebnisse deuten darauf hin, daß Endotoxoid eine ähnliche cytotoxische Aktivität auf Plättchen wie das Endotoxin ausübt, jedoch der Teil des Moleküls, der glomeruläre Fibrinniederschläge hervorruft, durch die biochemische Alteration verloren ging.
    Notes: Summary Glomerular fibrin deposition, the hallmark of the Generalized Shwartzman Reaction, can be produced by a continuous infusion of a sublethal dose of endotoxin in rabbits and rats. The infusion of the active endotoxin into pregnant rabbits produced disseminated intravascular coagulation in a shorter time and quantitatively more pronounced than in the non-pregnant animal. Although the higher susceptibility of pregnant animals to endotoxin is well established, the infusion of an endotoxoid with little pyrogenic activity did not result in glomerular fibrin deposition, but a decrease in platelet number was noted. These data indicate that the endotoxoid has the cytotoxic activity of the original endotoxin molecule but the part of the molecule which induces fibrin deposition in the microcirculation is missing after the biochemical alteration.

    Description / Table of Contents: Zusammenfassung Bei 77 Frauen (Ø 41 SSW, 〈7 Tage vor Geburt), wurde das Meßergebnis des Resistance Index (RI) in der Umbilikalart. (UA), desz. Aorta (DA), und einem intracran. Gefäß (ICA) mit dem Oxytozinbelastungstest (OBT) und Non-Streß-Test (NST) verglichen. Prüfkriterien waren operative Entbindung aus Asphyxiegründen bzw. met. Azidose. Die Sensitivität in der Erkennung der met. Azidose war in allen Testen niedrig, dagegen erzielten sowohl normale RI-Werte als auch ein unauffälliger OBT eine hohe Spezifität. Bei der Vorhersage der fetalen Asphyxie hatte der RI in der ICA eine hohe Sensitivität, in der UA and DA die höchste Spezifität aller Teste. Ein brain sparing Effekt scheint ein frühes Zeichen der Plazentarinsuffizienz zu sein.
    Notes: Summary In 77 women (Ø 41 week, 〈7 days before delivery) we compared the resistance index (RI) in the umbilical artery (UA), descending aorta (DA) and intracranial artery (ICA) with an oxytocin challenge test (OCT) and a non stress test (NST). Proof criterias were asphyxia resulting in operative deliveries and metabolic acidosis. The sensitivity in predicting metabolic acidosis was low in all tests, but a high and comparable specificity could be achieved in both the RI and OCT. In predicting fetal asphyxia the RI in the ICA had a high sensitivity, the RI in the UA and DA had the highest specificity of all tests. Centralisation and increased brain perfusion seem to detect fetal asphyxia as a result of placental insufficiency.
