    Description / Table of Contents: Résumé Du 1er janvier 1978 au 15 mai 1982 nous avons été amenés à traiter 41 polytraumatisés présentant des lésions vasculaires associées. Ce groupe représente 10% des 407 polytraumatisés qui ont abouti à notre service. La fréquence des lésions vasculaires de l'épaule, du cou, des membres supérieurs s'est élevée à 2,7% (n=11), celle des meilleures inférieures à 9,7% (n=19) de la cavité abdominale à 1% (n=4) et de l'aorte thoracique qui était rompue à 2,7% (n=11). Sept blessés (17%) sont morts: 4 de lésions cranio-cérébrales, 3 d'hémorragie par rupture aortique. Six fois fut pratiquée une amputation majeure (2 immédiates, 4 secondaires) et 2 fois une amputation limitée. Chez les autres 26 blessés un bon résultat fut obtenu après traitement de la lésion vasculaire. L'exploration vasculaire à l'aide de la tomodensitométrie et de l'angiographie est d'une importance extrême. Le traitement des lésions vasculaires doit être entrepris sans retard l'hémorragie et/ou l'ischemie menaçant les membres du blessé, les organes et même sa vie. Ceci est spécialement vrai quand l'aorte thoracique est rompue. La fréquence des blessures vasculaires périphériques chez le polytraumatisé est 2 fois plus importante (7,3%) que chez le blessé présentant une fracture isolée des membres (3,6%).
    Notes: Abstract In our surgical clinic, from January 1, 1978, to May 15, 1982, a total of 41 patients with polytrauma exhibited 45 concomitant vascular injuries requiring operation. This group of patients represents 10% of the 407 multiple trauma patients treated during this period. The incidence of vascular lesions of shoulder, neck, and upper extremities was 2.7% (n=11); lower extremities, 4.7% (n=19); within the abdominal cavity, 1% (n=4); and of acute thoracic aortic rupture, 2.7% (n=11). Seven patients (17%) died: 4 from additional brain trauma, 3 from hemorrhage by aortic ruptures. There were 6 major amputations (2 primary, 4 secondary) and 2 borderline amputations. In the remaining 26 patients, a good result following vascular repair was achieved. Angiologic examination, including computed tomography and angiography, is of great importance. Vascular repair deserves high priority since bleeding and/or ischemia threatens the patient's limbs, organs, or even life. This is especially true for the acute thoracic aortic rupture as a typical vascular injury in polytrauma. The incidence of peripheral vascular injuries in polytrauma is twice as high (7.3%) as in isolated extremity fractures (3.6%).

    Description / Table of Contents: Zusammenfassung Während der Intensivbehandlung von ca. 200 Polytraumen traten acht Fälle von akuter Cholecystitis auf. Die recht uniformen Verläufe werden geschildert. Zwei Patienten versterben an den Folgen der galligen Peritonitis. Das Leitsymptom ist ein steinloser Gallenblasenhydrops mit eitriger Cholecystitis nach einem freien Intervall von ca. 3 Wochen. Es liegen Hinweise vor, daß die Ursache für den mangelnden Gallefluß in einer hohen Morphinmedikation während der Beatmung liegt.
    Notes: Summary During the treatment of 200 polytraumatized patients, 8 cases of acute cholecystitis occurred in the intensive care unit. The case histories are related, all of which are somewhat uniform. Two patients died of peritonitis. The leading symptom is stoneless hydrops of the gallbladder with purulent cholecystitis after a free interval of about 3 weeks. There is some evidence that the cause of blocked bile flow may be the large amount of morphine medication administered during respirator therapy.

    Description / Table of Contents: Zusammenfassung Die Computertomographie liefert als einzige Untersuchungsmethode in der Radiologie horizontale Schnitte. Auch Weichteile, Tumoren, Bänder und Hämatome können beurteilt werden. Anderweitig schwierig zu untersuchende Bezirke wie Acetabulum, Kreuzbein und Iliosacralfuge kommen gut zur Darstellung. Die Untersuchung ist für den Patienten nicht belastend. Die Ausstellung zeigt Beispiele für Frakturen am Becken, die z.T. nur durch die CT erkannt wurden. Ebenso werden Beispiele für Tumoren, Hüftkopfnekrosen und Beckenosteomyelitis demonstriert.
    Notes: Summary Computed tomography is the only method of radiological examination to show horizontal sections of the patient. Soft tissues, tumors, ligaments and hematomas can be evaluated. Regions, such as the acetabulum, os sacrum and iliosacral joint, that are difficult to examine by other means can be displayed. The examination is of little harm to the patient. The exhibition shows examples of fractures of the pelvis, part of which could only be recognized by CT. There are also examples of tumors, femoral-head necrosis and osteomyelitis.

    Description / Table of Contents: Zusammenfassung Wegen der komplexen anatomischen Verhältnisse im Beckenbereich ist die exakte Beurteilung von Frakturen mittels konventioneller röntgenologischer Methoden oft schwierig. Durch die computertomographische axiale Ebene ergibt sich ein wesentlich anschaulicheres und genaueres Bild der Ausdehnung und des Verlaufs der Fraktur, der Gelenkverhältnisse und der Anzahl, Lage und Größe der Fragmente. Stufenbildungen und Interpositionen kleiner knöcherner Fragmente der Hüftpfanne sind sicher nachzuweisen. Weiterhin werden die biomechanisch wichtigen Verletzungen des hinteren Beckenrings sicher erfaßt.
    Notes: Summary Pelvic fractures are often difficult to evaluate by conventional tomography due to the complex bony structures of the pelvis. Axial CT scans clearly show the extent of the fracture, location and size of fragments. Abnormalities of the hip joint space, like loose bone fragments are easy to demonstrate. Furthermore the biomechanically important injuries of the sacroiliac joint and sacrum are more often detected by computed tomography.

    Notes: Conclusion Estimation of aprotinin in plasma with the described amidolytic assay can be achieved within 70–80min and is, therefore, convenient as a bedside-control of high dose proteinase inhibitor therapy. Moreover, an ELISA for aprotinin developed simultaneously (Müller-Esterl et al. this issue) allows the detection of up to 0.2KIU/ml within 4h and is suitable, therefore, for special purposes, e.g. retrospective studies and low dose inhibitor therapy.

    Description / Table of Contents: Zusammenfassung Eine Analyse von 375 Polytraumatisierten aus 4 Jahren ergab in 8 % ein isoliertes, in 6,7 ein multiples Organversagen. Das IOV betraf überwiegend die Lunge (27/30), beim MOV war grundsätzlich die Lunge der Ausgangsort. Die Letalität stieg mit der Zahl der befallenen Organe: Lunge allein: 37 %, Lunge und Leber 70 %. Ein Vierfach-Organversagen überlebten 2 von 6. Schock und Sepsis waren die prädisponierenden Faktoren. Bei ihrer Verhütung sowie ausreichender frühzeitiger Beatmung muß auch die Prophylaxe ansetzen.
    Notes: Summary The analysis of 375 polytraumatized patients during 4-year period showed in 8 % a single (SOF) and in 6.7 % a consecutive multiple organ failure (MOF). The SOF concerned the lung in 27 of 30 cases; in MOF the lung was the first organ concerned in all cases, followed by the liver. Mortality rose with the number of organs involved: lung alone 37 %, lung and liver 70 %. A quadruple failure was only survived by 2 of 6. Traumatic shock and sepsis were predisposing factors; their prevention was the main prophylaxis of MOF, together with sufficient and early respirator treatment.
