    Notes: Silicon-containing amorphous hydrogenated carbon films deposited by a plasma-enhanced chemical vapor deposition process were studied using both Raman and ellipsometry spectroscopies. Analyses of the experimental data from both these techniques yielded valuable information about the microstructure of the films. The silicon incorporation in amorphous hydrogenated carbon breaks down large size sp2 carbon clusters and enhances sp3 bonding. The reduction of large sp2 graphitic defects, the enhancement of sp3 bonding, and the associated microstructure changes are responsible for the desired properties of silicon-containing amorphous hydrogenated carbon. © 1998 American Institute of Physics.

    Notes: The Raman signature of the local Ge–C mode for substitutional C is identified as a narrow line (8 cm−1 full width at half maximum) near 530 cm−1 in alloy films of Ge1−yCy (0≤y≤0.07) grown on Ge (100) substrates by low-temperature (200 °C) molecular beam epitaxy. The intensity of the Ge–C line relative to the c-Ge line suggests that only a small fraction of the nominal C is in substitutional sites. In ternary alloys of Ge1−x−ySixCy with x=0.1 and 0.2 and y=0.03, the Ge–C mode disappears, suggesting a strong bias towards C bonding with Si as opposed to Ge. In Ge1−xSnx films the Ge–Sn mode is seen at 263 cm−1. © 1998 American Institute of Physics.

    Notes: Crystallization of the hydrophilic ectodomain of the epidermal growth factor (EGF) receptor has been accomplished in the presence of the ligand EGF. Two different crystal forms have been obtained, one of which was suitable for X-ray analysis. The space group of this form has been assigned to P21212 with unit-cell dimensions of a = 207.4, b = 113.3 and c = 120.4 Å. A native data set has been recorded and a heavy-atom search is currently under way. Diffraction from these crystals, however, is limited to low resolution and extensive trials to improve crystal quality further have all failed. To analyse the molecular shape and aggregation of the receptor protein in solution, small-angle X-ray diffraction and dynamic light-scattering techniques have been applied. Synchrotron radiation in combination with cryo-techniques is essential for data collection because of the high solvent content and radiation sensitivity.

    Notes: Abstract Purpose: A wide variety of fluorouracil (FU)-plus-leucovorin (LV) doseschedules are in clinical use for the treatment of advanced colorectalcancer. Only the monthly low-dose LV- plus-FU regimen, as used by the NorthCentral Cancer Treatment Group, has demonstrated a lasting survival benefitas opposed to FU alone (J Clin Oncol 1989; 7: 1407–1417). The SwissCancer Group adopted this regimen for a confirmatory phase III trial butused the same dose-intensity of fluorouracil in both treatment arms. Patients and methods: Patients with inoperable or metastatic colorectalcancer were randomized to receive monthly FU 400 mg/m2/dayplus LV 20 mg/m2/day as intravenous push daily for five days,or FU alone. Results: Three hundred nine of the 310 patients randomized were eligibleand included in the analysis. The objective response rate for patients withmeasurable disease was 9% with FU alone and 22% withFU-plus-LV (P = 0.0001). The median progression-free survival was 3.9 versus6.2 months (P = 0.003) and the overall survival 10 versus 12.4 months (P =0.02). The major prognostic factors for survival were performance status,weight loss, and disease symptoms. WHO 〉2 toxicity, consisting ofstomatitis (P = 0.001), diarrhea (P = 0.001), and nausea (P = 0.001), wasmore pronounced for FU-plus-LV, without fatal events. Conclusions: This is the largest published randomized trial to compareFU-plus-LV to FU alone in advanced colorectal cancer. It confirms the survivalbenefit obtained from biomodulating monthly FU with low-dose LV. The toxiceffects of FU-plus-LV were acceptable to most patients, and they respondedwell to FU dose reductions. In the absence of an ideal dose-intense FUmonotherapy regimen, monthly FU with low-dose LV provides a simple andeconomical means by which to achieve adequate FU efficacy in the treatment ofadvanced colorectal cancer.

    Description / Table of Contents: Zusammenfassung. Die Kostenexpansion im Gesundheitswesen ist bei der strukturellen Konvergenz aller entwickelten nationalen Systeme eine globale Herausforderung. In den USA entwickeln sich unter dem Druck der Kostenträger dynamische Strukturreformen, welche als „managed care“ zusammengefaßt werden. Sie charakterisieren den Umbau der medizinischen Versorgung zu einem primär ökonomisch orientierten Modell, in dem der Umgang mit medizinischen Leistungen den Charakter eines normalen Warengeschäftes angenommen hat. In „Managed-care-Modellen“ werden Patienten gezielt über Zuweisungs- und Behandlungsrichtlinien der jeweils kostengünstigsten Behandlungsform zugeführt. Der Behandlungsablauf wird durch vertragliche Vorgaben zum Leistungsspektrum, eine Standardisierung der Prozesse und Regeln zur Ablauforganisation beeinflußt. Finanzierungs- und Leistungsverantwortung liegen betriebswirtschaftlich in einer Hand. Die Autonomie der ärztlichen Entscheidung wird abgebaut. Führungsansprüche werden auf Kosten- oder Versicherungsträger verlagert. Diese Strukturveränderungen induzieren deutliche Einsparungen der regionalen US-Gesundheitsausgaben. Dennoch stellen die US-Verhältnisse im Vergleich zum deutschen Gesundheitswesen weiterhin ein erheblich teureres Versorgungsmodell dar. Historische Entwicklungen, aktuelle Konzepte und ihre potentielle Übertragbarkeit auf die Situation in der Bundesrepublik werden in einer Übersicht dargestellt.
    Notes: Summary. While the different national health systems merge structurally, cost expansion in health care is a global challenge. Structural reforms have been developed during recent years in the USA which can be summarized as “managed care”. They are characterized by the evolution of an economically orientated system, in which units of medical therapy are generally handled like conventional economical goods. In managed-care models, patients are deliberately directed to the most economic forms of therapy. The spectrum of medical interventions as well as diagnostic or therapeutic patterns are predefined by a system of contracted guidelines, which lead to a standardization of processes. Financing and medical executive responsibilities fuse. The autonomy of medical decisions is clearly reduced to enforce an integrated and economically oriented steering of the health system. Leadership is no longer primarily confined to doctors or scientists. It is progressively shifting to financing institutions, managing directors or insurance companies. Structural changes currently are expanding rapidly in the U. S. and have meanwhile led to marked regional reductions of medical costs. Nevertheless, the US model is still far more expensive compared to the German system. Historical development, current concepts of US-managed care, its potential influence and general applicability to the German situation are discussed in an overview.

    Notes: Abstract Lake Winnipeg, the seventh largest lake in North America, is located at the boundary between the Interior Plains and the Canadian Shield in Manitoba, Canada. Seismic profiles were obtained in Lake Winnipeg on two geoscientific cruises in 1994 and 1996. These data indicate the morphology of the bedrock surface. In most cases, a clear distinction between low relief Paleozoic carbonate rock and high relief Precambrian rock can be made. In northern Lake Winnipeg, the eastern limit of Paleozoic rock is clearly demarcated 30 km west of the previous estimate of its position. In southern Lake Winnipeg, all or most of the Paleozoic sequence terminates at a prominent buried escarpment in the centre of the lake. This indicates that Paleozoic rock on the eastern shore, known from drilling and outcrops, is an outlier. Major moraines are apparent as abrupt, large ridges having a chaotic internal reflection pattern. These include the Pearson Reef Moraine, the George Island Moraine and the offshore extension of The Pas Moraine. Little evidence for extensive or thick till was observed. Instead, fine-grained sediments deposited in glacial Lake Agassiz rest directly on bedrock over most of the lake basin. Hence an episode of erosion to bedrock was associated with glaciation and/or deglaciation. The Agassiz Sequence sediments are well-stratified, drape underlying relief and in some areas are over 100 m thick. In places, stratification in these sediments is disrupted, perhaps by dewatering. Evidence of erosion of Agassiz Sequence sediments by recent currents was observed. The contact between the Agassiz Sequence and the overlying Winnipeg Sequence sediments is a marked angular unconformity. The Agassiz Unconformity indicates up to 10 m of erosion in places. The low-relief character of this unconformity precludes subaerial erosion and the lack of till, moraines, or extensive deformation precludes glacial erosion. Waves appear to be the most likely erosional agent, either in waning Lake Agassiz or early Lake Winnipeg time. Winnipeg Sequence sediments, in places very thin, mantle most of the lakefloor. These sediments were deposited in the present Lake Winnipeg and are faintly stratified to massive and reach about 10 m in thickness in deep water. On the surface of the Winnipeg Sequence, vigorous, episodic currents are thought to contribute to the construction of flow-transverse sand waves as much as 6 m high in a deep, narrow constriction in the lake.

    Notes: Abstract  Soybeans are known to be allergenic for adults as well as for infants. Processed products derived from soybeans are used in a wide spectrum of foods, drugs and other industrial products. In particular, soybean lecithins are used as stabilizers and emulsifiers and may not be suspected as possible source of allergens. To test this hypothesis, six commercial soy lecithins were investigated for residual allergenicity and compared with extracts from raw and heat-treated soybeans. They were characterized, the protein content was determined by enzyme-linked immunosorbent assay (ELISA) and allergens were analyzed with specific IgE from patients' sera using the enzyme allergosorbent test (EAST), EAST inhibition and protein blotting followed by immunodetection. For further characterization a polyclonal antiserum directed against soybean extract and a monoclonal antibody (mAb 025) directed against the acidic subunit of the soybean storage protein glycinin were used. The EAST studies revealed that three of six sera from patients with allergy to soybeans contained IgE to four soy lecithins (Topcithin 50, Topcithin 300, Emulfluid FD 12, Epikuron 100 P), the same lecithins which were found to contain residual proteins. Two lecithins with a protein content of less than 20 ppb did not bind IgE. EAST inhibition showed that the allergens from soy lecithin were immunologically more closely related to allergens from heat-treated soybeans than to those from raw soybeans. Protein blotting and immunodetection of the protein extract from the lecithins resulted in various allergen bands between 14 kDa and 94 kDa. A heat-stable allergen of 39 kDa was recognized by the monoclonal antibody and thus identified as a subunit of glycinin. The results obtained were confirmed by a mediator release assay based on a rat basophil leukemia cell line. Lecithins that contained residual proteins caused a specific mediator release, suggesting that these products may induce allergic symptoms. Our results show that soybean lecithins are capable of introducing hidden allergens to processed foods and that the IgE binding potential corresponds to the total protein determined by ELISA. Furthermore, it appears to be possible that by monitoring the protein content soy lecithins can be applied which may be safe for the allergic consumer.

    Notes: Abstract. This is a report of clinical, CT and MRI findings in a patient with toxic spongiform leucoencephalopathy after heroin ingestion. The disease is observed in drug addicts who inhale pre-heated heroin. The clinical onset, which usually occurs some days or even longer after the last heroin consumption, is characterized by a cerebellar syndrome. The cerebellar hemispheres, the cerebellar and cerebral peduncles and the pyramidal tract may be affected. Spongiform demyelination is the morphological substrate of the lesions, which are not contrast enhancing, hypodense on CT and hyperintense on T2-weighted MRI. The frequently perfect symmetry of the affection of functional systems points to a toxic and/or metabolic pathophysiological mechanism.

    Description / Table of Contents: Zusammenfassung Für Primärdiagnostik und Staging des Bronchialkarzinoms stellt die Computertomographie (Thorax mit Oberbauch, Schädel) neben den konventionellen Röntgenaufnahmen in 2 Ebenen die Standarduntersuchung dar und bildet mit Oberbauchsonographie und Skelettszintigraphie das diagnostische Basisprogramm. Die MRT sollte gezielt für spezielle Fragestellungen zum Einsatz kommen und bei Patienten mit Kontrastmittelallergie. Der klinische Stellenwert der Positronen-Emissions-Tomographie (PET) v. a. beim Lymphknotenstaging wird derzeit in Studien evaluiert. In Folge der hohen Sensitivität der genannten Untersuchungsmethoden bei relativ geringer Spezifität, besteht grundsätzlich eine Tendenz zum „over-staging“, die v. a. für potentiell operable Patienten Risiken in sich birgt. Inoperabilität signalisierende Kriterien (T3, T4, N2, N3, in Einzelfällen auch M1) müssen daher vor folgenschweren therapeutischen Entscheidungen durch gezielte bioptische Verfahren, ggf. durch explorative Thorakotomie histologisch verifiziert werden.
    Notes: Summary In addition to conventional chest X-rays in AP and lateral projection, computed tomography of the chest, upper abdomen, and head, percutaneous ultrasonography of the abdomen, and bone scintigraphy represent the standard procedures for the primary diagnosis and staging of bronchial carcinoma. Magnetic resonance imaging should be reserved for special situations and patients with allergy to IV contrast medium. The clinical value of positron emission tomography (PET) primarily with respect to lymph-node staging is currently being evaluated in ongoing studies. Due to the high sensitivity of the listed staging modalities in combination with rather low specifity, there is a general tendency towards “over staging”, which carries certain risk particularly for potentially operable patients. Consequently the criteria which indicate inoperability (T3, T4, N2, N3 and, in individual cases, M1) have to be confirmed histologically by biopsy employing interventional techniques or even by explorative thoracotomy before definite therapeutic decisions are made.
