Zusammenfassung Eine duodenumerhaltende Resektion des Pankreaskopfes bei Patienten mit chronischer Pankreatitis hat gegenüber der partiellen Duodeno-Pankreatektomie (Whipple-Op.) den Vorteil: Erhaltung der Duodenumpassage, Erhaltung der exkretorischen Funktion des linken Pankreas, Resektatbegrenzung auf den Pankreaskopf unter Schonung der Pankreasnachbarstrukturen. Bei 29 Patienten mit großem Tumor im Pankreaskopf wurde die duodenumerhaltende Pankreaskopfresektion ohne Klinikletalität angewandt. Die postoperative Krankenhausliegezeit war im Durchschnitt 22 Tage, Spätletalität 6,9%. Bei einer durchschnittlichen Nachbeobachtungszeit von 2,1 Jahren sind 82% der Patienten wieder vollberufstätig.

Summary The duodenum-preserving resection of the head of the pancreas in chronic pancreatitis includes the following advantages: preservation of the physiological duodenopassage, preservation of the excretory function of the left pancreas, and resection of only the head of the pancreas without damage to adjacent tissues. Of the 29 patients with a tumor of the head of the pancreas who underwent surgery using the duodenum-saving pancreas resection, the operative lethality was 0. The hospitalization time averaged 22 days, with a late lethality of 6.9%. When examined at an average of 2.1 years afterward, 82% of all patients were found to be completely resocialized.
