Summary A hundred uncomplicated cases of fatty metamorphosis of the liver were evaluated as to clinical history, clinical course, and histological findings. Included were 81 fatty livers and 19 fat cirrhoses. The fat occurred as large droplets and was either zonal or diffuse. Chronic alcoholism was the primary cause for the fatty change; diabetes mellitus followed. The major clinical sign of the fatty liver was hepatomegaly. Clinical laboratory tests in 63 of the 81 cases were pathological, most frequently the BSP clearance. Histologically, inflammatory changes and degenerative parenchymatous destruction were most commonly found; that is, ahepatitis of the fatty liver. Necroses of variable extent were observed in a third of the fatty livers. They appeared to lead to a destruction of the lobular architecture, a collapse of the reticulum network and to a pseudolobularumbau, according to their extent they were either acute or chronically recurrent. The cirrhosis following a fatty liver, therefore, is most likely apost-necrotic cirrhosis. The toxin causing the damage apparently is responsible for the fatty liverand the destruction of the parenchyma in the same way. The occurrence of a complicating disease (infection) is not required. The importance of the liver biopsy for the diagnosis of fatty liver, fatty liver hepatitis, and fat cirrhosis is emphasized.

Zusammenfassung Hundert unkomplizierte Fälle mit großtropfiger, zonaler oder diffuser Leberverfettung (81 Fettlebern und 19 Fettcirrhosen), wurden nach Anamnese, Klinik und histologischem Bild ausgewertet. Als Ursache der Verfettung stand chronischer Alkoholismus mit Abstand an erster Stelle, gefolgt vom Diabetes mellitus. Das führende klinische Symptom der Fettleber war die Hepatomegalie. Laboratoriumsproben ergaben in 63 von 81 Fällen einen pathologischen Befund, am häufigsten die Bromsulphaleinclearance. Histologisch fanden sich relativ häufig entzündlich-mesenchymale Veränderungen und degenerativ-nekrotisierende Parenchymschäden, also eineFettleberhepatitis. Nekrosen verschiedenen Ausmaßes waren in einem Drittel der Fettlebern zu beobachten. Sie scheinen zu Zerstörung des Läppchengefüges, Kollaps der Gitterfaserstrukturen und pseudolobulärem Umbau zu führen, je nach ihrer Ausdehnung akut oder schubweise-schleichend. Damit dürfte es sich auch bei der Cirrhose nach Fettlebern um einepostnekrotische Cirrhose handeln. Die schädigende Noxe ist offenbar für Fettleberund Parenchymschaden in gleicher Weise verantwortlich, das Hinzutreten einer komplizierenden Krankheit (Infektion usw.) ist nicht erforderlich. Die Wichtigkeit der Leberbiopsie für die Diagnose von Fettleber, Fettleberhepatitis und Fettcirrhose wird unterstrichen.
