    Description / Table of Contents: Zusammenfassung Das Auftreten von Hirnmetastasen stellt für Patienten ein eingreifendes und oft lebensbedrohendes Ereignis dar. Eine adäquate Therapie erfordert besonnenes Handeln unter Abwägung aller Behandlungsoptionen. Multiple Hirnmetastasen sollten insbesondere bei schlechtem Allgemeinzustand einer palliativen Ganzhirnbestrahlung zugeführt werden. Bei Patienten mit einem günstigen Prognoseprofil, solitären oder wenigen Hirnmetastasen sind operative Exstirpation und Radiochirurgie gleichwertige Behandlungsmethoden, wobei die stereotaktische Einzeitbestrahlung nicht invasiv ist und auch in inoperablen Regionen durchgeführt werden kann. Der Stellenwert der zusätzlichen Ganzhirnbestrahlung ist noch umstritten. Die Überlebenswahrscheinlichkeit wird durch diese Kombination nicht verbessert, wohl aber kann die zusätzliche Ganzhirnbestrahlung das Auftreten neuer Hirnmetastasen verhindern.
    Notes: Summary Brain metastases are life-threatening events for patients. Adequate therapeutic strategies demand thoughtful planning and careful weighting of all treatment options. Multiple brain metastases should be treated in palliative therapy with whole-brain irradiation. In patients with a favourable prognosis and solitary or few brain metastases, surgical excision and radiosurgery are comparable adequate treatment options. Radiosurgery is an non-invasive procedure that can also be performed in non-operable brain areas. The value of additional whole-brain irradiation is under discussion. It has no influence on survival, but the frequence of new brain metastases may be reduced.

    Description / Table of Contents: Zusammenfassung Zur Bestimmung des Stenosegrades wurden Dopplersonographie und intraoperative Druckmessung mit der Standard-II-Ebenen Angiographie verglichen. Gruppe 1 (Stenose kleiner als 80 %): Doppler/Angio 63% (positive Korrelation); Druckgradient/Angio 84%; Doppler/Druckgradient 77%; mittlerer Gradient 12 mm Hg. Gruppe 2 (Stenose groβer als 80 %) : Doppler/Angio 82 %; Gradient/Angio 72 %, Doppler/Gradient 64 %, mittlerer Gradient 25 mm Hg. Die funktionelle Signifikanz der Stenose nahm bei geringerem Kollateralfluß zu, kenntlich an der dopplersonographischen ≫Überschätzung≪ und an einem relativ großen Gradienten. Aus klinischer Sicht sind Einengungen größer als 80 % ≫kritisch≪ und erfordern sofortige chirurgische Therapie, da in 4 von 15 Patienten lokale Verschlüsse auftraten, welche gerade noch rechtzeitig operabel waren.
    Notes: Summary The results of indirect and direct Doppler examination and pressure gradient determination were compared with the bilateral angiography in 50 patients with internal carotid artery stenosis. Group 1 (stenosis below 80 % square area reduction) : the accuracy of Doppler vs angiography was 63 %, pressure gradient vs angiography 84%, Doppler vs pressure gradient 77%, mean gradient 12 mmHg. Group 2 (more than 80% square area reduction), Doppler vs angiography 82 %, pressure gradient vs angiography 72 %, Doppler vs pressure gradient 64 %. Mean pressure gradient 25 mm Hg. In conclusion, every lumen reduction of more than 80 % is critical and surgical therapy is urgent, since sudden local occlusion occurred in 4 of 15 patients.

    Notes: Summary Positron emission tomography (PET) with [18F]-2-fluoro-2-deoxy-D-glucose (FDG) was used to investigate the regional cerebral metabolic rate of glucose consumption (rCMRGlc) in two patients with benign hereditary chorea (BHC) and 21 normal subjects. Relative and absolute values of cerebellar, striatal, thalamic, and cortical rCMRGlc were within normal limits for both patients with BHC, indicating that the choreic movement disorder encountered in these two patients was not caused by a decrease of energy metabolism in the striatum such as that found regularly in most patients with other forms of chorea (e.g. Huntington's and Wilson's disease).

    Notes: Abstract Controversial data have been reported with regard to the diagnostic value of the positron emission tomographic (PET) measurement of striatal glucose consumption (rCMRGlc) in chorea-free subjects at risk of Huntington's disease (HD). For further clarification of this issue we measured striatal and cerebellar rCMRGlc in 27 chorea-free subjects at risk of HD, 20 patients with manifest HD and 20 control subjects, using PET and18F-fluorodeoxyglucose. In 6 of the at-risk subjects cerebellar ratios of striatal rCMRGlc were decreased below the corresponding 99% confidence limit determined in the controls. This indicates that the PET measurement of rCMRGlc may, indeed, be valuable in establishing the diagnosis of incipient HD in presymptomatic at-risk subjects.

    Notes: Summary Serial CT studies performed for 1 year on a female patient suffering from a pinealoma are reported. Under cobalt therapy (5000 rads) a remission of the pinealoma was achieved. A short time later, metastases developed and cobalt therapy was repeated (6000 rads). Rapid tumor remission was seen again, but rapid occurrence of probable metastases too. The question whether radionecrosis or metastases were present cannot be answered.

    Notes: Summary Computed tomography (CT) has given us a new method for examining the orbit and its contents. The technique of examination is described, and the indications for the application of computed tomography in ophthalmology are considered: suspected orbital tumors, unilateral exophthalmos, pareses of the ocular muscles, traumatic lesions and malformations in the region of the orbit. The findings in 210 cases of orbital lesions examined in this study are reported and the differential diagnosis is discussed. Considerable improvement and greater accuracy in diagnosis have been achieved in the field of ophthalmology by using computed tomography. Early tumor visualization is possible without risk or discomfort to the patient.

    Notes: Abstract A 39-year-old healthy man had several transient ischaemic attacks suggesting left internal carotid artery (ICA) occlusion. There were no vascular risk factors and no preceding trauma. Colour-coded duplex sonography suggested a pseudo-occlusion of the left ICA, and cerebral angiography demonstrated dissection of the left ICA and both vertebral arteries. Angiography 6 months later was completely normal. This underlines the importance of four vessel angiography in young patients with dissections of cervical arteries.

    Notes: Abstract We describe serial studies of focal cortical dysplasia causing temporal lobe seizures and progressive aphasia in a 54-year-old woman. Initially, MRI volumetry of the temporal lobes showed significant left cortical thickening corresponding to an elevated aminoacid uptake in the left temporoparietal and inferior frontal cortex on SPECT using 3-[123I]iodo-α-methyl-l-tyrosine (IMT). After 1 year there was severe shrinkage of the left temporal lobe, possibly the result of recurrent complex partial seizures.
