    Notes: Summary Eleven adults have been transplanted for various reasons between July 1979 and July 1982: 2 with aplastic anemia (AA), 1 with paroxysmal nocturnal hemoglobinuria (PNH), 8 with acute leukemia (AL). Four patients suffered from acute lymphocytic leukemia (ALL) and four from acute non-lymphocytic leukemia (ANLL). Two of them were transplanted in relapse, 1 in a partial remission, and 5 in complete remission. All patients were in their late stage of disease. The PNH-patient had an identical twin, 8 patients had an HLA- and MLC compatible sib, 1 an unrelated donor, and 1 was transplanted from his father. Four patients are alive, 2 more than 3 years: 1 with AA and 1 with ALL who was transplanted in relapse. Six patients died of infectious complications (4 of interstitial pneumonia, 1 of a candidasepsis, 1 of acute toxoplasmosis). Patients living more than 3 weeks had a take. Acute graft-versus-host (GvH) disease did not present a major problem. All patients received methotrexate for GvH-prophylaxis, in three instances the marrow was additionally pre-incubated with anti-T-cell globulin.

    Notes: Summary For years, brown tumors have been considered to be a characteristic of primary hyperparathyroidism. However, since 1963 several reports indicate the incidence of brown tumors in patients with renal secondary hyperparathyroidism to be 1.5%–1.7%. The appearance of multiple brown tumor lesions is rather uncommon in secondary hyperparathyroidism which is also true for malabsorption as its cause. We report on a 56-year-old man presenting with pain in the bones and multiple osteolyses. A bone biopsy specimen and the laboratory examinations were indicative of secondary hyperparathyroidism caused by malabsorption most likely due to Billroth's II/I gastric resection. Thus, the patient's osteolyses represent brown tumors which have been induced by nutritional secondary hyperparathyroidism.

    Notes: Abstract Objective: To evaluate the diagnostic value of lumbar puncture (LP) in surgical intensive care unit (SICU) patients. Design: Retrospective chart review. Setting: Surgical intensive care unit at a major tertiary care medical center. Patients: All patients admitted to the SICU during the period 1987–1995 who had a lumbar puncture, excluding those with a history of recent head trauma or a neurosurgical procedure. Measurements: Cerebral spinal fluid (CSF) results, indication for lumbar puncture, admitting service, length of ICU stay, presence of fever and use of antibiotics. Results: Of 7,555 admissions to the SICU over the period 1987–1995, 204 patients underwent LP during the hospital admission. Four charts could not be located and 75 patients underwent LP in locations other than the SICU. Of the remaining 125 cases, 55 carried a neurosurgical diagnosis and 70 had non-neurosurgical problems. Fifty-three (75 %) of the patients had received antibiotics prior to the procedure. Among the 70 cases without a primary neurosurgical diagnosis, LP was performed to rule out infective meningitis suggested by fever and mental status changes (30 patients), mental status changes alone (7 patients), systemic illness with fever and/or mental status changes (10 patients), seizures (10 patients), meningismus (+/− fever, 4 patients), or a combination of neurological findings (8 patients). There were no cases in which meningitis was positively diagnosed. Medical management was altered in only two patients based upon the results obtained by LP. Conclusions: We conclude that LP in SICU patients without a recent history of head trauma or a neurosurgical operation is a low yield procedure, especially for the diagnosis of bacterial meningitis.

    Description / Table of Contents: Summary Objectives: The effectiveness of acupuncture treatment in patients with osteoarthritis of the hip was tested. ¶Design: This is a prospective, randomized, controlled patient and examiner blinded clinical trial. ¶Patients and setting: The study was performed at a university department for physical medicine and rehabilitation. Sixty-seven patients were separated into two treatment groups. ¶Interventions: Group 1 (treatment) had traditional needle placement and manipulation, whereas in group 2 (control) needles were placed away from classic positions and not manipulated. In both groups needles were placed within the L2 to L5 dermatomes. Endpoints were pain (VAS), functional impairment (hip score), activity of daily living (ADL) and overall satisfaction before treatment, 2 weeks and 2 months post-intervention. ¶Results: For all endpoints there was a significant improvement in both groups 2 weeks and 2 months following treatment versus baseline, but no significant difference between the two treatment groups. ¶Conclusions: We conclude from these results that needle placement in the area of the affected hip by itself improves symptoms of osteoarthritis. It appears to be less important to follow the rules of traditional acupuncture techniques.
    Notes: Zusammenfassung Fragestellung: Die klinische Wirksamkeit der Akupunktur ist umstritten. Trotz zunehmender Akzeptanz bei westlichen Ärzten und Patienten mangelt es an kontrollierten Wirksamkeitsnachweisen. Die klinische Wirksamkeit einer gezielten Nadelakupunktur wurde am Beispiel der Koxarthrose untersucht. ¶Design: In einer prospektiven, randomisierten, placebokontrollierten, patienten- und untersucherblinden Studie wurde erstmals die Wirksamkeit einer gezielten Akupunktur bei Koxarthrose überprüft. Es wurden 67 Patienten in die Studie aufgenommen, auf zwei Behandlungsgruppen verteilt und in 3 Wochen 9-mal behandelt. ¶Intervention: Die Akupunkturbehandlung richtete sich nach den Empfehlungen der Traditionell Chinesischen Akupunktur. Als Kontrollbehandlung wurde eine modifizierte Minimal-Akupunktur n. Vincent verwendet. ¶Zielparameter: Schmerzintensität (VAS), Hüftfunktionsindex n. Lequesne, Activity of daily living, ADL n. Bullinger, over-all-assessment n. Carlsson, ROM und radiologische Einteilung n. Kellgreen. ¶Ergebnisse: Zwei Wochen sowie zwei Monate nach der Behandlung zeigte sich in beiden Behandlungsgruppen eine signifikante Besserung der Beschwerden (p〈0,0005). Die Schmerzreduktion betrug im Mittel 27%, der Hüftfunktionsindex besserte sich um 18% und die Lebensqualität um 5%. Zwischen den beiden Behandlungen waren keine signifikanten Wirkunterschiede nachweisbar. Insgesamt profitierten etwa 50% der Patienten sowohl von der Verum-Akupunktur als auch von der Kontroll-Akupunktur.¶Schlussfolgerung: Die Ergebnisse legen den Schluss nahe, dass für den schmerzdämpfenden Effekt einer Nadelakupuntur zur Behandlung der Koxarthrose die lokale Behandlung mit Akupunktur ausreichend ist, wobei für die Durchführung der Akupunktur keine speziellen Akupunkturkenntnisse der Chinesischen Medizin nötig sind.
