    Description / Table of Contents: Zusammenfassung In jüngster Zeit wird die lokale Applikation einer Streptokinase-Streptodornase (SK-SD) Lösung zur intraperitonealen Adhäsionsprophylaxe vorgeschlagen. An Ratte und Kaninchen wurden mögliche Nebenwirkungen vor allem auf die Wundheilung von Darmnähten und systemische Reaktionen untersucht. Weder mikromorphologisch oder rasterelektronenmikroskopisch, noch bezüglich mechanischer Parameter (Berstungsdruck, Reißkraft), war bei den verwendeten Dosen eine gestörte Wundheilung nachweisbar. Nach intraperitonealer Applikation wurde schon früh ein Anstieg der Körpertemperatur sowie eine Erhöhung des ASK-Titers beobachtet. Die SK-SD scheint die Ausbildung von Adhäsionen zu verringern.
    Notes: Summary At present the local application of a streptokinase-streptodornase (SK-SD) solution is proposed to prevent intraperitoneal adhesions. Possible side effects of such a therapy, particularly on wound-healing in the intestine and systemic reactions, were investigated experimentally in rats and rabbits. With the doses used, no impaired wound-healing was observed in micromorphological and SEM studies or when testing stress capacity (bursting and tensile strength). An initial rise in body temperature and an increasing ASK titer were observed. SK-SD seems to reduce the formation of intraperitoneal adhesions.

    Description / Table of Contents: Zusammenfassung Im Vergleich zum übrigen Gastrointestinaltrakt ist die Chirurgie am Colon mit einer unverhältnismäßig hohen Rate an Anastomoseninsuffizienzen behaftet. Die Wertigkeit verschiedener Techniken der zusätzlichen Gewebeklebung (Fibrinklebung mit Tissucol auch mit Kollagenvlies; Gentamycin-haltiges Kollagen-Fibrinogen-Lyophilisat) wurde am Rattencolon untersucht. In der postoperativen Frühphase (−24 h p.op.) erreichen die zusätzlich mit den Klebetechniken versorgten Nähte statistisch signifikant höhere mechanische Belastbarkeit. Bei den Kollagengeweben wird eine die Wundheilung offensichtlich nicht störende Fremdkörperreaktion beobachtet. Der Kollagen-Fibrinogen-Verbund bietet bei Aminoglycosidzusatz den Vorteil des lokalen Antibioticumschutzes.
    Notes: Summary Colorectal surgery is still burdened by a comparatively high rate of anastomotic leakages. The benefits of different fibrin gluing techniques (fibrin adhesive Tissucol, also with collagen tissue; gentamicinincorporated collagen-fibrinogen lyophilisate) were studied in the rat colon. In the early postoperative course (up to 24h p.o.), the sutures that were also sealed by the gluing techniques showed a statistically significant capacity to withstand more stress. In the collagen tissues, the foreign body reaction, has no side effects on wound healing. The gentamicin-collagen-fibrogen tissue has the advantage of local antibiotic protection.

    Notes: Summary The present study characterizes effects of histamine in the presence of the H2-antagonist cimetidine on the coronary circulation of the isolated perfused spontaneously beating guinea pig heart. Infusion of histamine (2 ⋅ 10−8 mol/1–5 ⋅ 10−6 mol/l) induced a dose-dependent coronary dilation, comparable to the effect of isoproterenol and two highly selective H2-agonistic compounds, impromidine and dimaprit. In the presence of cimetidine (5 ⋅ 10−6 mol/l), however, coronary response to histamine was reversed in a manner that a dose-dependent coronary constriction occurred with coronary spasm and a flow rate approaching zero at histamine concentrations above 8 ⋅ 10−7 mol/l, whereas the dilatory effect of impromidine and dimaprit was completely antagonized. In contrast, the histamine-induced constriction in the presence of cimetidine could be nearly abolished by additional infusion of the H1-antagonistic compound mepyramine (5 ⋅ 10−5 mol/l). Is is concluded that H1- and H2-receptors are present in the coronary smooth muscle, H1-receptors mediating constriction and H2-receptors mediating coronary dilation. Speculation is provided that histamine may have hazardous effects in anaphylactic states if cimetidine is administered simultaneously, e.g., to prevent or cure peptic ulcer. Other possible clinical implications will be discussed.

    Notes: Summary The present study characterizes myocardial effects of two new histaminergic H2-receptor specific compounds, impromidine, and dimaprit, on cardiac contractile and metabolic parameters of the guinea pig heart and human papillary muscle in comparison to the well-known effects of catecholamines. Impromidine and dimaprit produced a dose-dependent stimulation of the right and left ventricular contractile force in the isolated perfused biventricular catheterized guinea pig heart with maximal stimulation rates equal to those of isoproterenol. Hemodynamic equieffective doses of isoproterenol (2.8 × 10−9 mol/l), histamine (1.1 × 10−5 mol/l), impromidine (4.6 × 10−7 mol/l), and dimaprit (8.5 × 10−6 mol/l) induced nearly identical increases in tissue concentrations of c-AMP. All compounds dose-dependently enhanced the activity of the myocardial adenylate cyclase with very similar KA-values in a particulate sarcolemmal membrane preparation of both guinea pig ventricles and human papillary muscles. No effect of either compound was seen on cardiac phosphodiesterase activity. Selective administration of theβ 1-blocking agent metoprolol and the H2-receptor antagonist cimetidine clearly discriminates two independent receptors linked to the sarcolemmal adenylate cyclase system in the guinea pig and human myocardium. This is further supported by results obtained fromβ-receptor-binding studies in which an interference of impromidine and dimaprit with the stereospecific binding of (−)[3H]-dihydroalprenolol to cardiacβ-receptors could be definitely excluded. The possible therapeutic role of both H2-agonists on the non-ischemic, surviving myocardium, which is transiently refractory toβ-adrenergic stimulation by catecholamines after myocardial infarction, will be discussed.

Der Funktionszustand peripherer T-Lymphocyten und Monocyten wurde bei Patienten mit postoperativer Sepsis nach großen visceralchirurgischen Operationen anhand der Sekretion von Cytokinen sowie der Expression von HLA-Klasse-II Molekülen auf Monocyten untersucht. Hierzu wurden die Kulturüberstände stimulierter T-Lymphocyten und Monocyten mittels ELISA auf den Gehalt der Cytokine IL-2, IFNγ, IL-4, IL-10, TNFα, IL1β und IL-12 p40 analysiert. Die Antigen-präsentierende Funktion von Monocyten wurde anhand der HLA-DR und -DP Expression ermittelt. Die T-Zell-Sekretion von IL-2, TNF-α und partiell auch von IFNγ war bei Patienten mit letaler Sepsis gegenüber der Kontrollgruppe und Sepsisüberlebern signifikant erniedrigt, während die IL-4 Sekretion nicht alteriert war. Bei der Monocyten-Cytokinsynthese zeigte sich eine Suppression von IL-1β und IL-12 p40, deren Persistenz mit einem letalen Verlauf einherging. Die Suppression der Cytokinsekretion war bei T-Lymphocyten und Monocyten bereits zu Beginn der Sepsis vorhanden. Dies gilt auch für die HLA-Klasse II-Expression peripherer Monocyten. Die Ergebnisse zeigen, daß es in der postoperativen Sepsis zu einer initial einsetzenden Immunparalyse von T-Lymphocyten und Monocyten kommt. Die Erholung der Immunparameter und das Ausmaß der initialen Suppression sind mit dem Überleben der Patienten assoziiert.
    Notes: Abstract. In vitro functions of stimulated peripheral T cells and monocytes were investigated in patients experiencing sepsis following major visceral surgery. Cell culture supernatants were analyzed by ELISA for IL-2, IFN-γ, IL-4, IL-10, TNF-α, IL-1β, and IL-12 p40. In addition, monocyte HLA class II expression was determined by flow cytometry. T cell secretion of IL-2, TNF-α, and in part IFN-γ (but not IL-4) was significantly diminished in non-survivors throughout the entire course of sepsis, compared to controls and sepsis survivors. Production of IL-1β and IL-12 p40 by monocytes was strongly reduced in both survivors and non-survivors at the onset of sepsis. Persistence of depressed monocyte cytokine secretion correlated with lethality. Thus, overall suppression of cytokine production by T cells and monocytes was already observed at the beginning of postoperative sepsis. HLA class II expression by monocytes exhibited a strong and sustained down-regulation with no significant differences between sepsis survivors and non-survivors. In summary, suppression of both T cell and monocyte functions develops early during postoperative sepsis. Recovery of immune functions and severity of immune defects are associated with outcome.

    Description / Table of Contents: Zusammenfassung Standardisierte Enterotomien (5 cm oral des Coecums) wurden bei Ratten durchgeführt und durch einschichtige Einzelknopfnaht nach Lembert sowie durch zusätzliches Aufbringen von Fibrinkleber mit und ohne Fibrinolyseinhibitor versorgt. Die Darmabschnitte wurden histologisch, histochemisch und biomechanisch untersucht. Bei Anastomosen mit Fibrinkleber und Fibrinolyseinhibitor erhalten wir signifikant höhere Berstungsdrucke. Die verminderte Traumatisierung und die zusätzliche Aufbringung setzt die Insuffizienzgefahr herab.
    Notes: Summary In a series of experiments, enterotomies on 180 rats were closed by suture (Lembert) and by suture in combination with plasma fractions with and without fibrinolytic inhibitors. After fixed intervals, a section of the small colon was dissected and tensile strength was measured. In addition, micromorphologic and histochemical studies were performed. The tensile strength of an anastomosis adhered with fibrinogen and synthetic fibrinolytic inhibitor was significantly higher. In addition, traumatization is less, and due to the gas and fluro-dense zone, the rate of insufficiency can be reduced.
